Chapter 1 – Introduction<br />
About <strong>Xeris</strong><br />
What is <strong>Xeris</strong>?<br />
<strong>Xeris</strong>, derived from the Greek word meaning “to know”, is an internet-enabled suite of tools developed by<br />
<strong>Hospice</strong> <strong>Pharmacia</strong> (HP), a division of excelleRx, to assist hospice partners with medication management.<br />
<strong>Hospice</strong> administrators, nurses and other staff consider <strong>Xeris</strong> on-line tools an efficient way of keeping secure,<br />
accurate, comprehensive patient information in accordance with the Health Insurance Portability and<br />
Accountability Act (HIPAA). For more information about the various <strong>Xeris</strong> modules, please visit HP’s website at<br />
http://www.hospicepharmacia.com/about<strong>Xeris</strong>.asp.<br />
<strong>Xeris</strong> Version 2.1<br />
The newest release of <strong>Xeris</strong> has been specifically designed to assist nurses, hospice administrators and<br />
hospice support staff in the process of managing pharmacotherapy for a hospice patient. Using this web portal,<br />
an authorized hospice employee can easily view information about a patient, manage patient census, view<br />
medication lists, request refills, track shipments, locate a network pharmacy and perform a number of other<br />
activities.<br />
Features which are being introduced in Version 2.1 will be flagged with “ New in version 2.1”<br />

How to connect to <strong>Xeris</strong><br />
System Requirements<br />
In order to use <strong>Xeris</strong>, your computer must have the following minimum requirements (higher settings are also<br />
acceptable):<br />
Internet access using either Microsoft Internet Explorer 5.0 or Netscape Navigator 6.0<br />
A color monitor with 800x600 pixel screen resolution<br />
56K modem<br />

HP’s website is accessed by pointing your internet browser to http://www.hospicepharmacia.com. This site<br />
provides much useful information about HP and allows you to login to <strong>Xeris</strong>.<br />
You may want to consider making this site one of your favorites and creating a desktop shortcut to allow quick<br />
access when returning.<br />

Logging In<br />
In the upper left of the screen, you will see two boxes under the <strong>Xeris</strong> Login heading. Type your Login ID<br />
(UserID) and your Password into the text boxes provided, then click the Log In button. You will enter the<br />
secure portion of our website and receive connection to your records.<br />

The first time you log in to <strong>Xeris</strong>, you will be asked to read and accept the <strong>Xeris</strong> End User License Agreement.<br />
Scroll through the text to read the entire agreement, and then click the Accept button.<br />

Password<br />
At your first log in to <strong>Xeris</strong>, or if your password has been reset, you will be asked to create a new password.<br />
Type in your newly selected password two times, following the rules listed on the screen. Enter the last 4 digits<br />
of your SSN to be used as a unique identifier. Click the Update hyperlink to save your new password.<br />

You will receive a screen indicating that your new password has been updated and will be effective for 45 days.<br />
Click the Go to <strong>Xeris</strong> hyperlink to begin using <strong>Xeris</strong>.<br />

If your hospice has elected to use PINs you will need to establish a personal PIN number. Your PIN is used to<br />
verify your identification when you call into <strong>Hospice</strong> <strong>Pharmacia</strong>.<br />
Logging Out<br />
When you have completed your <strong>Xeris</strong> activities, please be sure to click the LogOut hyperlink in the upper right<br />
of the screen to close out your session and prevent unauthorized access.<br />

If you forget to log out, or if you have an open session with no activity for twenty (20) minutes, you will<br />
automatically be logged out by <strong>Xeris</strong>.<br />

Problems<br />
If you forget your Login ID and/or password, contact your <strong>Xeris</strong> <strong>Hospice</strong> Administrator who will be able to reset<br />
your password. If you have continued problems, you may contact us via email at <strong>Xeris</strong>pw@excelleRx.com or<br />
by calling <strong>Xeris</strong> Administration Support at (215) 282-1651.<br />
Security<br />
<strong>Xeris</strong> utilizes 128-bit Secure Socket Layer (SSL) technology to insure that all information about your patients<br />
remains secure. Secure screens are indicated by the presence of the lock icon in the lower right of your internet<br />
session window.<br />

