With<strong>in</strong> time and resource limits, <strong>the</strong> <strong>in</strong>tention was to conduct a detailed assessment <strong>in</strong> one area and<br />

collect systematic data on case fatality, ra<strong>the</strong>r than opt for a more superficial assessment across more<br />

districts. With<strong>in</strong> <strong>the</strong> district <strong>the</strong> Takaba assessment site was suggested by VSF staff because CAHWs<br />

were l<strong>in</strong>ked to veter<strong>in</strong>ary pharmacies for drug replenishment, and also, to <strong>the</strong> District Veter<strong>in</strong>ary<br />

Officer (DVO) for disease report<strong>in</strong>g. The Takaba project site was located far<strong>the</strong>r from ei<strong>the</strong>r <strong>the</strong><br />

Mandera or Wajir VSF Suisse ma<strong>in</strong> field <strong>of</strong>fices than any <strong>of</strong> <strong>the</strong> o<strong>the</strong>r sites where <strong>the</strong> project was<br />

implemented. Therefore, <strong>the</strong> Takaba site was considered an appropriate site to draw key lessons on<br />

issues related to <strong>the</strong> susta<strong>in</strong>ability <strong>of</strong> a CAH system <strong>in</strong> remote pastoral areas.<br />

Four villages were randomly selected <strong>in</strong> <strong>the</strong> district viz. Geder, Kalwahil, Wargud and Wagadahan.<br />

• In each village 25 key <strong>in</strong>formants were <strong>in</strong>terviewed (total 100 <strong>in</strong>formants); <strong>in</strong> both Geder and<br />

Wargudud <strong>the</strong>se <strong>in</strong>terviews were group <strong>in</strong>terviews with <strong>the</strong> 25 <strong>in</strong>formants, whereas <strong>in</strong> <strong>the</strong> o<strong>the</strong>r<br />

villages <strong>the</strong>y were <strong>in</strong>dividual <strong>in</strong>terviews.<br />

• A total <strong>of</strong> six CAHWs were <strong>in</strong>terviewed, selected from all four villages.<br />

• Proportional pil<strong>in</strong>g was conducted with <strong>the</strong> same 100 <strong>in</strong>dividual <strong>in</strong>formants selected from <strong>the</strong> four<br />

villages; <strong>the</strong>se <strong>in</strong>formants represented 52 cattle herds, 77 camel herds and 94 small rum<strong>in</strong>ant<br />

flocks.<br />

3. Results<br />

3.1 F<strong>in</strong>ancial performance <strong>of</strong> pharmacies<br />

VSF Suisse first established a CAH system <strong>in</strong> nor<strong>the</strong>ast Kenya <strong>in</strong> 2002, us<strong>in</strong>g funds from ECHO. The<br />

CAHWs were equipped with a kit <strong>of</strong> veter<strong>in</strong>ary drugs and equipment valued at Ksh 40,000 (US$556)<br />

<strong>in</strong> two rounds. A timel<strong>in</strong>e <strong>of</strong> events for <strong>the</strong> overall CAH system, <strong>in</strong>clud<strong>in</strong>g support provided by <strong>the</strong><br />

VSF Suisse/ELMT project is presented <strong>in</strong> Figure 2.<br />

2002 • VSF Suisse tra<strong>in</strong>ed 60 CAHWs <strong>in</strong> <strong>the</strong> greater Mandera district.<br />

• Follow<strong>in</strong>g <strong>the</strong> tra<strong>in</strong><strong>in</strong>g each CAHW was given a kit <strong>of</strong> veter<strong>in</strong>ary drugs and equipment <strong>of</strong><br />

value Ksh 20,000; CAHWs were given more than eight refresher tra<strong>in</strong><strong>in</strong>gs between 2002<br />

and 2006.<br />

2006 • VSF Suisse provided a veter<strong>in</strong>ary stock <strong>of</strong> value Ksh 20,000 per CAHW and also<br />

established a veter<strong>in</strong>ary drug store belong<strong>in</strong>g to a group <strong>of</strong> CAHWs <strong>in</strong> Mandera town, as a<br />

f<strong>in</strong>al means <strong>of</strong> support to <strong>the</strong> CAH system 1 .<br />

2007 • VSF Suisse assessed <strong>the</strong> status <strong>of</strong> <strong>the</strong> CAH system and learned that <strong>the</strong> system had failed to<br />

operate as <strong>in</strong>tended, ma<strong>in</strong>ly due to mismanagement <strong>of</strong> <strong>the</strong> funds provided.<br />

2008 • The VSF Suisse/ ELMT project started and revitalized <strong>the</strong> earlier CAH system established<br />

<strong>in</strong> 2002.<br />

• The new project started a disease outbreak report<strong>in</strong>g system <strong>in</strong> collaboration with <strong>the</strong><br />

district veter<strong>in</strong>ary unit, us<strong>in</strong>g CAHWs and private practitioners.<br />

August 2009<br />

• VSF Suisse assessed <strong>the</strong> impact <strong>of</strong> <strong>the</strong> CAH system supported by ELMT project <strong>in</strong><br />

collaboration with PACAPS/Tufts University (this report).<br />

The CAHWs who became private drug suppliers were tw<strong>in</strong>ned with veter<strong>in</strong>ary pr<strong>of</strong>essionals at that time<br />

ma<strong>in</strong>ly for <strong>the</strong> purpose <strong>of</strong> licens<strong>in</strong>g <strong>the</strong>ir veter<strong>in</strong>ary pharmacies.<br />

The new VSF Suisse/ELMT project which started <strong>in</strong> May 2008 aimed to ensure <strong>the</strong> susta<strong>in</strong>ability <strong>of</strong> <strong>the</strong><br />

earlier system. The new project <strong>in</strong>cluded <strong>the</strong> follow<strong>in</strong>g support.<br />
