Less Invasive Procedures Give Vascular<br />

Patients New Options<br />

Less invasive procedures have become more common for the<br />

treatment of peripheral arterial disease (PAD), abdominal<br />

aortic aneurysm (AAA) and varicose veins. Minimally invasive<br />

procedures tend to have lower risk and easier recoveries<br />

than larger open operations. The impact has given patients<br />

new options:<br />

Men and women over the age of 60 who have<br />

smoked cigarettes, have a history of heart disease<br />

or a family member who has had an aortic<br />

aneurysm are at higher risk.<br />

{<br />

1. PAD patients enjoy many less risky options for<br />

management of their disease.<br />

2. Patients with AAA can be treated without the<br />

need of a major operation.<br />

3. Varicose vein patients are now almost exclusively<br />

treated in the office setting.<br />

Peripheral Arterial Disease or PAD<br />

PAD occurs when the arteries to the legs develop restrictions<br />

to blood flow due to atherosclerotic plaque. The most common<br />

risk factors of Atherosclerosis are related to cigarette smoking,<br />

high blood pressure, and high cholesterol. Restriction of blood<br />

flow to the legs can cause muscle pain that occurs with walking.<br />

When the restriction is severe, foot and toe pain can occur at<br />

rest or wounds can develop on the feet that do not heal.<br />

The most important point about PAD is that if<br />

atherosclerosis is present in the legs it may also<br />

be present in the arteries to the brain or heart,<br />

{thus indicating a risk for stroke or heart attack.<br />

Since the aorta is deep in our abdomens, it is not easy to feel so<br />

many patients do not know that they have an AAA. People at<br />

risk should ask their doctors to order a simple ultrasound.Some<br />

patients with AAA may need to have a major operation but many<br />

aortic aneurysms can now be repaired with a much less invasive<br />

procedure called endovascular aneurysm repair.<br />

VaRICOSe veins<br />

Large lumpy veins on the surface of the legs are often associated<br />

with pain, aching, heaviness of the legs and swelling. Varicose<br />

veins and venous insufficiency affects more individuals than most<br />

arterial problems combined. The treatment of varicose veins has<br />

undergone a drastic revolution to minimally invasive therapies.<br />

Once associated with vein stripping operations, most<br />

procedures are now performed in the office with local<br />

anesthesia. Endovenous closure with small catheters, minimal<br />

phlebectomy and sclerotherapy have replaced larger, more<br />

painful stripping procedures.<br />

Most patients with PAD need to only modify their lifestyles<br />

and begin an exercise program. For patients who need invasive<br />

therapy, newer technologies have allowed more patients to be<br />

treated with minimally invasive or endovascular procedures such<br />

as balloon angioplasty, stent placement or atherectomy. Some<br />

patients may require more extensive surgical procedures like leg<br />

artery bypass.<br />

Abdominal Aortic Aneurysm or AAA<br />

The aorta artery is our main artery. It starts at the heart and ends<br />

in the abdomen, giving rise to all the arteries in our bodies. In<br />

the lower abdomen, it splits in two supplying the main arteries to<br />

the legs. Some people develop a weakness in the elastic layer of<br />

the artery causing it to slowly expand over time. Like a balloon, if it<br />

gets too large it can rupture, causing internal bleeding and death.<br />

In the past five years, more than 300 men have undergone<br />

new cancer treatment with none of the damaging side<br />

effects of previous treatments. Innovative radiation therapy<br />

treatments at <strong>MIMA</strong> Cancer Center have achieved a 98<br />

percent success rate, with no return of cancer.<br />

Dennis French is one such patient. When Dennis was<br />

diagnosed with an aggressive form of prostate cancer, he<br />

was able to find hope close to home. <strong>MIMA</strong> Cancer Center<br />

is an integrated cancer center that offers board-certified<br />

surgeons, medical oncologists, radiation oncologists and<br />

some of Florida’s most advanced medical technology.<br />

“Prostate cancer surprised me. During a standard physical<br />

for my 50th birthday, I was diagnosed with an aggressive<br />

form – a Gleason 8,” said French. “Usually, that diagnosis<br />

comes without much hope. I heard about how toxic<br />

treatments could be and I was nervous about what I<br />

was facing.”<br />