Note: Please be aware of HIPAA patient privacy regulations when displaying patient information on<br />
your computer screens. Do not view sensitive information in a public area and do not leave<br />
open web sessions unattended.<br />

The <strong>Xeris</strong> home page, toolbar and icons<br />
Welcome Page<br />
The Welcome page is displayed after a successful login. The top of the page shows your name and <strong>Xeris</strong> role.<br />
Your role, selected by your <strong>Xeris</strong> <strong>Hospice</strong> Administrator, will determine which functions you can access within<br />
<strong>Xeris</strong>. Some of the functions described in this manual may not be available to you in your assigned role.<br />

Toolbar<br />
The left side of the Welcome page is called the Toolbar. Each icon shown will provide quick navigation to other<br />
sections of <strong>Xeris</strong>. Refer to the Navigation Icons section in this chapter for more detail.<br />
Hold your mouse over an icon to get a pop-up description; known as a Tool Tip, of the icon’s purpose. Click the<br />
double arrows icon<br />

Patient Hot List<br />
The Hot List gives you a quick snapshot of your patients whose last recorded symptom assessment scores<br />
were greater than six (6). Click on the different symptoms view a list of patients with scores greater than six (6)<br />
for those assessments.<br />

An alternate view of your patients’ scores can be obtained by clicking the View Charts hyperlink for any of the<br />
symptoms. You will receive a pie chart of all your patients’ recorded scores (from 1 to 10) and a count for each<br />
score. Click the View Hot List hyperlink to return to the list view.<br />
Dashboard<br />
The Dashboard gives you a quick snapshot of key demographics for your hospice.<br />

Navigation icons are used to jump to another part of <strong>Xeris</strong> or open a new internet browser window to perform an<br />
additional function without losing your place in <strong>Xeris</strong>.<br />

Find Patients –a list of recently viewed patients, search for patients, add a new patient<br />

Prescription Medication Icons<br />
Prescription Medication Icons are located next to an ordered medication and are used to indicate where that<br />
medication lies within the Medication Use Guidelines.<br />
MUG – Medications that are listed in the MUG’s and are included in the hospice’s per diem.<br />
Non-MUG – Medications not listed in the MUGs. These meds are billed outside the per diem to the<br />
hospice.<br />
MUG-R – Medications that are listed in the MUGs, but are not included in the per diem for this<br />
particular patient. These meds are billed outside the per diem to the hospice.<br />
Oncology – Medications not listed in the MUGs. These meds are billed outside the per diem to the<br />
hospice.<br />

Managing demographic and location information about your patients<br />
Things to Remember<br />
Timely and accurate entry of admissions, discharges, deaths and location changes are essential to ensuring<br />
that your patients receive the appropriate level of service and that per diem billing accounts are correct. Key<br />
points to remember include:<br />
• New patient status – referral vs. active<br />
Entry of referral patients supports a speedy admission process. When a referral patient is admitted,<br />
only a verification of existing data and a quick status change are needed. Patients in referral status will<br />
not appear on per diem billing reports, nor will they have activated HPRxCards.<br />
Note – HP executes periodic purges of referral patients who have been in the <strong>Xeris</strong> database for 15<br />
days or more and have not been admitted.<br />
Patients entered as active or changed from referral to active will activate the per diem billing process<br />
and the HPRxCard.<br />
• Organization –group/team<br />
When entering a new patient or when transferring a patient between home care and a facility, it is<br />
important to select the appropriate group/team name. The codes in the pull-down box will determine a<br />
patient categorization (e.g., if HP services your LTC and/or IPU patients, you will see group/team codes<br />
with an LTC or IPU suffix, no suffix indicates homecare).<br />
Your choices may also represent different teams within your hospice (e.g. North or South, Green or<br />
Blue). If you are unsure of how to interpret your selections, please contact your <strong>Xeris</strong> <strong>Hospice</strong><br />
Administrator for an explanation.<br />
Search for a Patient<br />
To find a patient, click the Find Patient icon in the toolbar. Enter your search criteria in one of the text boxes<br />
(described below) and click the Search hyperlink.<br />
• Name – hints for entering a name are shown by holding the mouse over the light bulb icon .<br />
Note – If you are having difficulty locating a patient, try typing fewer characters of the name to allow<br />
for spelling variations<br />
• Social – enter the 9-digit social security number of the patient (hyphens optional)<br />
• MR – Medical Record number (optional—only used for hospices who store this number in <strong>Xeris</strong>)<br />
• Phone – enter the 10-digit phone number of the patient (hyphens, parentheses and spaces optional)<br />

