dan<strong>of</strong>loxac<strong>in</strong> to treat CCPP 21 . On <strong>the</strong>se issues <strong>the</strong> assessment team noted some misunderstand<strong>in</strong>gs <strong>in</strong><br />

CAHW tra<strong>in</strong><strong>in</strong>g courses delivered by veter<strong>in</strong>arians. For example, Tylos<strong>in</strong> is a drug <strong>of</strong> choice for use<br />

aga<strong>in</strong>st respiratory diseases such as CCPP and CBPP but has never been used by CAHWs aga<strong>in</strong>st<br />

<strong>the</strong>se diseases. Instead it was used aga<strong>in</strong>st viral diseases, particularly PPR, and with no impact (Table<br />

3). This experience <strong>in</strong>dicates that when veter<strong>in</strong>arians are confused about <strong>the</strong> evidence support<strong>in</strong>g a<br />

particular <strong>the</strong>rapeutic option, such confusion can easily be passed on to CAHWs.<br />

O<strong>the</strong>r technical areas <strong>in</strong> need <strong>of</strong> revision <strong>in</strong> terms <strong>of</strong> CAHW tra<strong>in</strong><strong>in</strong>g and supervision were as follows.<br />

• O<strong>the</strong>r than supportive treatment <strong>of</strong> NSD <strong>in</strong> small rum<strong>in</strong>ants, <strong>the</strong>re was limited evidence to show<br />

that <strong>the</strong> supportive use <strong>of</strong> antibiotics to treat viral diseases was effective ‐ antibiotic usage had no<br />

demonstrable impact on mortality caused by pox diseases, PPR or FMD.<br />

• The f<strong>in</strong>d<strong>in</strong>g that <strong>the</strong> parenteral adm<strong>in</strong>istration <strong>of</strong> oxytetracycl<strong>in</strong>e reduced mortality due to NSD<br />

agreed with experiences <strong>in</strong> nor<strong>the</strong>rn Somalia, where <strong>the</strong> same drug was reported to <strong>in</strong>crease<br />

survival <strong>in</strong> NSD outbreaks; this effect was attributed to prevention or treatment <strong>of</strong> secondary<br />

pasteurellosis or certa<strong>in</strong> tick‐borne protozoal diseases 22 . Fur<strong>the</strong>r action dur<strong>in</strong>g NSD outbreaks<br />

could <strong>in</strong>clude whole‐herd acaricide application to reduce transmission by <strong>the</strong> tick vector.<br />

• The CAHWs and pharmacies were sell<strong>in</strong>g various short act<strong>in</strong>g antibiotic drugs although <strong>the</strong><br />

correct adm<strong>in</strong>istration <strong>of</strong> <strong>the</strong>se drugs (e.g. daily treatment over three to five days) is problematic <strong>in</strong><br />

pastoral areas 23 ; <strong>in</strong>complete treatment can lead to treatment failures and discourage use <strong>of</strong><br />

CAHWs by pastoralists.<br />

• Diarrhoea and death, probably due to enterotoxaemia, was commonly reported as a cause <strong>of</strong><br />

camel calf mortality; this needs fur<strong>the</strong>r <strong>in</strong>vestigation and follow up.<br />

• CAHWs were tra<strong>in</strong>ed to adm<strong>in</strong>ister antibiotic tablets orally to young animals for diarrhoea, and<br />

use antibiotic boluses to treat reta<strong>in</strong>ed placenta <strong>in</strong> cattle; it seems that to some extent <strong>the</strong>se<br />

preparations are be<strong>in</strong>g adm<strong>in</strong>istered orally to adult rum<strong>in</strong>ants by pastoralists with related<br />

negative impacts.<br />

While some stakeholders may criticize CAHWs for some <strong>of</strong> <strong>the</strong>se weaknesses <strong>in</strong> cl<strong>in</strong>ical expertise,<br />

ultimately it is <strong>the</strong> responsibility <strong>of</strong> veter<strong>in</strong>arians to provide correct technical tra<strong>in</strong><strong>in</strong>g, monitor<br />

CAHW performance and address any problems as <strong>the</strong>y arise. O<strong>the</strong>r than occasional impact<br />

assessments (such as this report) <strong>the</strong>re appears to be no systematic or organized system <strong>in</strong> <strong>the</strong><br />

government veter<strong>in</strong>ary service to monitor CAHW activities or <strong>the</strong> cl<strong>in</strong>ical work <strong>of</strong> o<strong>the</strong>r private<br />

practitioners.<br />

Disease report<strong>in</strong>g and l<strong>in</strong>ks to government<br />

Unfortunately <strong>the</strong> assessment team was not able to meet <strong>the</strong> DVO as he was not available. Accord<strong>in</strong>g<br />

to <strong>the</strong> CAHWs <strong>in</strong> Takaba <strong>the</strong> district veter<strong>in</strong>ary unit is allow<strong>in</strong>g <strong>the</strong> exist<strong>in</strong>g CAH system to cont<strong>in</strong>ue,<br />

but opposes fur<strong>the</strong>r expansion <strong>of</strong> <strong>the</strong> service e.g. by refus<strong>in</strong>g to tra<strong>in</strong> new CAHWs. At <strong>the</strong> same time<br />

<strong>the</strong>re appears to be no feasible alternative to CAHWs and <strong>the</strong>refore, <strong>the</strong> government strategy would<br />

leave many areas without CAHWs and allow <strong>in</strong>formal drug sales by people with no tra<strong>in</strong><strong>in</strong>g at all. As<br />

<strong>the</strong> results <strong>of</strong> this assessment shows (Tables 2 to 5), <strong>the</strong> use <strong>of</strong> veter<strong>in</strong>ary drugs by untra<strong>in</strong>ed people<br />

has limited impact relative to CAHWs.<br />
