APPLICATION INSTRUCTIONS
1. Completely FILL IN and PRINT this application.
2. SIGN all required forms manually.
3. SCAN the completed application.
4. SCAN a clear and legible copy of your passport.
5. SCAN a bank statement demonstrating that you or your sponsor has the financial resources
required for your education in the U.S. (Funds shown should reflect flight course tuition).
6. EMAIL all of the document scans to rdean@flymiami.com.
7. After sending the visa application, there is a USD $200 fee for shipping and processing. All
payments must be made using PayPal or a valid credit card. Please take the time to read the
instructions for making a payment below.
PAYMENT INSTRUCTIONS
1. On the PayPal form on the visa page, select the type of visa you are applying for. (If you are a
student applying to Dean International for the first time or for recency, select "I-20 Processing /
Shipping Fee $200". If you or someone you designate will be visiting Dean to pick up your I-20,
select "I-20 Processing Fee (No Shipping) $150".)
2. Type in your name the same way it has been written in this visa application.
3. Click "Pay Now" to be transferred to PayPal's payment site.
4. Follow PayPal’s instructions, making sure to include your shipping address (regardless of visa
application type) and payment method, as well as any extra comments.
5. Submit your PayPal payment.

TERMS OF APPLICATION
Upon receipt of the documents listed in the application instructions by email, an I-20 form and
acceptance letter will be sent to you (applicant). You will then be able to proceed to a U.S.
Consulate or Embassy in your country to request a visa.
Upon arrival to Dean International, students are enrolled according to the requirements of the
school and the U.S. Government.
A USD $9,350.00 tuition deposit is required prior to starting the Program. The remaining
portion of the tuition must be paid in $5,500.00 increments ($6,250 for C-172), every 30 days.
Dean International reserves the right to change its payment policies at will.