You will receive an alphabetized list of active patients (shown in green), referral patients (shown in tan) and<br />
discharged patients (shown in blue) who are associated with your hospice and group/team and who match your<br />
search criteria.<br />

If you wish to expand the search to include deceased patients, click the deceased check box, then click the<br />
Search hyperlink again. The deceased patients will be included in the results list (shown in red).<br />
If the list cannot be displayed on a single page, [Back] and [Next] hyperlinks will be provided to skip to other<br />
pages.<br />
To open the profile for a patient in the list, click the Select icon<br />
found, the patient’s profile will be automatically opened.<br />

Adding Patients<br />
To add a new active or referral patient, click the Find Patient icon<br />

in the toolbar, then click the Add New<br />
Note - Search the <strong>Xeris</strong> system prior to adding a new patient to see if the patient already exists in<br />
<strong>Xeris</strong> as an active or referral patient previously entered by another person in your hospice,<br />
by an HP representative or by an electronic <strong>Xeris</strong>Connect interface from your hospice<br />
management system. This will save not only your data entry time, but will avoid costly<br />
errors from duplicate patient records.<br />

On the Add/Edit Patient Information screen, fill in the text boxes.<br />
Note – Required fields are marked with a red asterisk (*)<br />

The Name should be the patient’s full name. Separate with a comma if you type the last name first.<br />

The Status pull-down list lets you select whether the patient is being entered as a Referral, to be admitted at<br />
a future date, or if the patient is Active, admitted to hospice.<br />
The Organization Name will default to your hospice name. If you have privileges for multiple organizations,<br />
you must select a name from a pull-down list.<br />
The Organization Group/Team pull-down list lets you select a patient category and/or team assignment.<br />
Refer to the Things to Remember section in this chapter.<br />
The Admit Date pull-down appears after selecting a patient status of active. <strong>Xeris</strong> will default the admit date<br />
to the current date or you may select a date within a 10-day range surrounding the current date. If you<br />
need to admit a patient outside the date range, you must contact a HP representative to perform the<br />
admission.<br />

