diabetes therapy, which are also the focus

the Rhine Corridor: Bonn - Cologne - Düsseldorf“ 

project is the national and international 

marketing of medical excellence, 

which provides the basis of its growing 

attractiveness to patients from Germany 

and abroad.

the project want to open the eyes of those 

accompanying patients to the many and 

varied tourist offers along the Rhine corridor. 

This offers great opportunities to draw 

additional attention from people all over 

the world to the region and the wellness 

state of North Rhine Westphalia.

The metropolitan region of Rhineland from Dusseldorf to Cologne and Bonn is one of the leading medical 

destinations in Germany. It is a location for high-tech, top-notch medicine and telemedicine, as 

well as pharmaceuticals and medical technology. Many medical services already have national and 

international appeal and attract patients and companies to the region along the Rhine. 

With the cities of Cologne, Bonn and Leverkusen 

and the Rhein-Sieg District, the 

Rhine-Erft District, the Oberbergisch District 

and the Rheinisch-Bergisch District, 

the health region in Rhineland comprises 

an area of approximately 4,400 square 

kilometres with a total of approximately 

3.5 million residents. The state capital 

of Dusseldorf has an area of approximately 

217 square kilometres with about 

600,000 inhabitants. The metropolitan 

region of Rhineland, from Dusseldorf, to 

Cologne to Bonn, makes up 14 per cent 

of the total area and 23 per cent of the 

population of NRW. 

The healthcare industry is one of the 

largest sectors of the economy in both 

North Rhine-Westphalia and the Rhineland. 

In addition to the high levels of employment, 

the healthcare industry in the 

Rhineland demonstrates great potential 

for innovation. 

Many sectors of the regional health economy 

have already completed the internationalization 

process successfully. In 

addition to the pharmaceutical and biotechnology 

industries, the medical technology 

sector is on the way to conquer 

global markets. Health-care facilities, such 

as hospitals and rehabilitation clinics, are 

also turning their focus internationally 

and are broadening their range of skills, 

in addition to the sources of extra-budgetary 

treatment revenues. This additional 

business is becoming increasingly important, 

particularly in times of globalization 

and the opening of national borders. 

As a result, clinics in the metropolitan 

Rhineland region have actively sought 

international patients. According to the 

Federal Statistical Office, some 70.000 

foreign patients annually seek inpatient 

treatment in German hospitals. Additionally, 

there is a far greater number of outpatients. 

Almost a fifth of these international 

guests opt for a clinic in NRW. Apart 

from Aachen, the cities of Dusseldorf, 

Bonn and Cologne in particular are the 

primary destinations for medical travel. 

With more than 25,000 beds in 90 hospitals, 

the health region of Cologne-Bonn, 

in cooperation with partners from Dusseldorf, 

has a significantly higher treatment 

capacity than Berlin or Munich. 

Particularly popular at the moment in the 

„Health Destination Rhineland“ are disciplines 

such as ophthalmology, orthopaedics, 

trauma surgery and sports orthopaedics, 

cardiology, cardiovascular surgery, 

oncology, neurology and neurosurgery, 

as well as the comprehensive services of 

the rehabilitation facilities. Diseases such 

as diabetes and obesity are common comorbidities 

of inbound patients and are 

treated directly. 

In addition, numerous tourist and service 

providers complement the spectrum 

of medical services. The cities of Bonn, 

Cologne and Dusseldorf offer first-class 

hotels, dining, arts and cultural centres, 

leisure attractions and extensive gardens 

and parks. The spectrum ranges from the 

picturesque Siebengebirge hills with the 

legendary Drachenfels to the world-famous 

Cologne Cathedral, to Düsseldorf‘s 

Königsallee, a shopping boulevard with 

designer shops and jewellery stores.

Bonn airports as well as numerous 

airlines offer many connections and excellent

Competence Centre for the Treatment of 

Obesity at the Johanniter Hospital in Bonn 

The clinic specialises in the treatment of overweight patients and is 

certified as a “competence centre for obesity and metabolic surgery”. 

