Risk Management Annual Report 2008 – 09 Scottish Patient Safety Programme (SPSP) The Corporate Risk Manager is a core member of the leadership workstream and a core member of the team identified to undertake the ward/department Walk Arounds. There has been significant success in achievement within each workstream, including the Safety Briefings, to reduce risk to patients as reported through NHSL Patient Safety Reports. This work was shared across NHSL and with partners at the successful internal ‘Strengthening Quality’ event. Subsequent to demonstration of this success, NHS Lanarkshire have been invited to present on 3 out of 4 clinical workstreams, 1 leadership workstream and 1 programme management workstream, at the National Learning Session scheduled for May 2009. Work has commenced on the development of an incident reporting form to record adverse events identified from the case note reviews. Whilst this work has been primarily undertaken in the Acute areas, early discussions are underway to assess the feasibility of a single-system approach to patient safety, for example high risk medications.