French did what most people would do; he got a second<br />

opinion. “I did my research and visited cancer centers.<br />

It became clear that <strong>MIMA</strong> Cancer Center was the right<br />

place for me: the technology was more advanced, I felt<br />

confident in the expertise of the physicians, and I wouldn’t<br />

have to be out of town and away from my support system,”<br />

says French.<br />

Dennis was impressed with the 3D radiation capabilities of<br />

the Varian Trilogy - the technology used at <strong>MIMA</strong> Cancer<br />

Center that enables image-guided radiation therapy (IGRT).<br />

Physicians have the ability to precision-target the exact area<br />

of a tumor and not only provide treatment, but treatment<br />

with no damage to surrounding tissues. It is so precise that<br />

it can even accommodate for a patient’s breathing during<br />

treatment so that it always targets just the disease.<br />

“IGRT has given us incredible versatility in cancer treatment,<br />

particularly prostate cancer,” says Dr. Todd Scarbrough,<br />

medical director of the <strong>MIMA</strong> Cancer Center. “It’ s exciting<br />

to give patients an option that virtually eliminates any side<br />

effects – particularly those that men with prostate cancer<br />

fear the most such as ED, incontinence and rectal damage.<br />

We’re curing men everyday and they are remaining cancer<br />

free, going to work, and playing golf all during treatment!”<br />

adds Dr. Scarbrough.<br />

Dennis French agrees. “It’s amazing what modern radiation<br />

can do. I continued to participate in all the activities of my<br />

life,” says French. “It took only 30 minutes a treatment for 9<br />

weeks and I was done! ” he said. As one of the first patients<br />

to be treated at <strong>MIMA</strong> Cancer Center, Dennis is today active,<br />

working, and happily married, with no side effects from<br />

treatment and most importantly, cancer free.<br />

Dr. Lee Scheinbart, a medical oncologist with <strong>MIMA</strong> Cancer<br />

Center says the approach to patient care has changed<br />

dramatically. “What makes our center such a benefit to our<br />

patients is the services they receive from an entire team<br />

of professionals that collaborate on their care,” says Dr.<br />

Scheinbart. “From the primary care physician to the specialist<br />

(such as the urologist in Dennis’ case), to medical oncologists,<br />

radiologists, radiation oncologists, case managers and<br />

technical professionals, we meet regularly to consult with<br />

each other about the patient and together make decisions.<br />

Patients receive a thorough treatment plan from a large<br />

number of professionals that addresses medical, social and<br />

emotional needs,” adds Dr. Scheinbart.<br />

<strong>MIMA</strong> Cancer Center manages a large infusion center with<br />

highly trained pharmacy technicians, nurses and medical<br />

assistants all under the doctor’s supervision to provide stateof-the-art<br />

drug treatments, even participating in clinical trials<br />

for drugs under development in the fight against many types<br />

of cancer. Similarly, the radiation facilities are some of the<br />

most advanced in the US, with clinicians and scientists from<br />

leading academic and community cancer centers across the<br />

globe visiting the center on a routine basis to observe and<br />

learn about innovative new technologies and techniques<br />

being implemented at <strong>MIMA</strong>.<br />

The “Softer Side” of Cancer –<br />

Case Managers Provide Personalized Care<br />

Today more and more people are surviving a cancer diagnosis. With this in mind,<br />

care of cancer patients has moved beyond just the medical to encompass not only<br />

the whole patient but also the family.<br />

As a leader in cancer care in Brevard County, <strong>MIMA</strong> Cancer Center recognized<br />

this trend and developed Oncology Case Managers who serve multiple roles that<br />

include providing emotional support to patients and families, facilitating support<br />

groups, assisting the under-insured, and helping patients navigate the complexities<br />

of cancer care.<br />

As oncology case managers, <strong>MIMA</strong> Cancer Center has developed a program called<br />

the “Softer Side of Cancer” that highlights the psycho-social care of the patient<br />

that can often be missed in other centers. “We believe that cancer is difficult<br />

enough to cope with on its own so we try to ease as many other stressors as<br />

possible so the patient and family can focus on healing,” said Jane Lyons, <strong>MIMA</strong><br />

