Date of Birth
REQUIRED Must be numeric characters representing month, day, and year. Should
be separated by slashes for paper submissions in the order (mm/dd/yy or
mm/dd/yyyy). However, for electronic fingerprint card submissions, the year must
be four characters without separators in the following order: ccyymmdd.
Sex
REQUIRED Sex must be one character, either F (female) or M (male).
Race
REQUIRED Race must be recorded using one of the following codes:
• A-(Asian or Pacific Islander)
• B-(Black)
• I-(American Indian or Alaskan Native)
• W-(White)
• U-(Unknown)

NOTE: Hispanics should be recorded using one of the above codes.
Height
REQUIRED Must be three numeric characters representing feet and inches. Do not
use fractions. Round to the nearest inch. Height coding examples may be found in

Weight
REQUIRED Must be three numeric characters representing weight. Do not use
fractions. Round to the nearest pound. Examples for weight entries may be found in
the Personal Descriptors section of this manual.
Eyes
REQUIRED Must be three character alpha code as listed in eye color section of
Personal Descriptors in this manual.
Hair
REQUIRED Must be a three character alpha code as listed in hair color section of
Personal Identifiers in this manual.
Use "XXX" for a completely bald individual and indicate in scar, mark, tattoo field
"166" (BALD).
