Completed Example - Impression Side impression side completed example FBI Arrest Fingerprint Card The State of Wisconsin fingerprint card is based on the FBI arrest fingerprint card. However, there are a few fields that appear on the FBI card that do not appear on the Wisconsin card. These fields all appear on the face side of the card. These fields include: Reply Desired? This field consists of a checkbox if the submitting agency would like a result of the FBI fingerprint search returned to them. Send Copy To The submitting agency may enter the ORI of any law enforcement agency they want a copy of the results of the FBI fingerprint search returned to. Listings of agency ORIs may be found in the Agency Identifier section of this manual. State Bureau Stamp This area if for CIB to indicate the fingerprints have been processed at the state level.
