PLAN<br />

Purpose<br />

This <strong>Pharmaceutical</strong> <strong>Care</strong> <strong>Services</strong> <strong>Plan</strong> describes current provision of community<br />

pharmacy services within <strong>NHS</strong> <strong>Lanarkshire</strong> at 1 st April 2011. It is expected that this<br />

document shall evolve over time as our expectations and vision for pharmaceutical<br />

services change and develop in years to come.<br />

Information is presented for <strong>NHS</strong> <strong>Lanarkshire</strong> as a whole and subdivided into 12 major<br />

towns/ areas within the Health Board.<br />

The prime purpose of this document is to summarise current service provision so that<br />

potential applicants for a new community pharmacy contract may consider the needs of<br />

the Board when considering their application.<br />

The plan will also be referred to by the Board’s Pharmacy Practices Committee hearings.<br />

Background to <strong>NHS</strong> <strong>Lanarkshire</strong><br />

<strong>Lanarkshire</strong> is predominantly urban but with a significant rural component towards the<br />

south of the county.<br />

It has a population of approximately 560,000 and contracted primary care services are<br />

delivered through<br />

which are widespread throughout the Health Board area.<br />

Community Pharmacy <strong>Services</strong><br />

<strong>Pharmaceutical</strong> care is currently provided across <strong>Lanarkshire</strong> through a network of 121<br />

community pharmacies included in the Board’s <strong>Pharmaceutical</strong> List. Presently there is<br />

one pharmacy included in the Board’s Provisional <strong>Pharmaceutical</strong> List until 25 th<br />

September 2011 (subject to any application for an extension of this date up to a period of<br />

nine months).<br />

Seventeen factors which are important for characterising community pharmacy services<br />

and how they address the needs of the local population are shown below. Local<br />

guidance and support materials for delivering these services are available on:<br />

1. Location.<br />

Accessibility to community pharmacies by the general public is a major consideration.<br />

ADEQUACY COMMENT<br />

Given the diverse nature of <strong>Lanarkshire</strong>’s geography we have not, at this time, set<br />

standards for either distance from or travel time to a community pharmacy. We believe<br />

that the travel time to a pharmacy within each of the localities should be appropriate for<br />

the rurality of that locality. This issue is considered on an individual basis at PPC<br />

hearings and takes factors such as public transport availability and the particular needs of<br />

vulnerable members of the community such as frail elderly and mothers with babies into<br />

account.<br />

2. Disability Discrimination Act<br />

All pharmacies have been given guidance about the Disabilities Discrimination Act and<br />

their requirement to take reasonable actions to make their pharmacies accessible for all.<br />

ADEQUACY COMMENT<br />

118 pharmacies in <strong>NHS</strong> <strong>Lanarkshire</strong> self report as being accessible. It is important that<br />

within a community patients with disabilities can easily access at least one pharmacy.<br />

Our records indicate that this is achieved with the sole exception being the village of<br />

Glenboig. Whist this village is the only area where this aspiration is not automatically met<br />

from a readily accessible pharmacy, the pharmacy staff will respond to request for help in<br />

accessing the service from any patient who has difficulty.<br />

3. Opening Hours<br />

The National Health Service (<strong>Pharmaceutical</strong> <strong>Services</strong>) (Scotland) Regulations 2009, as<br />

amended, requires <strong>NHS</strong> Boards to produce a scheme of model opening hours. The<br />

model opening hours for <strong>Lanarkshire</strong> are Monday to Friday 9.00am to 6.00pm, Saturday<br />

9.00am to 5.00pm with the ability to have one week day early closure of 9.00am to<br />

1.00pm. Each pharmacy is allowed an hour to close at lunch time.<br />

ADEQUACY COMMENT<br />

A high proportion of pharmacies within <strong>Lanarkshire</strong> open extended hours, one until<br />

10.30pm at night and 16 are open 7days per annum. In addition, very few pharmacies<br />

close over the lunch time period.<br />

Public holiday cover is arranged in consultation with the contractors, Area <strong>Pharmaceutical</strong><br />

Committee and Out of Hours services to ensure that all needs are covered.<br />

4. Dispensing of Prescriptions<br />

The timeous and accurate dispensing of prescriptions remains the principal function of<br />

the of the <strong>NHS</strong> community pharmacy service. All 121 community pharmacies in <strong>NHS</strong><br />

<strong>Lanarkshire</strong> provide this service and in 2009/10 over 11 million prescriptions were<br />

dispensed<br />

ADEQUACY COMMENT<br />

The Health Board has no evidence of patients having difficulty in accessing dispensing<br />

services<br />

5. Minor Ailments Service<br />

The Minor Ailments Service facilitates the <strong>NHS</strong> treatment of minor ailments by<br />

pharmacists working within their community pharmacies. This service is available to<br />

patients exempt from the standard categories of prescription charges and provides:<br />

