At present, the ARMMCIDU has a total strength of 23 composed of 5 PCOs,
15PNCOs and 3 Detailed Service PNCO from PRO-ARMM.
B. Review of Police Doctrine – Weekly PI & E and Monthly
Rank Inspection are being conducted at the Regional Office and Provincial Offices
disseminating to its personnel current issues, directive and policies of higher
headquarters.
C. Enhance of Capability Building – Thirty-six (36)
firearms were issued to personnel and properly accounted for every month,
thereafter.

3. On January 19, 2013 at about 4:30 in the afternoon, elements of this
Office led by PSI MELECIO M MINA arrested Jaime Guiaber a.k.a. Abdul
Mohaimen Mastura Guiaber, 20 years old, married, student, resident of Anacleto
Badoy, Poblacion 3, Cotabato City by virtue of Warrant of Arrest with Criminal Case
No. 2012-5423 for Robbery with Physical Injury. Said Operation is under the direct
supervision and control of the CIDG-15 Regional Chief PSUPT LEO QUEVEDO.
4. On February 1, 2013, elements of this CIDTeam led by the LSCIDT
Supervisor, PCI JOVIT CULAWAY, served the Warrant of Arrest against Mr.
Abulkair “Abul” Radia Rasad Tomawis at Bubong Madaya, Marawi City but it
yielded negative result. It was known that the said person might have detected the
approaching police officers and went into hiding before they could arrest him.
5. On February 1, 2013, elements of Lanao Sur CID Team led by the
LSCIDT Supervisor, PCI JOVIT CULAWAY, served the Warrant of Arrest against Mr.
Abulkair “Abul” Radia Rasad Tomawis at Bubong Madaya, Marawi City but it
yielded negative result. It was known that the said person might have detected the
approaching police officers and went into hiding before they could arrest him.
6. On February 6, 2013 at about 8:30 in the morning, elements of this
Regional Office led by PSI MELECIO M MINA proceeded to Sinsuat Avenue,
Cotabato City and effected the arrest of Mechelle Torres, 35 years old, single,
Pharmacy Clerk, and resident of Bonifacio Extn., Cotabato City by virtue of the
Warrant of Arrest with Criminal Case No. 2013 - 5456 for Qualified Theft WITH NO
Bail recommended, issued by Hon. Judge Bansawan Z Ibrahim of RTC 12, Branch
14, Cotabato City. Said Operation is under the direct supervision and control of the
CIDG-15 Regional Chief PSUPT LEO QUEVEDO.
7. On February 8, 2013 at
about 02:30 in the afternoon, joint
elements from this office led by PCI
JOVIT CULAWAY and Provincial
Intelligence Branch of Maguindanao
Provincial Police Office led by PCI
MANGELEN, proceeded to the
Municipality of Datu Unsay and effected
the Warrant of Arrest with Criminal
Cases No. Q-10-16265-66 and Q-10-

1637-66 for 58 counts of MURDER issued by Honorable Judge Jocelyn Solis-
Reyes, RTC Judge Quezon City against Maguid Amil for his alleged involvement in
the Maguindanao Massacre. But upon reaching in the area, the suspect sense the
presence of the arresting team, he (Maguid Amil) ran away to evade the arresting
team and threw a hand grenade towards the team, forcing the team to fired back
and was able to hit the suspect on his lower abdomen. The suspect was rush into
the nearest hospital but it was declared dead on arrival by his attending physician.
Said Operation is under the direct supervision and control of the CIDG-15 Regional
Chief PSUPT LEO QUEVEDO. The said accused has a monetary reward amounting
to two hundred fifty thousand pesos (Php 250,000.00) for his arrest under DILG
Memorandum Circular Nr. 2010-53 dated June 29, 2010.
8. On or about 8:30 in the morning of February
16, 2013, joint elements from ARMM-CIDG led by PSI
MELECIO M MINA and elements of the 45 th IB, 6 th ID,
Philippine Army implemented the arrest of Talembo
“Tammy” Masukat @ Talembo Abdulrakman suspect of
Maguindanao Massacre with Criminal Case Number Q-
09-162148-72; Q-10-162652-66; and Q-09-162216 for
Murder (58 counts). Said Operation is under the direct
supervision and control of the CIDG-15 Regional Chief
PSUPT LEO QUEVEDO. The said accused has a
monetary reward amounting to three hundred thousand
pesos (Php 300,000.00) for his arrest under DILG Memorandum Circular Nr. 2010-
53 dated June 29, 2010.
9. On March 8, 2013 at about 4:30 in the morning, joint elements of this
Office led by PCI JOVIT L CULAWAY and 44 TH SAC, 4 TH SAB, PNP-SAF led by PSI
JESS U DELA CRUZ served the Warrant of Arrest docketed 2013-5569 for
MURDER issued by Hon. Bansawan Z. Ibrahim, Executive Judge, RTC 12 TH Judicial
Region, Branch 13, Cotabato City against Monting Guiaman @ Tekok, Guinaid
Guiaman, Oting Guiaman @ Titing and several John Does at Tapayan, Sultan
Matura, Maguindanao.
Said police operation is under the direct supervision and control of the CIDG-
15 Regional Chief PSUPT LEO QUEVEDO and resulted to the apprehension of
Oting Guiaman @ Titing.