In recent years, Evangelischen Kliniken 

Bonn gGmbH has successfully established 

an interdisciplinary treatment 

programme for massively overweight 

patients, in which different methods of 

treatment, ranging from conservative to 

operative therapy, are effectively combined. 

As a result of this specialisation, the clinic 

was awarded the “Competence centre 

for obesity and metabolic surgery” 

certificate in November 2011 and is one 

of 34 specialist centres in Germany. 

The excellent treatment structure comprises 

a number of disciplines – surgery, 

diabetology, endocrinology and pneumology 

– and offers patients all-round 

and meticulous care on the path to 

weight reduction. In addition to medical 

professionals, further specialists such as 

dieticians, psychologists, psychiatrists, 

physiotherapists and sport trainers are 

involved. 

The patient benefits from holistic care 

“from a single source”. 

The therapy plans may include the typical 

conservative basic programme 

(medical and, where appropriate, psychotherapeutic 

components, nutritional 

advice, exercise therapy) and/or a proposal 

for surgical treatment (minimally 

invasive operations: gastric band, gastric 

bypass, and gastric sleeve). The programme 

is headed by Prof. Dr. A. Türler 

(surgery) and Priv.-Doz. Dr. U. Schmitz 

(internal medicine-conservative). 

In 2011, the department was successfully 

certified by the CAADIP (Surgical Working 

Group for Adiposity Therapy) of the 

DGAV (German Society for General and 

Visceral Surgery) as a competence centre 

for obesity and metabolic surgery.

Competence in Obesity Surgery at the 

St. Johannes Hospital in Troisdorf-Sieglar 

The multimodal obesity concept at the St. Johannes Hospital: 

Obesity surgery, self-help group, nutritional advice and physiotherapy 

– all “under one roof”. 

Since 2005, bariatric procedures have 

been conducted in the surgical department, 

i.e. operations intended to reduce 

severe obesity. So-called sleeve resections 

and gastric bypasses have established 

themselves as procedures. Recent 

research has shown that both operations 

not only result in significant weight reduction, 

but also have a curative effect 

on insulin-dependent diabetes mellitus 

and elevated blood pressure. 

Within the framework of the weekly 

consultation, the patient’s individual 

medical history is examined in an initial 

interview, questions are answered and 

fears allayed. In cooperation with resident 

doctors of nutritional medicine, diabetologists, 

psychotherapists, and working 

together with self-help groups and 

experts in nutrition, the conservative 

possibilities are first exhausted. 

A failure of conservative therapy results 

in an indication for surgery. The patients 

are admitted as inpatients to a specially 

equipped ward. The different specialist 

departments (internal medicine, radiology, 

anaesthesia) are involved in the preoperative 

preparations. 

The operation itself is conducted by 

specially trained surgeons under the supervision 

of the head of department Dr. 

med. U. Schumann. After an average of 

4 to 7 days, the patients can be released 

from inpatient treatment. Aftercare 

takes place within the framework of an 

obesity consulting hour. 

The Obesity Centre in the Holy Trinity 

Hospital in Wesseling 

With Smart-XL®, the Obesity Centre offers an optimal concept 

for battling obesity. 

Obesity – a chronic condition, which 

brings with it a high risk of further serious 

and even life-threatening illnesses, 

and leads to a considerable deterioration 

in the quality of life. 

The Wesseling Obesity Centre, which was 

certified in summer 2011, offers Smart- 

XL®, a unique multimodal therapy concept 

for fighting obesity. This includes 

exercise therapy from exercise therapists, 

nutritional advice from qualified nutritional 

scientists, and behaviour modification 

from behavioural therapists. The 

individual modules are offered both in 

group therapy and individual meetings. 

The whole programme takes 27 months. 

After submission of an application, the 

costs are borne by the insurer. 