The Primary/Attending Physician is required for active patients. A pull-down list of physicians will be<br />
supplied. If the physician you need to associate with the patient is not in the list, refer to the Physician and<br />
Nurse assignments section in this chapter.<br />
There are additional physicians that can be added to the patient to help clarify which doctor performs which<br />
task. These are optional fields and can be entered similar to the Primary/Attending physician.<br />
The Address (Street, City, State and Zip Code) must be entered accurately to ensure the best possible<br />
delivery of prescriptions using HP’s national courier service. A second, optional line for the Street address<br />
can be used.<br />
Note: If you are unsure of the patient’s zip code, you may type the city and state, and then click the<br />
zip code icon<br />
to obtain a display of zip code(s) from the USPS.<br />
Note: If you have chosen a Group/Team assignment ending with an "LTC" suffix, a drop<br />
down list of available facilities for your hospice will appear. After choosing the<br />
patient's correct facility, the address, phone number, and pharmacy fields will be<br />
automatically populated with the information that HP has on file. If the facility is not<br />
available in the drop down list or the pharmacy is incorrect, please notify an HP<br />
technician immediately with the correct information.<br />
The Phone is required. As some patients do not have telephone service, we do recommend that you enter<br />
the patient’s phone number to assist with customer service issues, delivery questions and patient searches.<br />
The telephone number may be entered without parentheses, hyphens or spaces and <strong>Xeris</strong> will format the<br />
number.<br />
The SSN is required to uniquely identify your patient for billing purposes. The social security number may<br />
be entered without hyphens or spaces and <strong>Xeris</strong> will format the number.<br />
The Birth Date may be entered without slashes and <strong>Xeris</strong> will format the number.<br />
The Gender can be selected from the pull-down list.<br />
The Race is required to assist HP’s Predictive Pharmacotherapy Outcomes System (PPOS). Studies<br />
have shown that different ethnicities have different metabolic reactions to some medications and knowing<br />
your patient’s race can assist us in providing the appropriate medication to your patient.<br />
The Complementary Therapy boxes can be checked if you wish to record this information.<br />
The Comments box allows you to share information about your patient with HP and other <strong>Xeris</strong> users.<br />
When you have completed filling in the form, click the Save hyperlink to create a patient record in <strong>Xeris</strong>. You<br />
will receive a confirmation screen. If information is incomplete, <strong>Xeris</strong> will return you to the form and show you<br />
which information is incorrect or missing.<br />

Physician Assignment<br />
To chagne the physician assigned to a patient, select the pencil icon in the upper left corner ( - edit patient<br />
information). Then select the Change link. If you are assigning a physician to the patient for the first time, start<br />
with the Change link step.<br />

Once again you must select Change link.<br />

Type in the physicians name and search. If you do not find the desired physician, select the Search All Doctors<br />
link.<br />

If you still do not find the desired physician, select the Add New Link.<br />

Type the desired physician name as Lastname, Firstname or Lastname, FirstInitial. Select the state or type in<br />
the zipcode, if known. Select the search link.<br />

Select the arrow next to the appropriate physician, add the correct phone number and fax if available, and select<br />
save. This physician is now available to add to the patient demographics.<br />

Nurse assignment<br />
After creating a new patient record, you will have the option to select a primary nurse for the patient. Select the<br />
name of the patient’s primary nurse from the pull-down list and click the Save hyperlink. If Nurse’s name does<br />
not appear in drop down list, select “”, type in nurses name, then click Save.<br />

Modify Patient Information<br />
To modify demographic information about your patient including physician or nurse assignment, open the<br />
patient’s profile and click the pencil icon to the left of patient’s name. Type your changes and click the Save<br />
hyperlink.<br />
Status Changes<br />
The status change icon<br />
HP service, for example:<br />
to the left of the patient’s name in the profile screen is used to change the status of<br />
Referral to Active<br />
Active to Deceased<br />
Active to Discharged (either discharge from hospice or to a facility not serviced by HP)<br />
Discharged to Active (re-admission)<br />
Click the status change icon, you’ll be asked to select an effective date, the per diem billing and the HPRxCard<br />
will be activated or terminated according to the patient’s new status. Click the Update hyperlink to save the new<br />

Group/team Changes<br />
The transfer icon to the left of the patient’s name in the profile screen is used to change the group/team<br />
assignment from one HP service to another, for example:<br />
Red team to Blue team<br />
Home care to LTC or IPU facility serviced by HP<br />
LTC or IPU facility serviced by HP to home care<br />
Note: If your patient is being transferred to an LTC facility serviced by HP, a drop down list of<br />
available facilities for your hospice will appear. After choosing the patient's correct<br />
facility, the address, phone number, and pharmacy fields will be automatically<br />
populated with the information that HP has on file. If the facility is not available in<br />
the drop down list or the pharmacy is incorrect, please notify an HP technician<br />
immediately with the correct information.<br />
You will need to enter the name of the person authorizing the transfer, and then click the Update hyperlink to<br />
save the new group/team assignment. This change will automatically update the per diem billing accounts for<br />
that patient.<br />

search by term, type all or part of the medical condition name into the text box, select whether you want to<br />