10. On March 12, 2013 at about 11:00 in the morning, elements of this Unit
led by PSI MELECIO M MINA arrested Mariam Ibrahim Datukali @ Bai Membei in
Philtrade Center, Sultan Kudarat, Maguindana by virtue of Warrant of Arrest Nr.
2012-5348 for ESTAFA.
11. On March 18, 2013 at about 10:30 in the morning, elements of this
Office led by PSI MELECIO M MINA, 43 RD SAC and 44 TH SAC, PNP-SAF, RCLO-
ARMM, PRO-ARMM and MAGPPO proceeded to Sitio Lab, Brgy Kuloy, Shariff
Aguak, Maguindanao to effect Search Warrant Number 4835 issued by Hon.
Bernalito R Fernandez of NCJR, RTC, Quezon City and the service of Warrant of
Arrest in Criminal Case Nr. Q-09-162148-72, Q-09-162216-31 and Q-10-162652-66
for 58 counts of Murder against Datu Dainga D Ampatuan. The said accused has
a monetary reward amounting to two
hundred fifty thousand pesos (Php
250,000.00) for his arrest under DILG
Memorandum Circular Nr. 2010-53
dated June 29, 2010.
Before the operating teams
could serve the aforementioned
Search Warrant and the Warrant of
Arrest, Datu Dainga D Ampatuan
who was then hiding inside his house fired upon them. The operating teams
thereafter returned fire hitting and killing Datu Dainga D Ampatuan himself. Said
Operation is under the direct supervision and control of the CIDG-15 Regional Chief
PSUPT LEO QUEVEDO.

The following are the significant accomplishments under CIDG
OPLAN “Paglalansag Omega”:
1. On January 24 2013, at about 08:20 in the
evening, joint elements of CIDG-ARMM led by PSI
MELECIO M MINA, SOTG-PRO-ARMM conducted
checkpoint along Narciso Ramos Highway at Brgy
Pinantao, Parang, Maguindanao that resulted to the arrest
of PO1 Samsoden A Sangcopan after he failed to present
any legal document. Recovered from his possession and
control one (1) unit of Caliber 9mm PEG Pistol with Serial
Number F22132 and one (1) magazine assembly loaded
with eleven (11) rounds of ammunitions for caliber 9mm
pistol. Said Operation is under the direct supervision and
control of the CIDG-15 Regional Chief PSUPT LEO
QUEVEDO.
On January 25, 2013, a case for Violation of PD
1866 as amended RA 8294 in relation to Omnibus
Election Code was filed in inquest proceeding against
aforementioned respondent before the Office of the
Cotabato Provincial Prosecutor under NPS DOCKET No.
XIV-07-INQ-13A-00307.
2. On January 25,
2013 at about 09:00 in the
morning, joint elements of ARMM-
CIDU led by PSI MELECIO M
MINA, Guindulungan MPS and
61 st Division Reconnaisance
Company, conducted checkpoint
along National Highway in Brgy.
Bagan,
Guindulungan,
Maguindanao, which resulted to the arrest of ANDAMEN SAMAD MAMA after he
failed to present a document from COMELEC to exempt him on the strengthened
election Gun-Banned, recovered from his possession and control one (1) SHOOTER
COMMODORE Caliber .45 Pistol with one (1) magazine assembly loaded with eight
(8) rounds of live ammunitions for Cal. 45. Said Operation is under the direct
supervision and control of the CIDG-15 Regional Chief PSUPT LEO QUEVEDO.
On January 28, 2013 at about 01:00 in the afternoon, a case for Violation of
RA 8294 in relation to Omnibus Election Code was filed in inquest proceeding before
the Cotabato Provincial Prosecutor’s Office under NPS Docket No. XIV-05-INQ-
13A-00074.
3. On February 3, 2013 at about 5:30 in the morning, joint elements of
this Regional Office led by PSI MELECIO M MINA, NCR-CIDU, SAF, PRO-ARMM,

