General Applicant Cards<br />

Minimum Data Requirements<br />

Fingerprint cards may be submitted for watchman/guard licensing, parimutuel/lottery<br />

positions, Indian gaming, teacher licensing, caregiver or daycare, and<br />

other employment requests for licensing purposes as required by state statute. In<br />

addition, certain applicant cards may be submitted pursuant to federal law, such as<br />

the Volunteers for Children Act or Public Law 105-277 (Home Health Care and<br />

Nursing Facilities). These are processed in the same manner as the law enforcement<br />

applicants with the exception that a "search fee" will be imposed for each request.<br />

Minimum required data must be supplied with the appropriate fee on the card and<br />

submission <strong>of</strong> the card to the CIB will result in one <strong>of</strong> the standard responses.<br />

Fingerprint cards for watchman/guard licensing, pari-mutuel/lottery positions, Indian<br />

gaming, teacher licensing, caregiver or daycare licensing are submitted using the<br />

ORI <strong>of</strong> the state agency authorized to request such background checks. Background<br />

checks for home health care and nursing facilities or submitted under the Volunteers<br />

for Children Act are submitted under CIB's ORI and the FBI responds directly to<br />

CIB.<br />

When an individual requests to be fingerprinted as an applicant, the person taking the<br />

fingerprints should always verify the identity <strong>of</strong> the person being fingerprinted prior<br />

to the taking <strong>of</strong> the prints. Some form <strong>of</strong> <strong>of</strong>ficial photo identification is the best<br />

method <strong>of</strong> verifying identification. Applicant fingerprint cards contain the following<br />

required data:<br />

Name<br />

REQUIRED Indicate the full and complete name <strong>of</strong> the person who is being<br />

fingerprinted<br />

Aliases<br />

Indicate any alias or maiden names the person being fingerprinted has used.<br />

Signature <strong>of</strong> Person Fingerprinted<br />

REQUIRED The person being fingerprinted must sign the applicant card.<br />

Residence <strong>of</strong> Person Fingerprinted<br />

Fill in the complete address <strong>of</strong> the person being fingerprinted.<br />

Contributor ORI and Name<br />

REQUIRED All applicant fingerprint cards should come pre-printed with the ORI<br />

and name <strong>of</strong> the agency submitting the fingerprints. All responses on applicant<br />

fingerprints will be sent to the agency identified by the information in his field.<br />

Date <strong>of</strong> Birth<br />

REQUIRED Fill in the date <strong>of</strong> birth <strong>of</strong> the person being fingerprinted in the<br />

appropriate format.<br />
