diseases, NSD and PPR can all be prevented by well‐designed and delivered vacc<strong>in</strong>ation as a<br />

rout<strong>in</strong>e preventive measure. Fur<strong>the</strong>rmore, as pastoralists are currently pay<strong>in</strong>g for antibiotic<br />

treatment for <strong>the</strong>se diseases (with mixed outcomes) it seems likely that payment for more<br />

effective vacc<strong>in</strong>es is feasible. Yet <strong>the</strong> veter<strong>in</strong>arian establishment, over decades, has failed to<br />

develop preventive strategies for <strong>the</strong>se and o<strong>the</strong>r diseases. Basic epidemiological and<br />

economic assessments with pastoralists can clarify <strong>the</strong> demand for effective vacc<strong>in</strong>ation, and<br />

assist government to develop better strategies. If such strategies could be developed and<br />

implemented, especially with private sector <strong>in</strong>volvement, <strong>the</strong> need to use antibiotics <strong>in</strong><br />

response to outbreaks <strong>of</strong> viral diseases would be reduced. The role <strong>of</strong> vacc<strong>in</strong>es to reduce<br />

mortality <strong>in</strong> camel calves should be exam<strong>in</strong>ed.<br />

5. This assessment focused on livestock mortality and from a livelihoods perspective, <strong>the</strong><br />

sudden or gradual loss <strong>of</strong> livestock (f<strong>in</strong>ancial assets). This approach needs to be expanded to<br />

o<strong>the</strong>r VSF Suisse project areas to build a more comprehensive evidence‐base, while also<br />

assess<strong>in</strong>g broader impacts related to production losses. Fur<strong>the</strong>rmore, future assessments<br />

would benefit from assessment <strong>of</strong> equity issues, such as use <strong>of</strong> CAHWs by livestock keepers<br />

accord<strong>in</strong>g to wealth and gender.<br />

6. Government support to <strong>the</strong> CAH system seems to be confused, with <strong>the</strong> current CAHWs<br />

allowed to practice and submit reports, but no expansion permitted. Assum<strong>in</strong>g that local<br />

government is will<strong>in</strong>g, possible support to government would <strong>in</strong>clude assist<strong>in</strong>g <strong>the</strong>m to<br />

monitor <strong>the</strong> performance <strong>of</strong> private practitioners, <strong>in</strong>clud<strong>in</strong>g pharmacy staff and CAHWs, <strong>in</strong> a<br />

systematic manner. Similarly, <strong>the</strong> disease surveillance system might be streng<strong>the</strong>ned by<br />

develop<strong>in</strong>g clear objectives for <strong>the</strong> system and test<strong>in</strong>g it aga<strong>in</strong>st <strong>the</strong> ma<strong>in</strong> surveillance<br />

<strong>in</strong>dicators <strong>of</strong> sensitivity, specificity, timel<strong>in</strong>ess, representativeness, simplicity and<br />

acceptability. However, it should be noted that a surveillance system which uses CAHWs is<br />

unlikely to be susta<strong>in</strong>ed unless government can respond to disease outbreaks, or ideally,<br />

prevent outbreaks.<br />

7. USAID should review <strong>the</strong> use <strong>of</strong> <strong>the</strong>ir logo on private bus<strong>in</strong>esses and waive brand<strong>in</strong>g<br />

requirements <strong>in</strong> <strong>the</strong>se situations; similarly, VSF Suisse should review it’s own brand<strong>in</strong>g<br />

policy.<br />
