Juvenile Fingerprint<br />

If the individual is under the age <strong>of</strong> 17, the "YES" box relating to "SUBMISSION"<br />

should be checked. If the individual has been waived into or will be prosecuted in<br />

adult court, the "YES" box relating to "TREAT AS ADULT" should be checked.<br />

Date <strong>of</strong> Arrest<br />

REQUIRED This should be the date the individual is arrested. Must be numeric<br />

characters representing month, day, and year, separated by slashes, in that order. For<br />

paper fingerprint card submission, the year can be two or four characters (mm/dd/yy<br />

or mm/dd/ccyy). However, for electronic fingerprint card submissions, the year<br />

must be four characters without separators in the following order: ccyymmdd.<br />

ORI Contributor<br />

REQUIRED This block must contain the nine character NCIC agency identifier and<br />

name <strong>of</strong> the agency submitting the fingerprint card. A listing <strong>of</strong> <strong>Wisconsin</strong> ORIs is<br />

found in the <strong>Wisconsin</strong> Agency Identifier section <strong>of</strong> this manual.<br />