As it is unique, Prof. Dr. C. A. Jacobi 

addresses national and international 

congresses about the concept. If, during 

the course of a patient’s treatment, it 

becomes clear that an operation makes 

sense, the appropriate optimal method is 

discussed with the patient. 

The bariatric procedures carried out at 

the Wesseling Obesity Centre include the 

fitting of a gastric band, gastric sleeve or 

gastric bypass. 

Following a surgical procedure, the patients 

continue to be given care in relation 

to exercise and nutrition. Inclusion in a 

self-help group helps to achieve an optimal 

and lasting outcome.

The St. Franziskus Hospital is an institution

Surgery at the St. Franziskus-Hospital 

in Cologne-Ehrenfeld 

The department is the only certified competence centre for obesity 

surgery in this district of Cologne. 

Chronic obesity and health consequences 

such as high blood pressure, arthrosis, 

fatty liver and diabetes give rise 

to a poor quality of life and millions of 

deaths. Starting from a BMI of over 40 

kg/m², a change of diet, sporting activities 

and behavioural therapy alone are 

generally ineffective. Only operations 

can lead to a long-term weight loss of 

approx. 70%. An exhaustive consultation 

is initially usually followed by participation 

in a conservative therapy programme 

lasting 6 or 12 months and in self-help 

groups. If that does not prove sufficient, 

we accompany the patient on the path 

to an operation. Prior to the procedure, 

risks are minimised by means of comprehensive 

risk screening. The following 

procedures are (almost always) conducted 

minimally invasively: 

- Gastric bypass 

(Y or “mini gastric bypass”), 

- Gastric sleeve, 

- Biliopancreatic diversion, 

- Gastric band and gastric balloon. 

The department specialises in the treatment 

of particularly heavy patients, “revision 

and redo surgery” (e.g. if there 

are problems after an earlier procedure) 

and abdominal wall reconstruction 

through plastic surgery. An interdisciplinary 

team supports the generally short 

in-patient treatment and the outpatient 

aftercare programme.

Obesity Surgery and Metabolic Surgery at 

the Cologne-Merheim Clinic 

The Obesity Surgery and Metabolic Surgery Department offers 

the highest level of competence, from initial contact through to 

long-term aftercare. 

One in two Germans has too high a bodyweight, 

which puts their health at risk. 

Obesity restricts the quality of life and 

is often related to accompanying illnesses 

and secondary ailments. In cases of 

extreme obesity, life may even be shortened. 

At the Cologne-Merheim Obesity Centre, 

headed by Dr. med. Jürgen S. Meyer, 

an interdisciplinary team with a graduate 

nutritional scientist, gradual psychologist 

and two experienced surgeons is 

available. 

A self-help group provides support. If it 

is established that a patient suffers from 

obesity requiring treatment, an attempt 

is first made to reduce his or her weight 

with conservative measures. Only if this 

proves unsuccessful are surgical measures 

considered. All customary operations 

are offered as minimally invasive. Thanks 

to the multidisciplinary nature of the 

clinic and team, adequate provision for 

obese patients is ensured. 

This applies in particular to patients with 

obesity III° or IV°, for whom nutritional, 

behavioural, exercise and, where appropriate, 

surgical therapy can achieve lasting 

weight reduction and a prevention or improvement 

of accompanying conditions.

The physical and psychological consequences of obesity are 

serious. The Centre in Leverkusen offers overweight patients 

new prospects. 

At the Centre for Metabolic Obesity Surgery 

(ZMAC), surgeons, anaesthetists, 

specialists in internal medicine, radiologists, 

nutrition advisers, physiotherapists 

and psychologists successfully treat 

(the most severely) obese patients. The 

range of treatments, the professional 

qualifications of the doctors and therapists, 

and the clinic‘s equipment are 

specially designed to meet the specific 

needs of its patients. Typical secondary 

ailments such as diabetes, high blood 

pressure and sleep apnoea can be significantly 

improved or cured by the procedures 

offered. 

