Section 4: Health Status<br />

Problems with vision are relatively common among<br />

older persons. Although few <strong>in</strong>dicate that <strong>the</strong>y<br />

cannot see at all, one fifth <strong>in</strong>dicate <strong>the</strong>y do not see<br />

clearly (<strong>and</strong> presumably ei<strong>the</strong>r have no eyeglasses or<br />

eyeglasses cannot correct <strong>the</strong>ir situation). Hear<strong>in</strong>g<br />

problems are somewhat less common. Aga<strong>in</strong> very few<br />

elderly cannot hear at all but 14 per cent <strong>in</strong>dicate <strong>the</strong>y<br />

do not hear clearly (<strong>and</strong> apparently lack a hear<strong>in</strong>g aid<br />

to correct <strong>the</strong> problem). Both vision <strong>and</strong> hear<strong>in</strong>g<br />

problems <strong>in</strong>crease substantially with age <strong>and</strong> are more<br />

common among older women than men <strong>and</strong> more<br />

prevalent <strong>in</strong> rural <strong>and</strong> urban areas. This is especially<br />

so for vision problems <strong>and</strong> <strong>in</strong> terms <strong>of</strong> <strong>the</strong> urbanrural<br />

contrast likely reflects more untreated cataracts<br />

among <strong>the</strong> rural elderly. Problems with <strong>in</strong>cont<strong>in</strong>ence,<br />

ei<strong>the</strong>r with respect to ur<strong>in</strong>ation or defecation, are<br />

reported by 17 per cent <strong>of</strong> older persons. These<br />

problems <strong>in</strong>crease with age, are higher for women than<br />

for men, <strong>and</strong> are somewhat more prevalent <strong>in</strong> rural<br />

<strong>and</strong> urban areas. F<strong>in</strong>ally, close to two thirds <strong>of</strong> older<br />

Thais report that <strong>the</strong>y were ill sometime dur<strong>in</strong>g <strong>the</strong><br />

past five years. Aga<strong>in</strong> this situation <strong>in</strong>creases with age<br />

<strong>and</strong> is somewhat higher for men <strong>and</strong> women <strong>and</strong> for<br />

rural than urban elderly.<br />

4.4 Functional limitations<br />

One serious consequence <strong>of</strong> decl<strong>in</strong><strong>in</strong>g health <strong>and</strong><br />

<strong>in</strong>creased frailty associated with age<strong>in</strong>g is difficulty <strong>of</strong><br />

physical movement <strong>and</strong> <strong>of</strong> <strong>in</strong>dependently be<strong>in</strong>g able<br />

to carry out basic activities <strong>of</strong> daily liv<strong>in</strong>g. As<br />

functional limitations <strong>in</strong>crease, assistance by caregivers<br />

becomes <strong>in</strong>creas<strong>in</strong>gly necessary. Table 4.2 exam<strong>in</strong>es<br />

measures <strong>of</strong> function<strong>in</strong>g by look<strong>in</strong>g at a variety <strong>of</strong><br />

potential disabilities that were asked about <strong>in</strong> <strong>the</strong> 2007<br />

Survey <strong>of</strong> <strong>Older</strong> <strong>Persons</strong> <strong>in</strong> Thail<strong>and</strong>. 12 The first three<br />

(eat<strong>in</strong>g, dress<strong>in</strong>g, <strong>and</strong> bath<strong>in</strong>g <strong>and</strong> us<strong>in</strong>g <strong>the</strong> toilet)<br />

are <strong>the</strong> most basic functional activities. Only<br />

relatively small percentages <strong>of</strong> persons age 60 <strong>and</strong> over<br />

are unable to do <strong>the</strong>se three activities by <strong>the</strong>mselves.<br />

Moderately higher percentages report hav<strong>in</strong>g<br />

difficulties squatt<strong>in</strong>g, climb<strong>in</strong>g two or three stairs, <strong>and</strong><br />

count<strong>in</strong>g change when us<strong>in</strong>g money. Considerably<br />

higher proportions <strong>of</strong> older persons are unable to do<br />

on <strong>the</strong>ir own those activities that require most<br />

physical exertion such as lift<strong>in</strong>g 5 kg or walk<strong>in</strong>g<br />

200-300 meters. Likewise us<strong>in</strong>g transportation such<br />

as buses or boats pose difficulty for a substantial share<br />

<strong>of</strong> older Thais. Overall, more than a third report at<br />

least one <strong>of</strong> <strong>the</strong> functional limitations listed.<br />
