Parents/guardians and students are requested to read the statement below, check the boxes, sign, and date the
following to signify that they have read and understand the rules and policies contained in the Northwestern High
School Parent/Student Handbook.

school’s discipline policies and dress code policies. My student and I have discussed and
understand this document. We agree that he or she will abide by these rules.
_ I verify that I have read the Northwestern policy on student disciplinary actions on eligibility for
extra-curricular activities and that I will adhere to all requirements so that I may participate in
athletic or extra-curricular activities at Northwestern High School. I also understand that this
statement must be returned to NHS signed by the student and the parent/guardian prior to being
allowed to participate in athletic or extra-curricular activities.