Cancer Center case manager. “Paul and I assist in the coordination of efficient<br />

scheduling for treatment plans and work with patients and their loved ones in<br />

assessing their needs in order to make necessary referrals.” This includes helping<br />

patients obtain assistance from community, pharmaceutical, and professional<br />

agencies, as well as helping patients and their families cope with cancer through<br />

a variety of support groups.<br />

“It’s almost hard to imagine that something like cancer could have a ‘soft’ side,<br />

but I know from experience that it exists,” said Walt M., whose wife Pat underwent<br />

treatment at <strong>MIMA</strong>’s Cancer Center. “The care and advice of nurses who have a<br />

genuine calling to what they do; the reassuring touch and concern; the confidence<br />

of great doctors; all combine to soften something that is otherwise so harsh. I<br />

think this combination is why Pat is alive today. Without this type of care, I can see<br />

why some patients would do poorly.“<br />

Healthcare in Brevard County<br />

In 1968, subspecialty medicine was well<br />

recognized in the US and the founding<br />

<strong>MIMA</strong> physicians were practicing<br />

subspecialty consultants who wanted<br />

As we grow, we take on new responsibilities and roles. This<br />

includes the creation of The <strong>MIMA</strong> Foundation, a not-forprofit<br />

subsidiary with growing activities in the community.<br />

We have distributed over $100,000 in scholarships to local<br />

area residents, and expanded activities to allow us to directly<br />

help less fortunate patients. The annual <strong>MIMA</strong> Classic Tennis<br />

