We have reviewed the proposed substitution and note the following:
1. Dimensions ( will / will not ) be affected (as shown on the attached drawings).
2. The manufacturer's warranty for the proposed substitution ( is the same as / differs from) the specified
product (as described in the attached literature).
3. The Contract Time will be ( unchanged / increased by days / decreased by days) if the
substitution is accepted ( by ).

5. Maintenance, service and spare parts for the proposed substitution are available from:

6. If the proposed substitution is accepted, we will execute a Change Order ( increasing / decreasing) the

Agreement.
By this submission we certify that:
1. We have reviewed, and agree to, the provisions of section 01600 of the specifications.
2. The proposed substitution has been fully reviewed and determined to be equal or superior to the product
specified.
3. The cost data stated above is complete. Claims for additional costs related to the substitution which may
subsequently become apparent will be waived.
4. Dimensions and clearances for all affected work will not change other than as noted above.
5. Coordination, installation, and changes in the work as necessary for accepted substitution will be complete
in all respects.
6. If the product is accepted for substitution and installed, the Contractor hereby agrees that should the
product fail within one year of Owner's occupancy of the building, the substituted product will be removed
and replaced with the product originally specified, at no cost to the owner.
