BC PNP APPLICATION CHECKLIST FOR MIDWIVES<br />

(All other applicants must use the standard BC PNP <strong>Application</strong> <strong>Checklist</strong>)<br />

The checklist below provides requirements <strong>for</strong> a complete <strong>Midwives</strong> application under the British Columbia<br />

Provincial Nominee Program (BC PNP). Please complete the checklist and submit it with your application to the BC<br />

PNP. If you cannot provide any of the required items, please provide a written explanation. Please note, the BC PNP<br />

reserves the right to ask <strong>for</strong> additional items if it deems such a request necessary.<br />

To speed up the processing of your application, please arrange the submitted documents in the following order:<br />

1) BC PROVINCIAL NOMINEE PROGRAM (BC PNP) FORMS<br />

Fee Payment Form (Money Order, Certified cheques, Master Card and Visa. Do not send cash.<br />

Nominee In<strong>for</strong>mation Form (to be completed by the nominee candidate)<br />

In<strong>for</strong>mation Release Form (to be filled out by the nominee candidate and spouse)<br />

The following item is only <strong>for</strong> midwife nominee candidates who are represented by an authorized representative<br />

(immigration lawyer or consultant). The BC PNP requires permission from the nominee candidate to disclose<br />

in<strong>for</strong>mation to the representative about the application.<br />

Please note: A representative is not required in order to submit an application to the BC PNP or to CIC.<br />

Consent from the Nominee Candidate (midwife applicant)<br />

This <strong>for</strong>m authorizes the BC PNP to disclose in<strong>for</strong>mation concerning the BC PNP application to an authorized<br />

representative. Nominee candidates can use the standardized <strong>for</strong>m: Letter of Consent <strong>for</strong> Release of In<strong>for</strong>mation to an<br />

Authorized Representative: Employee.<br />

2) EMPLOYMENT INFORMATION<br />

Canadian Immigration Permits or Records - Submit a Photocopy Only<br />

Nominee candidates who already have an existing work permit, study permit or visitor record must submit a copy<br />

verifying any current immigration status in Canada. Also include copies of permits or records <strong>for</strong> any spouse or<br />

dependent children living in Canada.<br />

Include a copy of your resume.<br />

Proof of Registration or Eligibility <strong>for</strong> Registration with the College of <strong>Midwives</strong> of British Columbia<br />

Include a signed letter from the registrar of the CMBC confirming that your credentials have been verified and that you<br />

meet the requirements <strong>for</strong> registration.<br />

Letter Confirming Practice Group Affiliation<br />

Provide an original letter from a British Columbia practice group confirming your acceptance as an affiliated midwife <strong>for</strong><br />

a period of at least 6 months. This letter must be on the practice group's official letterhead, and must be signed by a<br />

general registrant with the practice group and the midwife applicant. This letter is required by the BC PNP in place of the<br />

Guaranteed Job Offer Form and written Job Offer.<br />

3) CITIZENSHIP AND IMMIGRATION CANADA (CIC) FORMS<br />

• Submit only photocopies of these <strong>for</strong>ms to the BC PNP. Photos and police certificates are NOT required.<br />

• Do not submit any of the federal <strong>for</strong>ms to Citizenship and Immigration Canada (CIC) until you receive written<br />

Confirmation of Acceptance of your nomination. The BC PNP will provide nominees with instructions on submitting their<br />

federal <strong>for</strong>ms to CIC.<br />

• Do not submit federal application fees to the BC PNP<br />

Schedule 1: Background/Declaration<br />

IMM5406: Additional Family In<strong>for</strong>mation<br />

Schedule 4: Economic Classes - Provincial Nominee<br />

Any Spouse/Partner or Dependent Child who is 18 or older must complete a separate Schedule 1: Background/<br />

Declaration and IMM5406: Additional Family In<strong>for</strong>mation <strong>for</strong>m, whether or not she/he plans to accompany the applicant<br />

to Canada.<br />