Free of charge treatment<br />

Patient choice,<br />

Ready no-appointment required access and<br />

It also transfers some demand from GP and out of Hours services.<br />

Over 97 000 patients are registered with community pharmacies within <strong>NHS</strong> <strong>Lanarkshire</strong><br />

for this service and 15 000 prescriptions are written each month. Every community<br />

pharmacy in <strong>NHS</strong> <strong>Lanarkshire</strong> provides this service.<br />

ADEQUACY COMMENT<br />

The Health Board has no evidence of patients having difficulty in accessing the Minor<br />

Ailments Service<br />

6. Urgent Supply of Medication when a GP is not contactable<br />

Especially in the out of hours period and on public holiday weekends, <strong>NHS</strong> 24 can<br />

receive a high number of calls related to the resupply of medicines. A patient group<br />

direction which authorises community pharmacist to use their professional judgment in<br />

making a supply of medicines in response to an urgent need has proven to be very<br />

successful. All community pharmacies in <strong>NHS</strong> <strong>Lanarkshire</strong> provide this service.<br />

ADEQUACY COMMENT<br />

The Health Board has no evidence of patients having difficulty in accessing the urgent<br />

supply of medicines service<br />

7. Display of Nationally agreed Public Health messages<br />

All community pharmacies in <strong>NHS</strong> <strong>Lanarkshire</strong> participate in the core service of<br />

displaying posters with nationally agreed public health messages. The messages are<br />

coordinated nationally and pharmacies are required to display 4 posters per year for 6<br />

weeks at a time. Recent topics include:<br />

This service is consistently provided to national standards<br />

8. Smoking Cessation Service<br />

In the period 1/4/08 – 31/3/11 all Health Boards had a smoking cessation HEAT target to<br />

support 8% of their smoking population to successfully quit at 4 weeks. For <strong>NHS</strong><br />

<strong>Lanarkshire</strong> this equated to 10737 successful quits. This target number was met in March<br />

2011 – two thirds of successful quits were achieved from patients attending the <strong>NHS</strong><br />

<strong>Lanarkshire</strong> Stop Smoking Service and the remainder was met by the community<br />

pharmacy service.<br />

The HEAT target for 1/4/2011 – 31/3/2014 is to support 7.5% of the Boards smoking<br />

population successfully quit 4 weeks from their quit date through specialist services with<br />

60% of the quits coming from the 40% most deprived within Board SIMD areas. For <strong>NHS</strong><br />

<strong>Lanarkshire</strong> this equates to 9882 successful quits of which 5929 need to be within the<br />

most deprived areas.<br />

ADEQUACY COMMENT<br />

All but one community pharmacy participates in this service though two others in the<br />

immediate surrounds do so<br />

Community pharmacy contributions to HEAT target achievement in 2008-11 illustrate<br />

provision of an adequate service.<br />

9. PGD authorised supply of Emergency Contraception<br />

119 community pharmacies participate in this service and cumulatively in 2009/10 almost<br />

9 000 supplies of Emergency contraception were made<br />

ADEQUACY COMMENT<br />

All but two community pharmacies participates in this service though several others in the<br />

immediate surrounds do so.<br />

11. Supply of Chlamydia test kits and treatment of Chlamydia<br />

All pharmacies also participate in the supply of Chlamydia test kits. A PGD is also in<br />

place to authorise treatment of Chlamydia by community pharmacists though the supplies<br />

made via this service have been small approx 35 in 2010/11. Consequently there will be<br />

a review of this service in 2011/12.<br />

11. Chronic Medication Service<br />

This will be a particularly important service for helping patients ensure that their long term<br />

medication is used safely and effectively.<br />

ADEQUACY COMMENT<br />

The service began in earnest in 2010/11 but is still maturing. On 31 st Jan 2011 115<br />

community pharmacists in <strong>NHS</strong> <strong>Lanarkshire</strong> had registered 3371 patients for CMS.<br />

At this stage there is no evidence that patient demand is outstripping service availability.<br />

12. Assessment of patient’s compliance needs and dispensing in Monitored Dose<br />

systems<br />

As more vulnerable patients are cared for in the community, often with the support of<br />

family carers or Council employed home support workers, there is a need to assess<br />

whether or not patients can use medicines presented in conventional bottles or blister<br />

packs. Community pharmacists can assess patients for the suitability of a range of<br />

medicine presentations and supply the one which best meets the patient’s requirement<br />

e.g. a monitored dose system. In providing this service pharmacists are expected to<br />

consider the guidance provided by <strong>NHS</strong> <strong>Lanarkshire</strong> with regard to the assessment<br />

process and provision of monitored dose systems.<br />

118 pharmacies in <strong>NHS</strong> <strong>Lanarkshire</strong> provide this service. Annually approximately 4, 800<br />

patients receive specialised weekly dispensing.<br />

ADEQUACY COMMENT<br />

This is a service which can be under pressure from time to time. We have recently<br />

reissued guidance on the assessment of patients for this service to minimise the risk of<br />

inappropriate demand and supply.<br />

13. <strong>Services</strong> for Patients with substance misuse addictions<br />

