Desoutter Cast Cutter<br />

Removal of casts without a power saw is extremely hard work and very<br />

time consuming. This has implications for procedure costs and anaesthetic<br />

risk. The Desoutter saw is extremely reliable and makes short work of all<br />

cast materials. (The harder synthetics require the diamond blade).<br />

The <strong>Multi</strong> <strong>Saw</strong> is a multi-function oscillating saw which may be used as a<br />

cast cutter and a surgical oscillating saw. The standard rechargeable saw<br />

is supplied as a kit including charger, battery and Diamond blade.<br />

Our stainless attachment converts the multi saw into a surgical saw in a<br />

similar fashion to our orthopaedic drill. With the converter and shroud in<br />

place, all exposed surfaces are sterile. Excellent for total femoral head and<br />

neck excision, TPLO by Tibial Wedge Osteotomy and TTA. A wide range<br />

of blades fit the multi saw. We recommend either the Desoutter S82 series<br />

or the Halls 5071 series. The latter offers a slimmer cut 0.8mm c.f. 1.2mm.<br />

The well established <strong>Multi</strong> <strong>Saw</strong> is also available in a mains powered (240v)<br />

version. This plug in version has the advantage of reliability of power and<br />

significantly the speed of the saw is variable allowing a slow start speed.<br />

The disadvantage is that a flex is not convenient and care should be taken<br />

to eliminate electrical hazards in theatre.<br />

The option of a slow speed is also an advantage when used as a cast saw.<br />

The surgical attachment and blades are common to both battery and<br />

Slocum Blade Adaptor<br />

This adaptor converts our standard <strong>Multi</strong> <strong>Saw</strong> into a radial/bi-radial blade<br />

driver. The mount is in fact the old Stryker triangular mount. Please bear in<br />

mind the following, once mounted the saw sits at right angles to the long<br />

axis of the saw c.f. standard Slocum type saws. Additionally the cut is less<br />

aggressive in that the oscillation is 4˚ rather than the 6˚ of standard saws.<br />

The cut will, therefore, be slower. Ideal as a budget alternative or as a<br />

back up to your air saw.<br />

VI MiniDriver<br />

The MiniDriver Is Dead!! Long Live the MiniDriver!!<br />

3Ms Orthopaedic was sold to Linvatec who killed off the MiniDriver and<br />

the MaxiDriver as they competed with their own product range. The<br />

MiniDriver remains in widespread use and in many ways is the ideal<br />

small animal orthopaedic power system particularly when used with the<br />

Mark II saw. <strong>Veterinary</strong> <strong>Instrumentation</strong> is pleased to announce that we<br />

are having the entire MiniDriver range re-manufactured including the<br />

K220 which takes a larger blade than the original Mark I saw. In fact we<br />

have improved on some of the accessories. New MiniDriver sets are<br />

available from £2800. Some say the torque on the MiniDriver is too low.<br />

OK then buy the high torque chuck which more than triples the torque!<br />

achieved using ‘K’ or arthrodesis<br />

(‘A’) wires. Driving a long pin in a<br />

chuck often results in bending and<br />

kinking making driving difficult.<br />

Incrementally advancing the wire<br />

through a chuck is fiddly and time<br />

consuming. The wire driver allows<br />

the easy incremental advancement<br />

of the wire through the handpiece via the use of a clutch arrangement.<br />

The K120 sagittal saw uses a push button mechanism for blade<br />

attachment. It is relatively widely available and less expensive than the<br />

more robust Mark II K220.<br />

The Mark II saw is more robust in construction than the K120 saw. Blade<br />

attachment is by use of a hex wrench. While this gave a more rigid<br />

attachment of the blade to the saw it was also the weak point of the<br />

attachment. We have re-engineered the blade holding mechanism to<br />

make it stronger and made the whole attachment from stainless steel.<br />

The Mark II saw drives a larger ranger of blades including longer ones<br />

up to 42mm useful in the TTO and TTA procedures. Much more<br />

expensive than the Mark I version.<br />

MiniDriver Hose<br />

The standard hose is a 3 meter long double hose which vents back at<br />

the wall gas outlet or the regulator. In the UK the standard drive gas is<br />

bottle sterile Medical Air delivered at 7bar (100psi). Elsewhere in the<br />

world Nitrogen may be used. The standard hose supplied is fitted with<br />

an MA7 gas probe (Medical Air @ 7bar) which prevents connection to<br />

other gases e.g. Oxygen.<br />

Chuck keys wear quickly, become lost and can be awkward to use in some<br />

situations. The keyless chuck, as are found on most DIY drills allow the<br />

surgeon to quickly switch drills and pins without a key. The distal part of<br />

the chuck is held using fingers and the chuck opened or closed by<br />

engaging the drill in either the forward or reverse drive respectively. The<br />

downside is that the chuck is slightly bulkier than the keyed equivalent.<br />

Maxi Driver Modular System<br />

The MaxiDriver remains the<br />

workhorse of very many human<br />

and veterinary orthopaedic<br />

units. It is extremely reliable and<br />

almost infinitely repairable. It’s<br />

robust construction will tolerate<br />

a good deal of abuse.<br />

<strong>Veterinary</strong> <strong>Instrumentation</strong> is remanufacturing<br />

