Name Field - Single Names
For persons who use only one name; e.g., Moon, the name should be entered as the
surname and X entered as the first name. For example:
• MOON/X
Name Field - Gang or Street Names
These types of names usually consists of a single name, making them difficult to
index or search. These single name gang or street identifiers should be placed in the
remarks section of the fingerprint card. Please identify the remark as a "street" or
"gang" name.
Name Field - No Middle Name
If no middle name or initial exists, leave the field blank. Using None, NMN or NMI
will cause the middle name field to be populated by those identifiers, rather than
properly leaving it blank.
Signature of Person Fingerprinted
This space is to record the signature of the person fingerprinted. If a individual will
not sign write "Refused" in this space.
Social Security No.
This number of the person fingerprinted should be included on the fingerprint card if
available to your agency.
Aliases/Maiden
When you know that the individual has used other names or dates of birth they
should be listed in this block. Maiden names are considered alias names. The
master name on a given record is the primary name submitted on the first fingerprint
card for that individual.
Maiden or alias names must contain both a first and a last name. If Janet Beth
Aldrone marries John Smith and legally takes his name, the next fingerprint card
contributed for this subject should have the name Janet Beth Smith listing Janet Beth
Aldrone in the alias field. "Nee", indicating a former name, should not be included in
the alias field.
FBI No.
The FBI number is assigned to an individual the first time the FBI receives an arrest
fingerprint card. The number remains with that person as long as the FBI retains the
file.
State Identification No.
The SID number is assigned to an individual the first time an arrest fingerprint card
is received at the CIB. The number remains with that person as long as the CIB
retains the file.
Some electronic fingerprint submission devices or record management systems may
not be able to accommodate state identification numbers exceeding eight characters
