1. Do you possess B.E (In respective branch i.e. Civil/ Mechanical/
Electrical) or equivalent qualification as mentioned in appendix B of
HSE (Class-II) Public Works Department (Irrigation Branch) Rules,
1970.
2. Do you possess knowledge of Hindi or Sanskrit upto Matric level If
yes, attach proof thereof.
3. Are you a member of Scheduled Castes of Haryana If so, attach proof
thereof.
4. Are you a member of Backward Classes of Haryana If so, attach proof
in support of your claim.
5. Are you an Ex. ECO/Ex-Serviceman/SSCO of Haryana
6. Are you dependent son/daughter of Ex-Servicemen of Haryana If so
furnish a latest certificate of eligibility from the concerned Secretary
Zila Sainik Board.
7. Are you a widow of Ex-ECO/Ex-Servicemen, who was killed in war or
a wife of Ex-ECO/Ex-Servicemen who is more than 50% disabled If
so, attach proof in support of your claim.
8. Are you dependent son/daughter of killed/ECO/Ex-Servicemen or who
is more than 50% disabled belonging to Haryana State If so, furnish a
certificate of eligibility from the concerned Secretary, Zila Sainik Board
9. Are you a physically handicapped of Haryana with atleast 40%
disability If so, mentioned your sub-category of physically
handicapped (Blind, Ortho, Deaf & Dumb).
10. Have you ever been debarred from future employment or disqualified
from appearance in examination by the Union / State Public Service
Commission or by Department of Govt. If yes, State clearly.
11. Are you posted in a forward remote area If yes, attach proof thereof
from the concerned Competent Authority.