You will receive a list of one or more ICD-9 codes for that description. Click the Select hyperlink for the code<br />

In the final step you must verify that this is the patient’s primary hospice diagnosis. Selecting this box creates<br />
the patient’s coverage code category (CAF, HRT, LUNG, OTHR) utilized in the MUGs.<br />
The onset date is not needed for new patients, but becomes more important if your patient develops new<br />
symptoms while on service. Click the date in the calendar to select an onset date.<br />
Click the Update hyperlink to save the patient’s diagnosis information.<br />
After entering the primary diagnosis, additional diagnosis codes should be added to the patient’s profile to reflect<br />
secondary diagnoses or concomitant disease states.<br />

The recent requests list shows new prescriptions or refill requests from the last thirty (30) days. Click on the<br />
glasses icon to the right of the status to view the shipping information. Click on the plus icon to the left of<br />
the request number to view a list of prescriptions in the request. Click on the minus icon to hide the list. Click<br />
on the glasses icon<br />
to the left of the Rx# to view details about the prescription.<br />

Active Medications<br />
A list of active medications is shown when a patient’s profile is opened. If the display for a patient has been<br />
changed to the InActive View or Discontinued View (current view indicated by the darker green bar), click the<br />
bar to return to the Active View. The Active Medication View represents a substantial<br />
improvement to the presentation of medications for hospice use. The new Active Medication view in <strong>Xeris</strong> 2.1<br />
and the corresponding Active Medication Reports give a simplified, but complete presentation of the patient's<br />
active medications. This is accomplished by combining Dispensed and Profiled medications and by eliminating<br />
redundant medications. Consequently, each Active Medication is presented only once, even if there have been<br />
multiple prescriptions for that medication. Furthermore, each medication is moved to the Inactive Medication<br />
section after its expiration or stop date.<br />
The list shows all active medications for the patient, grouped by diagnosis or indication with the corresponding<br />
ICD-9 code.<br />
MUG – Medications that are listed in the MUG’s and are included in the hospice’s per diem.<br />
Non-MUG – Medications not listed in the MUGs. These meds are billed outside the per diem to the<br />
hospice.<br />
MUG-R – Medications that are listed in the MUGs, but are not included in the per diem for this<br />
particular patient. These meds are billed outside the per diem to the hospice.<br />
Oncology – Medications not listed in the MUGs. These meds are billed outside the per diem to the<br />
hospice.<br />

To profile a new medication for the patient, click the Add New hyperlink.<br />
Note – Entry of a profiled medication tells HP that the patient is known to be taking a given<br />
medication. A prescription can be created for your patient during a Pharmaceutical Care<br />
encounter.<br />
Enter all or part of a medication name (either brand or generic); select whether you want to search for a<br />
medication that starts with your text or contains your text and click the Search hyperlink.<br />

Requesting Refills<br />
Medications eligible for refill have a check box next to the medication name in the Active Medications view.<br />
Check one or more boxes and click the Request Refills hyperlink in the upper right corner of the medications<br />
list. A new request will be placed at the top of your Recent Requests list. Click the glasses icon to view<br />
more detail about the prescription in the request. Click the X icon to cancel a single medication from the<br />
request. Click the Submit Request hyperlink to submit the request to HP for processing.<br />

