4.5 Service Protocols and Standard<br />

Operating Procedures<br />

Standard Operating Procedures shall be developed in partnership between the pest<br />

management company and the food plant. All scope of work information including<br />

methods of inspection and treatment must be documented and approved by the food<br />

plant. Protocols and procedures shall include at least the following:<br />

• Procedures for all current or anticipated pest management activities including<br />

inspections and audits<br />

• <strong>Pest</strong> sighting log<br />

• <strong>Pest</strong> activity records<br />

• Treatment Records (see 4.6 for minimum information required)<br />

• Trend reports<br />

• Plant layout with pest management activity information listed such as rodent stations,<br />

monitoring devices, etc.<br />

4.6 Service and Treatment Records<br />

All service records for service including applications and/or monitoring shall include:<br />

• EPA or PMRA registration number<br />

• Product brand name<br />

• Target pest<br />

• Rate of application or % concentration<br />

• Time of application<br />

• Location or site of application<br />

• Amount of finished product used<br />

• Date<br />

• Lot number of product<br />

• Signature of technician<br />

• Signature of plant contact<br />

• Certification or registration number of technician<br />

• Emergency phone number<br />

• Notes and observations and any other records required by the state pest management<br />

enforcement agency<br />
