Contributing/Arresting Agency Completion of Final
Disposition
When an individual is taken into custody for an offense the fingerprint card and Final
Disposition Report must be completed for submission to the Crime Information
Bureau. Final Disposition Reports are only retained in the CIB files when there is a
related fingerprint card on file. The portions of the final disposition that must be
completed by the contributing/arresting agency are shown following.
The contributing/arresting agency is required to complete the data on the left side of
the Final Disposition Report with the exclusion of the charge blocks on the lower left
side. These are for the use of the Prosecutor/District Attorney to complete when the
charges are being changed to something other than those filled out by the
contributing/arresting agency. The Final Disposition Report must be initiated at the
time the original fingerprint card is completed and all required data must appear
exactly as it appears on the related fingerprint card submitted. If the transaction(s)
listed in the "offenses charged at arrest" block are disposed of by the
contributing/arresting agency, the final disposition must be completed with the
appropriate information and mailed to the Crime Information Bureau for processing.
In the event the case goes to the prosecutor the Final Disposition Report must be
forwarded to the prosecutor with the appropriate case papers. A separate Final
Disposition Report must be completed for each arrest charge listed on the fingerprint