the handpieces<br />

and attachments and has taken<br />

steps to ensure that all spares<br />

are available or can be re-made.<br />

The New MaxiDriver has a full 1<br />

MAXL100R<br />

year warranty. The handpiece is<br />

significantly bigger than the<br />

MiniDriver (940g vs 480g). The modular system takes 15 quick release<br />

attachments. All attachments also available in new re-manufactured<br />

condition. Please phone for details. A huge range of saw blades is<br />

available making the MaxiDriver Kit an extremely good value<br />

introduction to orthopaedic power. MaxiDriver Kits are particularly<br />

All surgical burs have a 2.36mm shank (HP fitting) and will fit in all high speed surgical bur machines e.g. 3M's Minos, Halls Surgairtome, Foredom. We<br />

have selected Komet burs for their high quality. All are single pack and non sterile. In human surgery burs are regarded as disposable items. In veterinary<br />

surgery the cost precludes using burs as disposables. However, use of burs which are less than sharp extends procedure time and creates heat. We do<br />

advise that burs are checked and changed on a regular basis.<br />

Diamond burs are sharper and create less heat that carbide equivalents. Adequate irrigation is essential. Please see chart for options and prices.<br />

For maximum efficiency saw blades must be sharp, this is particularly true<br />

of radial and biradial blades which are inherently inefficient. It is not<br />

possible to sharpen a blade by hand to the same degree of sharpness as a<br />

computer controlled laser cutter. See above pictures.<br />

Blunt blades are the biggest source of TPLO complication through thermal<br />

Modular Stainless TPLO Blades<br />

Slocum TPLO blades create a patented biradial cut which means that the<br />

tibial plateau segment fits the distal tibia section exactly with maximum<br />

bone contact. While this is a good thing the design of the blade does not<br />

cut very efficiently and needs regular hand sharpening if excess heat is not<br />

to be produced. <strong>Veterinary</strong> <strong>Instrumentation</strong> has been looking at TPLO<br />

blades for a number of years and is introducing a range of Radial blades<br />

which cut better and are cheaper to replace than the Slocum original. They<br />

have the same triangular stryker connection.<br />

none of them cut as<br />

well as they should.<br />

In addition some of<br />

the blade lengths<br />

available did not<br />

match the clinical<br />

requirement. We<br />

have, therefore,<br />

designed our own.<br />

Currently available blades either have no ‘set’ or a one sided ‘set’. The ‘set’<br />

of a saw is the way that alternate teeth protrude outside the blade of the<br />

saw. The set, which is found on all woodworking and metal working saws,<br />

is there to create a slot in the material, wood, metal or bone, which is<br />

slightly larger than the blade so that the blade does not produce friction in<br />

the slot. Friction creates heat which although not serious in either wood or<br />

metal is very serious in bone. Thermal necrosis along the saw cut of TPLO<br />

osteotomies is a common cause of delayed union and failure. In addition to<br />

heat, friction leads to jamming of the blade in the slot which is both<br />

frustrating and destructive. VI TPLO blades have a full bilateral set for<br />

maximum efficiency.<br />

Blade length varies from 35mm in the 15mm blade to 65mm in the 30mm<br />

blade as requested by leading surgeons.<br />

A re-sharpening service is available and encouraged on a service<br />

exchange basis.<br />

The theoretical benefits of a bi-radial blade are well known. However, in<br />

practice the bi-radial blade is not very efficient at cutting bone even when new.<br />

After hand sharpening the efficiency falls still further. An inefficient blade<br />

creates heat. The thermal necrosis produced by the bi-radial blade neutralises<br />

any benefit derived from the bi-radial cut.<br />

Our titanium blades are light, thin, hard and replaceable. In addition the blades<br />

have a ’set’ (the teeth are alternately offset as in all wood cutting saws) which<br />

creates a slot slightly wider than the blade. This reduces the friction and heat<br />

still further as well as preventing the blade jamming in the osteotomy. When<br />

the blade becomes worn simply replace with new sharp blade.<br />

of the triangular Stryker type. The<br />

means of attachment to the saw is by<br />

hex drive grub screws which are<br />

notorious for working loose. Our ‘T’ bar<br />

easily and firmly attach the blade to the<br />

All the air equipment runs on Medical Air at 7 Bar (100PSI) Drills and saws<br />

can be run from compressors as their air requirement is intermittent. High<br />

speed drills need bottled gas or a big compressor. A twin dial regulator is<br />

best to control gas pressure (001756). One dial gives bottle pressure, the<br />

other one gives line pressure (7 Bar).<br />

UK supplied hoses come with an MA7 probe (Medical Air 7 Bar pressure).<br />

Schrader fittings on hoses purchased from the USA look similar to UK MA7<br />

probes. Many will however not lock into UK MA7 sockets. The best<br />

solution is to change the probe. A cheaper alternative is to use a<br />

Schrader/MA7 adaptor (001758).<br />

Quality pre-owned equipment typically costs a fraction of new. Buying<br />

second hand is often the only way to justify the purchase of capital<br />

equipment for orthopaedics and arthroscopy.<br />

Equipment may be purchased from Ebay or other auction houses but<br />

carries signficant risk. There is usually a reason why the previous owner is<br />

selling. We carefully source quality brands, check for function and service<br />

and repair as necessary. Everything comes with a six month warranty.<br />
