CMISP 835 Health Care Claim: Professional
Functional Group ID=HP
1. INTRODUCTION
OVERVIEW
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) legislation mandates that many
of the major health care electronic data exchanges, such as electronic claims and remittance advices, be
standardized into the same national format for all payers, providers, and clearinghouses, who submit
governed data electronically to the Sacramento County Department of Health And Human Services
(DHHS), must submit in the mandated HIPAA formats.
HIPAA specifically names several electronic transactions that must be followed when certain health care
information is exchanged. These transactions are published as National Electronic Data Interchange
Transaction Set Implementation Guides. They are commonly called Implementation Guides (IGs) and are
referred to as IGs throughout this document. Additionally, an addendum to each of the IGs has been
published and must be used to properly implement each transaction.
TRANSACTION TYPE
This 835 companion document is provided by DHHS Sacramento County, Information Technology
Services and Office of HIPAA Compliance for all providers, and clearinghouses, Receiving Electronic
Data Interchange (EDI) 835 Claim Payment/Invoice Transaction from DHHS. This document shows how
Sacramento County DHHS Create the EDI ASC X12 835 Electronic Transactions, 004010X091A1 for
compliance with the federal HIPAA Regulations of 1996.
SCOPE
The Companion guide covers data elements that are either required or situational to meet HIPAA
validation & DHHS Business Requirements for 835 Claims Transaction Only.
Note: Information about how a particular claim is adjudicated by DHHS is also outside of the scope of this
companion guide.
PROGRAM DESCRIPTIONS - CMISP / PEMS
CMISP
The County Medically Indigent Services Program (CMISP) is a program of "last resort" designed to meet
the health care needs of individuals in our community who are not otherwise eligible for healthcare
programs such as Medi-Cal, Medicare or private health insurance, and who meet the County's "last
resort" socioeconomic eligibility standards. CMISP is a program mandated by the State of California, Title
17 of the Welfare and Institutions Code, to provide access to medical care for medically indigent persons.
The program is administered as a combined effort of the Sacramento County Departments of Health and
Human Services and Human Assistance. The Department of Health and Human Services includes the
Office of Medical Case Management under the direction of the Chief of Primary Health Services, Medical
Case Management is responsible for provider payments and patient billing in Accounting and Fiscal
Services for secondary and tertiary care.
PEMS
The Physicians Emergency Medical Services (PEMS) fund program is outlined in county code – TITLE 6
HEALTH AND SANITATION, Chapter 6.105 EMERGENCY, OBSTETRIC, PEDIATRIC MEDICAL
SERVICES DISTRIBUTION OF PENALTY ASSESSMENT AND TOBACCO TAX FUNDS.
PRIVACY AND SECURITY PROTECTION
This companion guide does not specifically address privacy and security protection regarding the use of
the system or application technology to send and receive a transaction set. For example, digital
certificates, authentication, authorization, and other access restrictions are not addressed in this
companion guide. This document assumes that the transaction exchange will take place in a processing
and communication environment that is secure at both ends for the senders and the receivers of data.
2. GETTING STARTED
PROVIDER REGISTRATION
Please make sure to Add 835 details in Provider registration document, if you Intend to receive 835 EDI
files from DHHS.
Note: The form is available via County Web Site or email, EDI-Dropbox@SacCounty.net.
3. 835 CONTROL SEGMENTS/ENVELOPE STRUCTURE
OVERVIEW
The communication envelope consists of an interchange envelope and functional groups. The
interchange control structure is used for inbound and outbound files. An outbound interchange control
structure is the envelope that wraps all transaction data (ST-SE) sent from DHHS. Examples include 835
and 997 transactions. An outbound interchange control structure wraps transactions that are created by
DHHS and returned to the respective providers, or clearinghouses. Examples of outbound transactions
include only 835 Remittance Advice, 997, and TA1 Acknowledgements only.
DHHS- SEGMENTS/LOOPS – TECHNICAL INFORMATION
a. The following list includes basic technical information for each transaction:
• The following delimiters are used for all Inbound transactions:
o * (asterisk) = data element separator
o : (colon) = sub element separator
o ~ (tilde) = segment terminator
b. DHHS will send one ST-SE per Functional Group.
c. DHHS will use following values for Claim Status in 835 file, “25” – Accepted, “4” – Rejected
(CLP Segment)
d. DHHS Assigned MRN numbers will be sent in 835 Transaction (REF Segment)
e. Claim Remark Codes will be included in the EDI file for rejected claims (LQ Segment)
HEADING, DETAIL AND SUMMARY TABLES
The following sections describe DHHS’s use of Header Interchange Control Segments (ISA-IEA),
Functional Group (GS-GE) and Transaction Segments (ST-SE).This implementation guideline identifies
all required segments for 835 transactions. A situational segment is not required for every type of
transaction; however, a situational segment may be required under certain circumstances.
Because most payers maintain the same Originating Company Identifier in the
TRN03/BPR10 for all product lines or contractual relationships, the CLP06 is used by
the provider as a table pointer in combination with the TRN03/BPR10 to identify the
unique, provider-specific insurance plan code needed to post the payment without
human intervention. The value should mirror the value received in the original claim (2-
005 SBR09 of the 837), if applicable, or provide the value as assigned or edited by the
payer.
An exception to note 1 occurs with institutional claims when the room per diem is the
