General Applicant Cards
Minimum Data Requirements
Fingerprint cards may be submitted for watchman/guard licensing, parimutuel/lottery
positions, Indian gaming, teacher licensing, caregiver or daycare, and
other employment requests for licensing purposes as required by state statute. In
addition, certain applicant cards may be submitted pursuant to federal law, such as
the Volunteers for Children Act or Public Law 105-277 (Home Health Care and
Nursing Facilities). These are processed in the same manner as the law enforcement
applicants with the exception that a "search fee" will be imposed for each request.
Minimum required data must be supplied with the appropriate fee on the card and
submission of the card to the CIB will result in one of the standard responses.
Fingerprint cards for watchman/guard licensing, pari-mutuel/lottery positions, Indian
gaming, teacher licensing, caregiver or daycare licensing are submitted using the
ORI of the state agency authorized to request such background checks. Background
checks for home health care and nursing facilities or submitted under the Volunteers
for Children Act are submitted under CIB's ORI and the FBI responds directly to
CIB.
When an individual requests to be fingerprinted as an applicant, the person taking the
fingerprints should always verify the identity of the person being fingerprinted prior
to the taking of the prints. Some form of official photo identification is the best
method of verifying identification. Applicant fingerprint cards contain the following
required data:
Name
REQUIRED Indicate the full and complete name of the person who is being
fingerprinted
Aliases
Indicate any alias or maiden names the person being fingerprinted has used.
Signature of Person Fingerprinted
REQUIRED The person being fingerprinted must sign the applicant card.
Residence of Person Fingerprinted
Fill in the complete address of the person being fingerprinted.
Contributor ORI and Name
REQUIRED All applicant fingerprint cards should come pre-printed with the ORI
and name of the agency submitting the fingerprints. All responses on applicant
fingerprints will be sent to the agency identified by the information in his field.
Date of Birth
REQUIRED Fill in the date of birth of the person being fingerprinted in the
appropriate format.
