Management Services Agreement Between the
Florida Governmental Utility Authority ("FGUA"),
a legal entity and public body created by interlocal agreement pursuant to
Section 163.01(7), Florida Statutes
and
Government Services Group, lnc. ("GSG")
A. SUMMARY OF SERVICES TO BE RENDERED
Government Services Group, lnc. (GSG) will provide all necessary inspection services for the
MacDill AFB ISDC (lnitial System Deficiency Correction) project for WWTP Security to be
completed by U.S. Water services Corporation under work authorization USW 11-02.
The compensation to GSG for inspection costs related to this ISDC project equates to 5% of
the total project costs, which is consistent with the FGUA proposal to the Government for
MacDill AFB, and consistent with the budgeted amounts.
The inspection services to be provided by GSG shall include, but are not limited to:
t. Participation in preconstruction planning
2. Drawing Review
3. VisualSite lnspection
4. Project Progress Meetings
5. Verify that the contractor follows all safety standards as required by OSHA.
6. Work Site Documentation
7. Verification of Materials Used and Stored on Site
8. Verification of Equipment-Materials Used is in Accordance with Approved Plans
9. Witness all start-up and testing
10. ProjectDocumentationClose-out

Total Lump Sum Amount $ 6,956
This lump sum amount shall be paid in 4 equal monthly installments beginning in April
2}tt. Payment of this amount is contingent upon the FGUA receiving payments from the
Government as proposed for the construction of the designated ISDC projects.
