Contributing/Arresting Agency Completion <strong>of</strong> Final<br />

Disposition<br />

When an individual is taken into custody for an <strong>of</strong>fense the fingerprint card and Final<br />

Disposition Report must be completed for submission to the Crime Information<br />

Bureau. Final Disposition Reports are only retained in the CIB files when there is a<br />

related fingerprint card on file. The portions <strong>of</strong> the final disposition that must be<br />

completed by the contributing/arresting agency are shown following.<br />

The contributing/arresting agency is required to complete the data on the left side <strong>of</strong><br />

the Final Disposition Report with the exclusion <strong>of</strong> the charge blocks on the lower left<br />

side. These are for the use <strong>of</strong> the Prosecutor/District Attorney to complete when the<br />

charges are being changed to something other than those filled out by the<br />

contributing/arresting agency. The Final Disposition Report must be initiated at the<br />

time the original fingerprint card is completed and all required data must appear<br />

exactly as it appears on the related fingerprint card submitted. If the transaction(s)<br />

listed in the "<strong>of</strong>fenses charged at arrest" block are disposed <strong>of</strong> by the<br />

contributing/arresting agency, the final disposition must be completed with the<br />

appropriate information and mailed to the Crime Information Bureau for processing.<br />

In the event the case goes to the prosecutor the Final Disposition Report must be<br />

forwarded to the prosecutor with the appropriate case papers. A separate Final<br />

Disposition Report must be completed for each arrest charge listed on the fingerprint<br />

card. However, when an individual is being charged with multiple counts <strong>of</strong> the<br />
