Record Check Requests
Wisconsin adult criminal history information is considered public record and is
available pursuant to s. 165.82 Wis. Stats. Information is available by mail or online
at http://wi-recordcheck.org.
Juvenile criminal history information is considered confidential and is not available
unless specifically authorized by statute.
Criminal History Search Fee
165.82 Criminal history search fee. (1) Notwithstanding s. 19.35 (3), the
department of justice shall impose the following fees, plus any surcharge required
under sub. (1m), for criminal history searches for purposes unrelated to criminal
justice or, before July 1, 1993, to s. 165.825:
(a) For each record check requested by a nonprofit organization, $2.
(ag) For each record check by a governmental agency, $5.
(ar) For each fingerprint card record check request requested by a governmental
agency or nonprofit organization, $15.
(b) For each record check by any other requestor, $13.
(1m) The department of justice shall impose a $5 surcharge if a person requests a
paper copy of the results of a criminal history search requested under sub. (1).
(2) Except as provided in s. 175.34, the department of justice shall not impose fees
for criminal history searches for purposes related to criminal justice.

Identification Supplies Order Form
The Crime Information Bureau furnishes contributors with fingerprint cards, death
notice forms, final disposition report forms, and postage paid addressed envelopes
upon request.
The CIB preprints the fingerprint cards with the TIME jurisdiction number, name
and address of the contributing agency. Fingerprint cards should be ordered before
the contributor's supply is depleted to allow time for the printing. A minimum of
three months' requirement should be ordered.
Each shipment of supplies will contain a "Supply Request/Order Form" DJ-LE-235
for future orders. This form may also be found at