Information, support, provision: 

If conservative treatment with nutritional 

advice, exercise therapy and, if necessary, 

behavioural therapy does not 

bring the desired success, an operation 

can be considered. In a series of consultations, 

doctors then explain in detail the 

different possible procedures to the pa- 

tient and discuss at length the possible 

risks involved and long-term consequences. 

We offer a range of options for losing 

weight, including the following:

All interventions are minimally intensive, 

i.e. particularly gentle for the patient. At 

the outpatient consultation, individual

is decisive in achieving lasting successful 

therapy. 

Lifestyle medicine 

In the field of lifestyle medicine, ZMAC 

also offers treatment with a stomach balloon 

or fat dissolution through injection 

lipolysis. 

The Obesity Centre at the St. Marien 

Hospital Ratingen GmbH 

Two thirds of the population of industrialised countries are 

overweight. One in five is even obese. 

At the Ratingen obesity centre, a conservative, 

multimodal therapy concept for 

combating chronic obesity is offered by 

a team of specialists. The concept consists 

of nutritional therapy, exercise therapy 

and behavioural therapy. 

Certified, interdisciplinary treatment in 

accordance with international standards 

is thus offered to its patients. 

Moreover, for many years the surgical 

treatment of obesity patients at the

Moreover, it has proven expertise precisely 

in nutritional medicine, the treatment 

of diabetes and all other classic 

accompanying conditions of obesity, as 

well as in the field of psychosomatics

This makes possible a diagnostic and 

therapeutic concept, adjusted individually

Due to attendant symptoms, the chronically overweight are an 

increasing medical and economic problem for our society. 

The theory elaborated in scientific studies, 

that long-term weight reduction 

and thus an improvement in metabolic 

complications can be achieved through 

diets and changes in living habits, does 

not normally prove true in practice. On 

the other hand, surgical therapy of obesity 

is currently the most effective form 

of therapy to not only lower weight, but 

also control Type 2 diabetes. Metabolic 

operations are today among the most 

frequently conducted interventions, although 

relatively little is currently know 

about how they work. 

Obviously, it is not stomach reduction 

per se and the resultant reduced intake 

of calories that leads to long-term weight 

reduction. The main mechanism of action 

is a lessened sense of hunger and 

an increased sense of satiety through 

complex physiological mechanisms. In 

addition to the classic surgical procedures, 

such as laparoscopic gastric bypass, 

interventional therapy concepts, such as 

EndoBarrier Therapy, are considered for 

certain groups of patients.

The Rhein-Ahr-Sieg diabetes centre at 

the Community Hospital Bonn offers 

outpatients and inpatients a differentiated 

programme for the improvement of 

metabolic control and for the treatment 

or prevention of secondary ailments 

of diabetes. The objective is to create 

an individual therapy concept for you, 

which facilitates coping with the condition 

in everyday life, so that in cooperation 

with you the best possible therapy

The Community Hospital Bonn has many 

years of experience in the treatment of 

diabetic foot syndrome. 

The “Internal Medicine/Endocrinology 

and Diabetology” clinical unit provides 

safe treatment of diabetes and its 

consequences as well as of secondary 

ailments in the area of internal medicine. 

Moreover, conservative therapy for 

purely neuropathic ulcers is provided, 

including orthotic relief. The work of the 

treatment centre has been recognised 

by the German Diabetes Society. 

The main focus of the disciplines “Interventional

Led by Prof. Dr. med. Frank M. Baer, the 

medical clinic of the St. Antonius Hospital 

has developed from being a highlyregarded 

certified clinic specialising in 

diabetes to become one of Germany’s 

largest cardio-diabetes centres with the 

integrated treatment of related cardiovascular 

illnesses. 