MAGUINDANO-PPO, and 6 th ID, Philippine Army effected the Search Warrant No.
13-21362 for Violation of PD 1866 as amended by RA 8294 against DATU MUNGA
MIDTIMBANG issued by Executive Judge Marino M Dela Cruz of National Capital
Judicial Region, Branch 22 dated 31 st day of January, 2013, on his residence at
Brgy. Poblacion, Talayan,Maguindanao, but yielded a negative result.
4. On February 3, 2013 at about 5:30 in the morning, joint elements of
this Regional Office led by PSI MELECIO M MINA, NCR-CIDU, SAF, PRO-ARMM,
MAGUINDANO-PPO, and Philippine Army effected the Search Warrant No. 13-
21362 for Violation of PD 1866 as amended by RA 8294 against DATU MUNGA
MIDTIMBANG issued by Executive Judge Marino M Dela Cruz of National Capital
Judicial Region, Branch 22 dated 31 st day of January, 2013 on his residence at Brgy.
Poblacion, Talayan, Maguindanao, but yielded a negative result.

about 02:30 in the afternoon, joint elements
from this Office led by PCI JOVIT
CULAWAY and Provincial Intelligence
Branch of Maguindanao Provincial Police
Office led by PCI MANGELEN, proceeded
to the Municipality of Datu Unsay and
effected the Warrant of Arrest with
Criminal Cases No. Q-10-16265-66 and
Q-10-1637-66 for 58 counts of MURDER

and control of the CIDG-15 Regional Chief PSUPT LEO QUEVEDO.
6. On February 8, 2013 about 03:30 in
the afternoon, joint elements of this Office led by
PCI JOVIT CULAWAY, personnel from
Maguindanao Police Provincial Office led by
PSUPT JOEL CASUPAAN, 43 rd SAC, 4 th SAB,
SAF led by PSI LINO AKIANGAN, and the Chief of
the Regional Legal Service, PCI ENRIQUE D.
SAN MIGUEL proceeded to Poblacion Ampatuan,
Maguindanao and effected the Search Warrant
No. 2013-002 for violation of PD 1866 as
amended by RA 8294 (Illegal Possession of Firearm) against Jun Baliwan issued by
Hon Judge Bansawan Z Ibrahim, RTC 12, Branch 14, Cotabato City which resulted
to the confiscation of one (1) caliber 9mm pistol, one (1) caliber .45 Pistol and One
(1) 12 gauge Shotgun. Said Operation is under the direct supervision and control of
the CIDG-15 Regional Chief PSUPT LEO QUEVEDO.

