Directive 3.5 – Prisoner ID and Documentation<br />

3.5.1 Prisoner ID and Documentation<br />

A. Officers accepting prisoners from another agency shall ensure that warrant and/or<br />

booking records/numbers and other descriptors assigned to the individual are<br />

confirmed before transport.<br />

B. Proper documentation must accompany a prisoner being transported between facilities<br />

and agencies.<br />

Prisoners transported to jail must be accompanied by the U-10-100 (original copy) and<br />

the appropriate commitment paper(s), as applicable. These forms usually consist of the<br />

“Commitment Before/After Conviction and Sentence”. Commitments are not needed<br />

when the prisoner is being slated on State charges.<br />

Personal property removed from a prisoner will be transferred with him/her to the<br />

receiving detention/holding facility. If the receiving detention/holding facility refuses the<br />

prisoner’s property it will be receipted onto a Property Receipt (PD-99-163) and secured<br />

in a property locker.<br />

If the prisoner has been treated for an illness or injury prior to his/her transportation, the<br />

documented medical treatment record(s) will be forwarded with him/her to the receiving<br />

facility.<br />

C. Officers who transport a prisoner from a non-adjacent county in the State of <strong>Ohio</strong> or from<br />

another state will ensure that the proper extradition papers have been filed. The<br />

extradition paper(s) will be transported with the prisoner and a copy of the paper(s) will<br />

be included in the arrest file.<br />

D. Information regarding potential security risks presented by a prisoner will be documented<br />

and sent to the receiving detention/holding facility with the transporting officer. This<br />

information may include, but not limited to, escape risks, suicidal tendencies, or unusual<br />

illness (es) of a prisoner. The arresting officer is responsible for advising the transporting<br />

officer(s) of any specific risks presented by a prisoner and for ensuring that the proper<br />

documentation will be forwarded to the receiving facility.<br />
