14.1.5 Report Preparation<br />

A. An accurate record of events that transpire at the scene of a crime in connection with the<br />

investigation is required at the time of trial. It is the responsibility of the detective who<br />

processes the scene to submit a detailed report describing the sequence of events<br />

associated with a scene investigation as it relates to the collection of evidence. This<br />

documentation should be on a Supplemental Report and submitted with the<br />

investigative/grand jury packet.<br />

B. In addition to the inventory requirements, the detective’s report shall contain the<br />

following:<br />

Date and time of arrival at the scene<br />

Location of the crime<br />

Name of the victim(s), if known<br />

Name of the suspect(s), if known<br />

Narrative of the evidence technician’s actions at the scene, including the number<br />

of photographs taken, latent prints lifted, and measurements made<br />

Incident report number<br />

C. If a specialist is called to the scene (i.e. laboratory technician, pathologist, etc.), the date<br />

and time of request and the requesting officer’s name will be added to the investigation<br />

synopsis. All information obtained by the specialist will be documented with the<br />

detective’s report or the Incident Report. If the specialist prepares his/her own report, a<br />

copy of it shall be attached to the Supplemental Report.<br />

D. The same responsibilities for accurate crime scene reporting also apply to officers and<br />

detectives when completing an Incident or Supplemental Report and conducting an<br />

investigation.<br />