Specialists in diabetology, cardiology, 

angiology, wound surgery and radiology 

make possible an individualised 

treatment of diabetes from initiation 

of treatment through to stem cell therapy 

in the case of complex vascular 

late damage. They are supported by an 

experienced team of diabetes advisers, 

wound nurses, podiatrists, psychologists 

and graduate sport teachers. The competence 

developed over many years is 

embedded in an optimal care structure 

with the networking of radiological 

cross-sectional diagnostic imaging (multislice 

CT, MRT) and a state-of-the-art 

catheterisation laboratory, a facility of

The Cologne-Holweide Hospital has a unit 

focused on diabetology. Here the patients 

are cared for around the clock by a diabetes 

team. Moreover, all other units have 

diabetes mentors, who are in close contact 

with the specially trained diabetes team. 

The day-to-day work of the unit includes 

not only all important measurements, but 


also HBA1c monitoring, the monitoring of 

eating habits, special foot hygiene, the conduct 

of oral glucose tolerance testing, the 

creation of a diabetes passport from the 

German Diabetes Society (DDG) and a final 

consultation on the day of release. For people 

with diabetes mellitus who are cared for 

in the Holweide Hospital, individual advice/

Hormonal Disorders 

Hormones affect our lives in many different ways. Metabolic and 

endocrine disorders such as diabetes mellitus or osteoporosis are 

on the rise worldwide. 

Since September 2012, the Department 

for Internal Endocrinology and Diabetology 

has existed at the PAN Clinic. As 

endocrinology is a key interface to numerous 

specialities, which are already 

represented at the PAN Clinic, the competence 

of the PAN Clinic is strengthened. 

Patients with diabetes mellitus in 

particular can also profit from this, since 

through cooperation with disciplines 

such as cardiology, vascular surgery and 

ophthalmology, comprehensive patient 

care becomes possible. 

The spectrum ranges from early detection 

of diabetes mellitus, through the 

initiation or change of therapy, to the 

treatment of secondary ailments. Risk 

factors of diabetes mellitus, such as obesity, 

can be tackled specifically through 

nutritional advice and exercise therapy 

in cooperation with the PAN prevention 

centre. If outpatient care does not prove 

to be sufficient, inpatient care is also offered 

at the PAN Clinic.

Diabetology is led by Dr. Georg Mansmann, 

a specialist in internal medicine 

and endocrinology.

The Clinic for Internal Medicine II – Diabetology 

and Endocrinology, led by Prof. 

Dr. Stephan Schneider, focuses on the diagnosis 

and therapy of diabetes, thyroid 

and all other hormonally induced diseases. 

The adverse effects of diabetes are 

prevented by tailored therapy and any 

diabetes complications are optimally 

treated. 

A particular focus is the treatment of diabetic 

foot syndrome. Here it is primarily 

a holistic view of the patient, resulting 

from interdisciplinary dialogue among 

colleagues from the vascular surgery 

and cardiology departments, which is of 

help. 

As diseases such as diabetic foot syndrome 

are frequently accompanied by 

vascular diseases, the further stages 

of treatment are determined together, 

for the well-being of the patient. Consequently, 

the clinic has the country’s 

lowest foot amputation rate (and was 

awarded the NRW 2012 health prize). 

In addition to modern first-class medical 

treatment, training in real-life conditions, 

human warmth and caring, as well 

as the highest level of care are elementary 

components in how we look after 

our patients. 

Thanks to its excellent success in treatment, 

the Clinic for Internal Medicine II 

is one of the few departments in Germany 

to have been recommended by the 

German Society for Endocrinology.

At the Clinic for Endocrinology and Diabetology

The West German Diabetes and Health 

Centre (WDGZ) health centre offers a 

comprehensive preventive care programme:

patient’s state of health through to an 

individual diabetes programme with 

subsequent telemedicine aftercare. 

Headed by Professor Dr. med. Stephan 

Martin, one of the leading experts in the 

prevention of cardiovascular diseases 

and the treatment of diabetes mellitus, 

the West German Diabetes and Health 

Centre (WDGZ) conducts medically indicated 

examinations 

Anamnesis and health checks 

After a medical consultation regarding 

previous illnesses and the patient’s current 

state of health, extensive blood and 

ultrasound testing (heart, blood vessels, 

and abdominal organs) and stress tests 

(lungs and heart) are conducted. A complete 

health check normally takes two 

hours. All findings are explained in detail. 