On February 11, 2013 at about 01:00 in the afternoon, a case for violation of
PD 1866 as amended by RA 8294 (Illegal Possession of Firearms) and Omnibus
Election Code was filed before the Office of the Provincial Prosecutor under NPS
Docket No. XIV-05-INV-13B-00208 against Jun Baliwan.
7. On February 11, 2013 at about 8:10 in the morning, joint elements of
this Office led by PCI JOVIT CULAWAY Provincial Public Safety Company, RPSB-
ARMM intercepted a black Yamaha DT motorcycle with Plate No. LS 6313 while
conducting check point at crossing Simuay, Sultan Kudarat, Maguindanao during the
visit of HIS EXCELLENCY PRESIDENT BENIGNO SIMEON AQUINO III at Camp
Darapanan, Sultan Kudarat, Maguindanao.
Arrested Norudin Musa
Kusain and Rohamin G Sanday, both residents of
Pagalungan, Maguindanao on board said
motorcycle when confiscated from their possession
and control the following items without pertinent
documents, to wit:
- One (1) Cal 45 (ANAD) w/ SN
2071930 w/ one (1) magazine loaded w/ eight (8) rounds ammunition from Norudin;
- One (1) Cal 45 (ARMSCOR) w/ SN 1354493 w/ one (1) magazine
loaded with ten (10) rounds ammunition from Rohamin; and
- One (1) unit Yamaha DT (black) w/ Plate No. LS 6316.
8. On
or about 8:30 in
the morning of
February 16,
2013, joint
elements from
ARMM-CIDG led
by PSI MELECIO
M MINA and
elements of the 45IB, 6ID, Philippine Army implemented the arrest of Talembo alyas
“Tammy” Masukat alleged suspect of Maguindanao Massacre with Criminal Case
Number Q-09-162148-72; Q-10-162652-66; and Q-09-162216 for Murder (58
counts), he was captured after the encounter of more or less twenty-five (25) fully
armed men at Brgy. Manungkaling, Mamasapano, Maguindanao recovered from him
an M16 Rifle (Elisco) with Serial Number RP 155206 with attached M203 Grenade
Launcher with Serial Number 137781 including three (3) rounds of CTG 40MM HE.
Said Operation is under the direct supervision and control of the CIDG-15 Regional
Chief PSUPT LEO QUEVEDO.
9. On February 17, 2013 at about
06:00 in the morning, joint elements of this
Office led by PCI JOVIT CULAWAY,
personnel from MBLT-1, Philippine Marines,
Intelligence Group (IG-ARMM), Anti-
Kidnapping Group (AKG), and RID-PRO 12

proceeded to Sitio Bubong, Brgy Kalanganan 2, Cotabato City and effected the
Warrant of Arrest Number 2013-002 for violation of PD 1866 as amended by RA
8294 (Illegal Possession of Firearms and Explosives) against Manan Taib @
Commander Malvar, which resulted to the confiscation of One (1) Rifle Grenade and
One (1) Fragmentation Hand Grenade. Said Operation is under the direct
supervision and control of the CIDG-15 Regional Chief PSUPT LEO QUEVEDO.
On March 14, 2013, a case for Violation of RA 9615 (Illegal Possession of
Explosives) was filed before the Office of the Cotabato City Prosecutor against
Manan Taib @ Commander Malvar under NPS Docket No. XII-02-INV-2013-C-
01693.
10. That on February
18, 2013 at about 4:35 in the
afternoon, joint elements of
ARMM-CIDG led by PSI
MELECIO M MINA and 68 IB,
PA conducted mobile
chokepoint at Sitio Matalam,
Brgy Poblacion Talayan,
Maguindanao which resulted to
the arrest of Rondon Ugot, 35 year old, resident of Balas Guindulongan,
Maguindanao, and to the confiscation from his possession and control one unit of
Caliber .45 Pistol (Norinco) with Serial Number 617461 with one magazine assembly
loaded with eight (8) rounds of ammunitions and the recovery from him One (1) unit
carnapped Motorcycle with sidecar Honda Wave 100cc color black with Plate Nr:
QR2992, Chassis Nr: KWY10037840 and Engine Nr: KWY00E037845 amounting to
Fifty Thousand Pesos (50,000.00). Said Operation is under the direct supervision
and control of the CIDG-15 Regional Chief PSUPT LEO QUEVEDO.
On February 20, 2013, a case for Violation of PD 1866 as amended by RA
8294 (Illegal Possession of Firearms and Ammunitions) was filed in inquest
proceeding before the Office of the Cotabato City Prosecutor against Rondon Ugot
under NPS Docket No. XIV-05-INQ-13B-00078.
11. On February 21, 2013 at about 07:00 in the
evening, joint elements of ARMM-CIDU led by PSI
MELECIO M MINA, 44 th SAC, PNP-SAF, and 31 st Marines
Company, MBLT-1 led conducted checkpoint at CCI
Detachment along Brgy Poblacion 7, Cotabato City which
resulted to the arrest of one NILO GACRAMA PAYPA, 49
years old, a resident of 195 Feleciano Res, Sampaguita St,
RH-9, Cotabato City after he failed to present a document
from COMELEC to exempt him on the strengthened
election Gun-Banned, recovered from his possession and
control One (1) COLT MK IV Caliber .45 Pistol with Serial
Number 817719; One (1) magazine assembly for .45 pistol;
Seven (7) rounds of live ammunitions for Cal. 45.

