If I kept it to myself Steps to be taken Community-based projects can link with medical and social support services to provide: • Medical and nursing care, including an element of home based care (HBC) • Access to appropriate diagnosis • Treatment and prevention of opportunistic infections, including TB • Management of HIV-related illnesses • Antiretroviral therapy • Psychological support (counselling) • Socio-economic support to families, children affected by AIDS, and key populations vulnerable to HIV, such as children and youth • Human rights and legal support, where human rights have been wronged • Active community involvement • Support for care givers • Referral mechanisms. Successful home based care programmes • Uphold the rights and dignity of people infected and affected by HIV and AIDS • Ensure active participation of family and community members • Provide space for equal partnership and mutual respect between family and community members of the home based care team facilitators • Build on the realities of living with HIV while maintaining hope based on community collective action • Maximize use of community resources while identifying and using additional external resources as needed • Have a strong element of confidentiality (shared)* • Scaling up programmes to meet future needs. *This is a situation where information and circumstances related to personal life should only be available at the discretion of the individual.