Specify Coverage<br />
You may mark which medications are related or not related to the patient’s terminal diagnosis by clicking the<br />
Specify Coverage hyperlink in the Active Medications list. Each medication will be shown with a pull-down list<br />
to designate whether you wish to categorize the medication as Related or Not Related. You may leave any<br />
medication as Not Specified. Click the Save hyperlink to record your changes which will be displayed on the<br />
Active Medications list and report.<br />
Note – Use of this optional function is for hospice categorization of medications only. This is<br />
separate and distinct from MUG or non-MUG classifications and has no bearing on HP<br />
billing.<br />
Inactive Medications<br />
A list of inactive medications is shown when a patient’s profile is opened and the<br />
bar is<br />
clicked. The list shows all inactive medications for the patient, grouped by diagnosis or indication with the<br />
corresponding ICD-9. Click the glasses icon to view more detail about the prescription history.<br />
Discontinued Medications<br />
A list of discontinued medications is shown when a patient’s profile is opened and the<br />
bar is clicked. The list shows all discontinued medications for the patient, grouped<br />

Opioid Conversion Calculator<br />
Click the calculator icon in the <strong>Xeris</strong> toolbar to launch a new window with the Opioid Conversion<br />
Calculator. In the Convert From section, select a route, medication, dosage and # of times taken in 24 hours<br />
from the pull-down lists. Click the Add Additional Meds hyperlink if you wish to convert the patient from<br />
multiple medications to a single new opioid. In the Convert To section, select a route, medication and frequency,<br />
then click the Submit hyperlink. You will receive directions for the medication you are looking to convert to for<br />
the patient. When you are through with the calculator, click the CLOSE hyperlink; your <strong>Xeris</strong> session will still be<br />

Find a Pharmacy<br />

Using <strong>Xeris</strong> Reports<br />
Although each <strong>Xeris</strong> report is unique, there are common functions used in developing your selection criteria and<br />
manipulating the results.<br />
Many <strong>Xeris</strong> reports require you to choose a date or range of dates as your selection criteria. A calendar (or a<br />
pair of calendars representing To and From dates) is displayed with the current date highlighted. You may click<br />
on a different date within the month or click a month’s name in the upper left and right corners to select a date<br />
from another month.<br />

security purposes, your current PIN will be displayed as X’s).<br />
If you do not currently have a PIN, or if you have had your PIN reset by you XHA, leave the Old PIN<br />
text box empty; otherwise enter your existing PIN in the Old PIN text box. Enter your new PIN in the<br />
New PIN and Confirm boxes and click Save (Note: your PIN must be unique in your hospice<br />
organization; if <strong>Xeris</strong> detects duplicate you will be prompted to select a new PIN).<br />

• To reset a user’s forgotten passcode, click the Reset User Account hyperlink. A one-timeonly<br />
password will be displayed and the user must establish a new password upon next<br />
successful login.<br />
• To deactivate a user’s account, click the Inactivate User Account hyperlink . It is important to<br />
deactivate account for hospice personnel who have left the organization.<br />
• To reset a users forgotten PIN, click Reset User PIN. The next time that user logs in they may<br />
establish a new PIN without having to enter their old PIN.<br />
• To modify a user’s name or email address, click Edit in the User Demographics section.<br />

Creating New User Accounts<br />
To avoid creating duplicate <strong>Xeris</strong> accounts for a single user, it is recommended that you user the<br />
Search function to verify that the user does not already have a <strong>Xeris</strong> account prior to creating a new<br />
one.<br />
Click the Add New User(s) hyperlink to create a new <strong>Xeris</strong> passcode.<br />
• Type the user’s full name as either “First Last” separated by a space or “Last, First”<br />
separated by a comma. Do not use special characters (e.g. *-,’…etc.) and do not<br />
enter titles or middle initials.<br />
• Enter the user’s email address.<br />
• Select a User Role from the pull-down box:<br />

• Clinical users have full <strong>Xeris</strong> privileges to maintain patient demographic, census,<br />
profile and clinical information, including requesting refills.<br />
• Census users have privileges to maintain patient demographic, census and<br />
profile information, but are not able to request refills using <strong>Xeris</strong> via the internet or<br />
the telephone system.<br />
Note: Both Clinical and Census users can have access to multiple organizations and may<br />
be assigned any, all or none of the Additional or Reporting options described below on an<br />
individual basis.<br />