12. On March 8, 2013
at about 4:30 in the morning,
joint elements of this office led
by PCI JOVIT L CULAWAY and
44 TH SAC, 4 TH SAB, PNP-SAF
led by PSI JESS U DELA CRUZ
served the Warrant of Arrest
docketed 2013-5569 for
MURDER issued by Hon.
Bansawan Z. Ibrahim, Executive
Judge, RTC 12 TH Judicial Region, Branch 13, Cotabato City against Monting
Guiaman @ Tekok, Guinaid Guiaman, Oting Guiaman @ Titing and several
John Does at Tapayan, Sultan Matura, Maguindanao.
Said police operation is under the direct supervision and control of the CIDG-
15 Regional Chief PSUPT LEO QUEVEDO has resulted to the apprehension of
Oting Guiaman and the seizure of one (1) Home-made KG9, one (1) Magazine
Assembly and seven (7) ammunitions of caliber 9MM.
On March 20, 2013, a case for Violation of PD 1866 as amended by RA 8294
(Illegal Possession of Firearms and Ammunitions) was filed inquest proceeding
before the Office of the Cotabato Provincial Prosecutor against Titing Guiaman
under NPS Docket No. XIV-07-INQ-13C-00338.
13. On March 18, 2013 at about
10:30 in the morning, elements of this
Unit led by PSI MELECIO M MINA, 43 RD
SAC and 44 TH SAC, PNP-SAF, RCLO-
ARMM, PRO-ARMM and MAGPPO
proceeded to Sitio Lab, Brgy Kuloy,
Shariff Aguak, Maguindanao to effect
Search Warrant Number 4835 issued by

Warrant of Arrest in Criminal Case Nr. Q-

Before the operating teams could
serve the aforementioned Search Warrant and the Warrant of Arrest, Datu Dainga D
Ampatuan who was then hiding inside his house fired upon them. The operating
teams thereafter returned fire hitting and killing Datu Dainga D Ampatuan himself.
They were also able to arrest two (2) companions of Datu Daiga D Ampatuan

violation of PD 1866 as amended by RA
8294 (Illegal Possession of Firearm) was
filed against Kinog Mamaluba for
possessing M16 Rifle above-described.
Another case for violation of PD 1866 as
amended by RA 9516 was filed against
Kinog Mamaluba for possessing four (4)
M203 Ammunitions which are known and
classified as High Explosives and Norin
Ariraya for possessing a hand grenade.
Said cases were filed at Maguindanao Prosecution Office and docketed under NPS
Docket Numbers XIV-05-INQ-13C-00082 for Violation of RA 9516 and XIV-05-
INQ-13C-00083 for Violation of RA 8294 respectively.

led by PCI JOVIT CULAWAY, conducted random checkpoint at the Municipality of
Balindong, Lanao del Sur in connections with the CIDG Flagship Project “OPLAN
PAGLALANSAG OMEGA” (Campaign Against Loose Firearms) and in relation to
COMELEC Resolution No. 9601 dated January 8, 2013, Ban on Bearing, carrying or
transporting of Firearms or other deadly weapon. That as a result thereof, the
apprehension of six (6) malefactors and the recovery of six (6) assorted firearms and

ammunitions. After they are requested to present any legal documents to possess
those seized items and they failed to do so.

