WI Identification No. (SID)
This space is to record the Wisconsin State Identification Number assigned to the
individual by the CIB, if known.
Contributor ORI
REQUIRED This space is to record the contributor of the fingerprints. The name of
the contributing agency must be entered exactly as it appears on the related
fingerprint card sent to the Crime Information Bureau. The nine digit ORI
jurisdiction number assigned to the agency must be included.
Arresting Agency ORI
REQUIRED This space is to record the "arresting agency" if different than the
"contributor" of the fingerprint card. The nine digit ORI jurisdiction number
assigned to the agency must be included.
Contributor OCA No.
REQUIRED The "contributing agency" OCA (originating case agency number)
must appear here and it must be the same as that recorded on the related fingerprint
card submitted to the CIB.
Date of Arrest
REQUIRED This space must contain the date the person fingerprinted was arrested
or fingerprinted and this date must be exactly the same as the date indicated on the
related fingerprint card submitted to the CIB.
Date of Offense
REQUIRED This space should contain the date or approximate date of the alleged
offense.
Offense Reported by Arresting Agency
REQUIRED This block is to record necessary data relating to the charge(s) for
which the person fingerprinted was arrested or taken into custody. Space is provided
for one arrest charge on each Final Disposition Report and it must appear exactly as
it appears on the related fingerprint card submitted to the CIB. First record the
"literal" charge, using authorized abbreviations. Also include the following:
Charge/Citation (Literal, Statute No. or Ordinance, NCIC
Code)
REQUIRED This space is to record the state statute corresponding to the literal
charge. When the charge is made for a local ordinance violation, enter "local ord"
rather than cite the specific number of the local ordinance. The statute numbers
must appear exactly the same as they appear on the fingerprint card submitted
to the CIB.
Record the NCIC OFFENSE code which corresponds to the "Literal Charge" and
"statute/ord" information. THE NCIC OFFENSE CODE MUST BE EXACTLY