In <strong>NHS</strong> <strong>Lanarkshire</strong> over 5, 000 patients have an drug addiction problem and over 2,000<br />

are receiving substitution therapy in the form of methadone or buprenorphine. Many<br />

patients require regular and frequent dispensing of their medication (often daily) and<br />

supervision of the consumption of that medication within the pharmacy. It is therefore a<br />

major service.<br />

108 pharmacies in <strong>NHS</strong> <strong>Lanarkshire</strong> provide this service and between them supervise<br />

the consumption of over 250 000 doses of methadone and approx<br />

28 000 doses of buprenorphine each year.<br />

In recognition that drug injecting behaviour remains widespread it is important that good<br />

harm reduction strategies are developed. Recently the concept of Needle Syringe<br />

Exchange has been enhanced to a fuller “Injection Equipment Provider” (IEP) service. In<br />

<strong>NHS</strong> <strong>Lanarkshire</strong> 18 community pharmacies geographically spread to facilitate as wide<br />

access as possible provide an IEP service and approximately 18 000 transactions are<br />

carried out each year.<br />

ADEQUACY COMMENT<br />

There is close working between <strong>NHS</strong> <strong>Lanarkshire</strong> addictions services and community<br />

pharmacists to make sure that the community pharmacy services can meet the demands<br />

of the service<br />

14. Oxygen <strong>Services</strong><br />

The provision of a community based oxygen service is essential for patients and local<br />

access via a community pharmacy is of great benefit to patients. It is however a<br />

specialist service and not all pharmacies have the experience or indeed the space to<br />

provide this service. Within <strong>NHS</strong> <strong>Lanarkshire</strong> 47 pharmacists provide this service.<br />

ADEQUACY COMMENT<br />

The widespread locations of the pharmacies providing domiciliary oxygen coupled with<br />

the fact that pharmacists transport oxygen to the patients homes ensures that all patient<br />

need is met.<br />

15. Palliative <strong>Care</strong> Safety Net Pharmacy Provision<br />

In palliative care it is sometimes necessary to change a patient’s medicine and use<br />

relatively unusual medicines. Because the medicines can be unusual it is not reasonable<br />

to expect every pharmacy to have them immediately in stock. Yet, patients and family<br />

would reasonably expect the health service to rapidly respond to their needs. The<br />

concept of the palliative care safety net pharmacies is that 18 pharmacies, geographically<br />

spread throughout <strong>NHS</strong> <strong>Lanarkshire</strong>, each stock an agreed quantity and range of unusual<br />

medicines so that a patient/family can be assured that the pharmacy can rapidly respond<br />

to a changing prescription.<br />

ADEQUACY COMMENT<br />

There is regular feedback about this service and 3 pharmacies in relatively rural locations<br />

were added to the list of providers in 2010/11. This expanded network is fully expected to<br />

meet all patient need.<br />

16. Stoma <strong>Care</strong><br />

The provision of stoma services is optional but all community pharmacies in <strong>NHS</strong><br />

<strong>Lanarkshire</strong> provide this service<br />

ADEQUACY COMMENT<br />

All pharmacies provide this service to national standards<br />

17. Consultation Rooms/Private Advice Areas<br />

With community pharmacy developing a greater clinical role the need for a consultation<br />

room is increasing.<br />

ADEQUACY COMMENT<br />

117 community pharmacies in <strong>NHS</strong> <strong>Lanarkshire</strong> have at least 1 consultation room within<br />

the pharmacy and those which do not have a private advice area.<br />

Conclusion<br />

This pharmaceutical care services plan describes many factors which are important in<br />

assessing the adequacy or otherwise of community pharmacy provision within a particular<br />

neighbourhood.<br />

These factors, coupled with the appendices which illustrate provision within <strong>NHS</strong><br />

<strong>Lanarkshire</strong> as a whole and within specific localities will be of value to potential applicants<br />

for a new pharmaceutical contract.<br />

<strong>NHS</strong> <strong>Lanarkshire</strong> looks forward to developing its <strong>Pharmaceutical</strong> <strong>Care</strong> <strong>Services</strong> <strong>Plan</strong> in<br />

line with Regulations and advice from SGHD in the expectation that this will facilitate the<br />

ongoing rational provision of pharmaceutical services in response to evolving services<br />

and the needs of our local populations.<br />
