<strong>Identification</strong> Requests<br />

After a series <strong>of</strong> discussions with several sheriffs’ <strong>of</strong>fices and correctional facilities,<br />

it became apparent that altering prisoner-handling procedures at either end was not<br />

feasible. Thus a low-tech method was devised. It requires the person who searches<br />

the criminal history record to fill out a simple form and to see that this form follows<br />

the prisoner from the county facility to the correctional facility. The form is printed<br />

on brightly colored paper so that the correctional staff will notice it and remove it<br />

from the prisoner’s file at intake.<br />

The new process is as follows:<br />

1. Determine the person’s DNA status by searching the CIB criminal<br />

history records.<br />

2. If a DNA sample has not already been taken, please obtain a sample<br />

following the instructions in the DNA Offender Sample Kit.<br />

3. The new step depends on where the person will be going after being<br />

processed by your agency.<br />

• If the person will be going to a <strong>Department</strong> <strong>of</strong> Corrections Facility,<br />

then please fill out the “DNA SAMPLE TAKEN FORM”. This<br />

form must be included with the person’s file when they are<br />

transported to the correctional facility. At intake, the form will<br />

be used to enter the DNA status into the DOC database. Please<br />

note that if you do not take a sample because the CIB records<br />

indicated one already had been taken, this information still needs<br />

to get to Corrections. There is a box to check to indicate that a<br />

sample had been taken previously. The date the prior sample was<br />

taken is not shown on the CIB records and need not be filled in on<br />

the form.<br />

• If the person will stay in a county facility or if the person is on<br />

parole, the new form does not have to be completed.<br />

A law enforcement agency may desire to check an individual's identity or past<br />

criminal record for an investigation being conducted before an actual arrest has been<br />

made. The Crime Information Bureau will process these fingerprint cards and return<br />

the completed card to the contributing agency with either "NO PREVIOUS<br />

RECORD" or "IDENTIFICATION TRANSCRIPT ATTACHED" stamped in the<br />

charge block <strong>of</strong> the fingerprint card.<br />

The law enforcement agency submitting a fingerprint card for this purpose must<br />

indicate in the "final disposition block" the reason the card is being submitted as<br />

"criminal investigation." This will ensure proper processing <strong>of</strong> the fingerprint card.<br />