If you have responsibility for multiple organizations, you may choose to assign a new user to one or<br />
more organizations. Click on an organization or hold down the Ctrl key to highlight more than one<br />
organization.<br />
Note: A user’s PIN must be unique within all offices to which they are assigned. When expanding an<br />
existing user’s privileges to include additional office(s), <strong>Xeris</strong> checks that user’s PIN within all offices<br />
and alerts the XHA of any duplication. The XHA must then reset that user’s PIN and the user will need<br />
to create a new PIN during their next <strong>Xeris</strong> session.<br />

To assign Additional <strong>Xeris</strong> User Options, check one or more of the checkboxes:<br />
• Content Management users can prepare hospice announcements to be displayed on the<br />
<strong>Xeris</strong> Welcome page.<br />
• User Administration allows the user to add, change and delete <strong>Xeris</strong> accounts for other<br />
hospice employees.<br />
• PIN Enabled User settings are only applicable to those hospices that have elected to use<br />
PINs. Note to XHAs of PIN-enabled hospices - the default and recommended setting for a<br />
Clinical user is PIN-enabled and for a Census user is NOT PIN-enabled, however these<br />
settings may be modified on an individual basis.<br />

To assign Report Options, check one or more of the checkboxes:<br />
• Clinical Reports gives access to a menu of reports showing pharmacotherapy profile<br />
information to assist clinical staff with managing their patients.<br />

• Administrative Reports gives access to a menu of specific and general summary reports to<br />
assist administrative staff in managing census and expenses.<br />
• Billing Reports gives access to invoice summary and supporting data for HP billing of per<br />
diem charges, dispensed medications and other ancillary fees for monthly billing periods.<br />
• Collaborative Reports are only available to those hospices participating in a Collaborative<br />
Practice Model, giving access to a menu of reports showing detailed encounter<br />
information about assessments and plans of care.<br />
• Organizational Aggregate Reports (formerly known as Summary Reports) are only<br />
available to users with access to multiple organizations, providing a menu of summary<br />
versions of Administrative Reports across multiple organizations.<br />
• Note – all <strong>Xeris</strong> users have access to patient-specific reports.<br />

When all choices have been made, click Add User to save the changes. You will receive a screen with<br />
the new user’s Login ID and a temporary Password. This information must be printed or recorded<br />
immediately as it is only displayed once and cannot be retrieved later. This password is temporary and<br />
will only be used for the individual’s first login, at which time they will be prompted to select their own<br />
secure password for HIPAA reasons. You may want to provide your new <strong>Xeris</strong> users with a copy of the<br />
<strong>Xeris</strong> User Info document that explains Windows settings for HIPAA, how to log in for the first time and<br />
how to modify <strong>Xeris</strong> account settings.<br />

About Collaborative Practice<br />
<strong>Hospice</strong>-specific, pharmacist-driven Collaborative Practice Protocol management is available for hospices that<br />
subscribe to the collaborative model. A major benefit of working under collaborative practice is the reduced time<br />
to care plan implementation by obtaining referral sources’ approval to prescribe under protocol. In the <strong>Hospice</strong><br />
Collaborative Practice System (HCPS), the prescriber adopts the protocols as the Prescriber Care Plan and the<br />
multidisciplinary care team follows the plan. The plan of care is communicated to the prescriber through routine<br />
feedback reporting using <strong>Xeris</strong>. For more information about this exciting program, contact HP by email at<br />

Collaborative Protocols<br />
The toolbar for hospice partners participating in HCPS will contain an icon of the hospice’s logo as a link to the<br />
hospice’s online book of protocols. Click on any symptom in the bookmark list to review or print the flowchart for<br />
that symptom.<br />
Clinical Assessments<br />
Clinical assessments for patients served by a hospice partner participating in HCPS are more robust and<br />
tailored to the protocols jointly established.<br />
Collaborative Reports<br />
<strong>Hospice</strong> partners participating in HCPS have access to additional reports.<br />
