Name<br />

(REQUIRED) Always list the complete last and first names. If middle name is<br />

known it should also be included. Middle initial should be used when complete<br />

middle name is not known, or if the person only uses an initial. If the name is<br />

illegible the fingerprint card will be rejected. Printing or typing <strong>of</strong> the name is<br />

recommended.<br />

Name Field - Suffixes<br />

Suffixes are names that follow the last name, such as II, III, Jr., Sr., etc.<br />

Examples: John Robert Smith III, John Smith Sr., John Smith Jr.<br />

Name suffixes should not be confused with abbreviations following the name<br />

indicating some type <strong>of</strong> degree, such as MD, PhD, DDS, DVM, JD, etc.<br />

Name Field - Prefixes<br />

Prefixes precede the name and should not be included. Prefixes include Mr., Mrs.,<br />

Ms., Miss and Dr.<br />

Name Field - Compound or Hispanic Names<br />

For compound names enter both surnames with or without the hyphen.<br />

For Hispanic names, which usually contain compound surnames (paternal and<br />

maternal), the full name including both surnames, if known, should be entered in the<br />

name field. For example:<br />

• Subject’s Name - Jose Rodriguez-Gonzalez (Hyphenated)<br />

• Enter Name - RODRIGUEZ-GONZALEZ/JOSE<br />

• Subject’s Name - Jose Rodriguez Gonzalez (non-hyphenated)<br />

• Enter Name - RODRIGUEZ GONZALEZ/JOSE<br />

• Subject’s Name - Jose Rodriguez y Gonzalez (Spanish letter “y” in<br />

place)<br />

• Enter Name RODRIGUEZ Y GONZALEZ/JOSE<br />

Additionally, each surname component <strong>of</strong> the full name should be entered<br />

individually as a separate alias to ensure the possibility <strong>of</strong> a hit should either surname<br />

be inquired upon.<br />

When entering each surname component individually as an alias, enter the hyphen or<br />

letter “y” if present in the name. Using the above name as an example, the following<br />

two aliases would be appended to the base record: RODRIGUEZ/JOSE and<br />

GONZALEZ/JOSE.<br />

Other compound surnames should be entered in the same manner as described above.<br />

Name Field - Oriental Names<br />

For Oriental names the last unit should be entered as the master last name. Each <strong>of</strong><br />

the other units should be entered as a surname/alias, with the other units shown as<br />

first and middle name. For example:<br />

• NAI/DONALD/LEE<br />

• LEE/DONALD/NAI<br />