On January 14, 2013, a case for Violation of RA 9287 (Illegal Numbers Game)
against aforementioned respondents before the Office of the Cotabato City
Prosecutor with NPS DOCKET No. XII-02-INQ-2013-A-C1262.
2. On March 3, 2013, this Office received information
from unknown concerned citizen about proliferation of Illegal
Numbers Game locally known as Last2/3 in Macacua Street,
Cotabato City. In response to the said report, at about 06:00 in
the evening of the same date, elements from this Office led by
PSI MELECIO M MINA dispatched and proceeded to the said
area to conduct surveillance and for possible Police Operation,
that upon arrival therein, the Team confirmed the report and
immediately arrest Jean Paragas Singco and resulted to the
confiscation from her possession several gambling
paraphernalia. Said Operation is under the direct supervision
and control of the CIDG-15 Regional Chief PSUPT LEO QUEVEDO.
On March 4, 2013, at about 02:20 in the afternoon, a case for Violation of
PD 1602 as amended by RA 9287 (Illegal Numbers Game) was filed before the
Cotabato City Prosecution Office in inquest proceeding against Jean Paragas
Singco docketed under IS No. XI-02-INQ-2013-C-01517.
3. On March 12, 2013 at about 07:40 in the evening, elements from this
Office led by PSI MELECIO M MINA, conducted Police Operation against Illegal
Gambling Games IN Rosal Street, RH-6, Cotabato City which resulted to the arrest
of the following persons:

of more or less One Thousand four Hundred board feet (1,400 bd ft) owned by Kasla
Buisan situated at Brgy Kalanganan 2, Cotabato City and resulting to the
apprehension of the following persons to wit;
1) Sawal Kadir/Male/42yo/ a residence of Bunggo Island, Parang, Maguindanao;
2) Oting Kusain/23yo/Male/a residence of Bunggo Island, Parang, Maguindanao;
3) Amir Acmad/18yo/Male/ a residence of Bunggo Island, Parang, Maguindanao;
4) Sadun Jamil/22yo/ a residence of Bunggo Island, Parang, Maguindanao; and
5) Ryan Acmad/20yo/Male a residence of Bunggo Island, Parang, Maguindanao.
Initial investigation conducted by this
Unit disclosed that the One Motorized Bangka
loaded with good lumber estimated dimension
of more or less One Thousand four Hundred
board feet (1,400 bd ft) which was navigated
by Sawal Pirsal Kadir and would deliver the
said lumber to Cotabato City.

On February 20, 2013, a case for Violation of PD 705 (Anti-Illegal
Looging Act) in relation to E.O 23 (Declaring a moratorium on the cutting and
harvesting of timber in the Natural and Residual Forest and Creating the Anti-
Illegal Logging Task Force) was filed before the Office of the Cotabato City

Prosecutor against Kasia Buisan under Docket Number XII-02-INV-2013-B-
01672.

February 17, 2013 at
about 11:30 in the
morning, elements from
this Unit led by PSI
MELECIO M MINA in
coordination with 1 st
Marine Battalion Landing
Team (MBLT1) and 44 th
SAC, PNP-SAF proceeded to CCI Detachment, Cotabato City and intercepted One

wood (Malakubing) in different dimension after they failed to present any permit or

On February 20, 2013, a case for Violation of PD 705 (Anti-Illegal Logging
Act) in relation to E.O 23 (Declaring a moratorium on the cutting and harvesting of

At about 2:00 in the afternoon of January 13, 2013, an entrapment operation
was conducted inside this Regional Office and resulted to the arrest of RONY SY
and EBELEN BIGCAS after they tried to bribe PSINP MELECIO MINA, PO2 Dexter
A Santos and ARMMCIDG investigators money amounting to Sixty Thousand Pesos
(P60, 000.00) in exchange for the release of the seven (7) persons arrested during

the service of the Search Warrant Number 2013-001 issued by Executive Judge
Bansawan Z Ibrahim, Al Haj, of RTC 12 th Judicial Region, Br 13, in the house of RG
MAY LAU YAP @ JIMJIM YAP located at Oblate Drive, Barangay Rosary Heights
Mother, Cotabato City for violation of PD 1602 as amended by RA 9287 (Illegal
Gambling Law) on January 11, 2013 at about 08:00 in the evening.
Said Operation is under the direct supervision and control of the CIDG-15
Regional Chief PSUPT LEO QUEVEDO.
On January 14, 2013, a case for Violation of Art. 212 of the RPC
(Corruption of Public Officer) against the aforementioned respondents before
the Office of the Cotabato City Prosecutor with NPS DOCKET No. XII-02-INQ-
2013-A-01264.
