REQUIRED Indicate the race of the person being fingerprinted using the
appropriate one-character alphabetic code.
Height
REQUIRED Indicate the height of the person being fingerprinted using the
appropriate three digit numeric code.
Weight
REQUIRED Indicate the weight of the person being fingerprinted using the
appropriate three digit numeric code.
Eye Color
REQUIRED Indicate the eye color of the person being fingerprinted using the
appropriate three character alphabetic code.
Hair Color
REQUIRED Indicate the hair color of the person being fingerprinted using the
appropriate three character alphabetic code.
Place of Birth
REQUIRED Indicate the place of birth using the appropriate two character
alphabetic code.

Include any specific information necessary to further identify the applicant
fingerprint card for your agency. CIB uses this field for billing purposes to record
the submitter’s billing account number. An account number is required in this field
for any applicant fingerprint for which a fee is charged.
FBI No.
Include the applicant’s FBI number, if known.
Armed Forces No.
Include the applicant’s military service identification number, if applicable.
Social Security No.
Include the applicant’s social security number.
