Miscellaneous Identifying Numbers<br />

The appropriate two-letter identifying code must precede the number and is<br />

separated from the number by a hyphen (-). Any alphabetic character(s) which is<br />

part <strong>of</strong> the number is to be included, except as noted in the following.<br />

U.S. military services are now using Social Security numbers to identify military<br />

personnel. This use applies to active duty, reserve and retired service personnel.<br />

This number should be entered in both the Miscellaneous Number and Social<br />

Security Number fields. If a member <strong>of</strong> the Army National Guard has a social<br />

security number <strong>of</strong> 420-19-0210, it should be entered at 420190210 in the Social<br />

Security field and as AS-410190210 in the Miscellaneous Number field.<br />

Information from a license issued to operate a motor vehicle should be entered.<br />

Expired license information may be entered when current data is not available.<br />