Diabetes management and telemedicine 

In cases of diabetes mellitus or high 

blood pressure, the WDGZ offers longterm 

care. Alongside the latest therapy 

through medication, great emphasis is 

placed on lifestyle factors such as weight 

and physical activity. 

Telemedicine equipment is also available 

for this, by means of which blood 

glucose, blood pressure, weight and 

physical activity are transmitted to the 

WDGZ, so that outpatient aftercare can 

be guaranteed.

Roth Eye Clinic at the St. Josef Hospital Bonn 

This establishment, which is equipped for all eventualities, 

provides particular safety specifically for older patients. 

Since it was joined by Dr. Felix Roth and 

Dr. Claudia Inhetvin-Hutter, who enjoys a 

national reputation, the Roth Eye Clinic, 

run by the fourth generation of the family, 

offers ophthalmology at the highest 

level Both in terms of diagnosis and in its 

therapeutic possibilities, its work is based 

on state-of-the-art medicine. Thus a joint 

eye-care practice and a medical care centre 

for anaesthesia and ophthalmology 

are integrated into the clinic. These are 

complemented by a laser centre. 

The associated ophthalmology inpatient 

ward of the St. Josef-Hospital Bonn supplements 

the interlinked outpatient and 

inpatient treatment. Finally, the clinic 

is connected with an ocular diagnostics 

centre for use by the resident colleagues.

premises, the clinic provides everything 

which modern ophthalmology offers

The Hochkreuz Eye Clinic 

Here the focus is on seeing well. 

Since it was founded in 2004 by the pioneers 

in outpatient eye surgery, Prof. Dr. 

Koch and Dr. Peter Brauweiler, the Hochkreuz 

Eye Clinic has become one of the 

leading specialist clinics for outpatient 

eye surgery. Every year, well over 4,500 

outpatient eye operations are carried out. 

The Hochkreuz Eye Clinic’s range of 

treatments comprises all ophthalmological 

disorders. These include, in the 

anterior segment, in particular operations 

on the cornea, iris, eye lens and all 

refractive surgery. The latest operating 

equipment and lasers, including the excimer 

laser, are available to the doctors 

for their work. 

In the posterior segment, the clinic 

treats all diseases in the vitreous and 

retinal regions, such as age-dependent 

macular degeneration, floaters (“seeing

The Cologne University Eye Clinic offers the entire spectrum of modern 

surgical and conservative ophthalmology at the highest level. 

The Cologne University Eye Clinic uses 

modern, minimally invasive procedures, 

which minimise the risk involved in 

a necessary eye operation. Under the 

supervision of Prof. Dr. med. Cursiefen, 

lamellar corneal transplantation surgery 

for endothelial dystrophies (DMEK, 

DSAEK) and keratokonus (DALK) is possible. 

Furthermore, refractive laser and 

lens surgery (implantable lenses, LASIK, 

LASEK) are offered to improve sight and 

render spectacles unnecessary. In addition 

to conventional glaucoma treatment, 

modern, gentle non-filtering glaucoma 

surgery using canaloplasty and Trabectome® 

surgery is a method used at the 

Cologne University Eye Clinic. 

Under the supervision of Prof. Dr. med. 

Kirchhof, complex retinal disorders are 

diagnosed and treated. He specialises, 

for example, in the intravitreal treatment

Improving and preserving sight and preventing blindness 

using the resources of modern medicine. 

Diseases of the eye are diagnosed and 

cured by the Cologne-Merheim Eye Clinic 

through medication and surgical therapy. 

The excellently trained team of doctors 

uses the latest diagnostic techniques,

Entry formalities for medical treatment in Germany 

EU nationals 

EU nationals do not require a visa to enter the Federal Republic 

of Germany. 

Non-EU nationals 

Generally speaking, non-EU nationals require a visa for stays 

in Germany. A visa is not required for semi-annual visits of up 

to 90 days for nationals of those countries for which the European 

Community has abolished the visa requirement. For the 

issuing of a visitor or tourist visa for short stays not exceeding 

3 months in a member state of the Schengen area with medical 

treatment as the purpose of the trip, an application must 

be submitted by the patient to the respective mission of the 

destination country. Accompanying persons must themselves 

submit an application for a Schengen visa. 

Responsibility for the granting of visas 

Pursuant to section 71 (2) of the Residence Act, responsibility 

for issuing visas lies with the missions of the Federal Republic 

of Germany i.e. its embassies and consulates-general. 

Local responsibility for issuing the visa lies with the mission responsible 

for the area in which the applicant has his/her ordinary 

place of residence or domicile. 

Visa for medical treatment in Germany 

A visa application must be submitted by the applicant in person 

to the mission responsible for his/her place of residence. Acutely 

ill patients may have their applications submitted to the 

visa section of the German Embassy by an authorised person. 

For submission by a third party, an authorisation signed by the 

patient in front of a notary and a corresponding current medical 

certificate must be presented to the embassy. As a rule, 

missions require between two and ten working days to process 

a Schengen visa application. Applications for a longer period of 

medical treatment may take several months to process. 

The following documents are required for the application: 

• Identification card and copies of the pages with personal data 

• Completed application, signed in person, for the issuance 

of a Schengen visa with attached current, colour biometric 

passport photo (size 3.5 x 4.5 cm) 

• plus a second biometric passport photo 

• A declaration signed in person pursuant to section 55 of the 

Residence Act 

• Travel health insurance with proof of payment and a copy 

(valid for all member states of Schengen for the entire stay 

applied for with minimum cover of EUR 30,000) 

• Original birth certificate and a copy 

(for minors of up to 18 years: birth certificate and declaration 

of consent by both parents) 

Additional information for foreign patients 

1. Confirmation by the clinic / attending physician 

The letter of confirmation from the clinic / attending physician 

regarding the necessity of the forthcoming medical stay has to include 

detailed information about the start of treatment, the probable 

duration of treatment and the likely costs (with a comprehensive 

breakdown of the individual costs). The letter must not be 

over 3 months old. It must be in writing. A declaration by a doctor 

or a medical institution may be required to the effect that the necessary 

medical treatment is not possible in the home country. 

If an agency is commissioned with the organisation of the treatment, 

it should be noted that any information regarding the costs, 

scope and duration of the treatment must be confirmed solely by 

the clinic itself or attending physician himself/herself. 

2. Financing confirmation of the clinic or declaration on the 

assumption of costs 

The patient must submit a declaration that the financing of the 

treatment and the stay is secured (e.g. bank statements for at 

least 3 months / bank certificates regarding the current balance 

of the account / proof of the transfer of the probable costs or, 

by way of confirmation by the clinic, that the payment is secured). 

In the case of a declaration on the assumption of costs 

by another party, a formal declaration of liability pursuant to 

sections 66 to 68 of the Residence Act is required. This must 

not be more than 6 months old and must refer to the medical 

treatment (the original and a copy must be presented). 

3. Reservation confirmation for accommodation 

The place of stay of the patient (for outpatient treatment) must 

be demonstrated by him or her by way of evidence (e.g. confirmation 

of hotel reservation). If the patient is to be accommodated 

with a private person, a formal declaration of liability pursuant to 

sections 66 and 68 of the Residence Act must be presented. 

4. Proofs of willingness to return 

It may be the case that proofs of the social and economic situation 

of the patient (e.g. by means of letters from employers, salary 

statements, company licences, account statements for the last 

three months, proofs of ownership etc.) have to be presented. 

Additional information for accompanying persons: 

The accompanying person(s) of the patient is/are obliged to 

present certain evidence when a visa application is made. These 

include in particular: 

- Proof of financing 

- Proof of accommodation 

- Proof of willingness to return