tournament, a partnership with the Kiwi Tennis Club, is a<br />

major stop on the USTA women’s professional tour. All of<br />

the money distributed by the <strong>MIMA</strong> Foundation goes to<br />

the citizens of South Brevard County, and none of it is<br />

used to pay us for medical care or facilities. Additionally,<br />

our physicians donate more free medical care to the<br />

citizens of South Brevard than all other physicians in the<br />

area combined.<br />

Another developing role for <strong>MIMA</strong> is community outreach.<br />

In recent years we have added social workers and taken<br />

on many of the community programs traditionally run<br />

by hospitals, including the American Cancer Society<br />

Community Closet, “For Ladies Only” support groups,<br />

“Cancer 101” education and resource group, and sponsorship<br />

of “Look Good, Feel Better.”<br />

<strong>MIMA</strong> opens new medical<br />

facility in malabar<br />

<strong>MIMA</strong>’s newest medical facility in Malabar next to<br />

Palm Bay Community Hospital will give new access for<br />

residents in south Brevard to physicians, laboratory<br />

services, and diagnostic services. “We are committed<br />

to more efficient, comprehensive and convenient access<br />

of services to more of the residents in Brevard,” says<br />

Joseph McClure, M.D., chief executive officer and <strong>MIMA</strong>’s<br />

chairman of the board. Its proximity to the greater Palm<br />

Bay and Malabar area, along with being next door to Palm<br />

Bay Community Hospital, gives physicians and patients<br />

alike, added convenience and access to cutting-edge healthcare.<br />

The 36,000-square-foot facility will feature a two-story centralized<br />

reception room, retail kiosk, 42 exam rooms, up to 12 physicians, four<br />

procedure rooms, and state-of-the-art diagnostic services. Patients can<br />

now access medical services in south Brevard including laboratory tests,<br />

Cardiology testing, Nuclear Cardiology, CT (including Cardiac CT), Open<br />

Advanced Pulmonary Function<br />

Test Services at <strong>MIMA</strong><br />

Over the past 30 years, pulmonary function testing has<br />

come into widespread use. Pulmonary function tests (PFT)<br />

are breathing tests that find out how well you move air in<br />

and out of your lungs, your lung capacity, and how well<br />

oxygen enters your body.<br />

Michael Ott, M.D., board-certified specialist in pulmonary<br />

diseases, critical care, and undersea and hyperbaric medicine, along<br />

with Drs. Prakash Reddy, Theodore Amgott, and Jerry Hung have<br />

created one of Central Florida’s most comprehensive PFT Laboratories.<br />

Complete with two PFT testing stations, two smaller fixed stations,<br />

and a portable station, the lab performs spirometry (blowing hard into<br />

a tube connected to a machine), diffusion studies (a series of breaths<br />

into a tube and holding your breath) and plethysmography (measuring<br />

how much air remains in your lungs when breathing in and out). For<br />

patients unable to enter the testing chamber for plethysmography, an<br />

alternative method is also available involving estimates of lung capacity<br />

from breathing inert gas.<br />

In order to provide more expanded care, and to meet the needs of scuba<br />

divers (a keen interest of his), Dr. Ott has built an advanced diagnostic<br />

PFT test lab that can only be found at <strong>MIMA</strong>.<br />

One such test offered is the HAST – High Altitude Simulation Test.<br />

Designed to simulate the conditions in higher altitudes during airplane<br />

travel, patients are tested to measure their tolerance to increased altitudes<br />

to find out if they need additional oxygen. Patients with heart or lung<br />

disease can now more easily check their oxygen levels and if needed,<br />

utilize portable, battery-operated oxygen concentrators that have made<br />

travel easier and less expensive in recent years.<br />

Dr. Ott has also put together a shunt study, which checks blood oxygen<br />

levels under different conditions to assess whether oxygen may be<br />

by-passing the normal pathway. Using such tests as the methacholine<br />

challenge, exercise challenge, saline challenge, and cold air challenge, the<br />

lab can simulate many different conditions that might lead to asthma and<br />

reduced lung function.<br />

This advanced level PFT laboratory is a valuable service for the<br />

community by giving area patients and physicians a unique range of<br />

services not available anywhere else in the area. For more information,<br />

A Mission to Honduras<br />

Along with half a dozen other<br />

central Florida physicians, Drs.<br />

Peter Marzano and Jim Ronaldson<br />

traveled to San Pedro Sula<br />

Honduras for a medical mission trip.<br />

Sponsored through Rotary Club<br />

of Orlando, physicians get an opportunity to serve<br />

residents in Honduras with much needed medical<br />

care by highly qualified doctors.<br />

The second trip for both Dr. Marzano and Dr.<br />

Ronaldson, they cared for patients of all ages,<br />

most often dealing with conditions such as scabies,<br />

intestinal parasites, and upper respiratory infections.<br />

“Seeing the tremendous need for health care in this<br />

little village, as well as how much help we could provide<br />

in such a short period of time, reinforced why I became<br />

a physician in the first place,” said Dr. Marzano.<br />

Over the course of just two<br />

days, four physicians cared<br />

for more than 574 patients.<br />

One day of the mission<br />

the physicians made their<br />

way through the jungle to a<br />

very primitive site with no<br />

electricity or running water.<br />

“These people have absolutely<br />

no access to healthcare. They<br />

live in such depressed conditions, and bring their<br />

whole families…walking a day to get there,” said Dr.<br />

Marzano. “It is very rewarding to help people who are<br />

unable to help themselves.” Dr. Marzano adds that he<br />

and his wife Linda, a nurse, plan to return every year.<br />

“We are proud of the many <strong>MIMA</strong> physicians, like<br />

Dr. Marzano and Dr. Ronaldson, that serve our local<br />

community and travel abroad to participate in<br />

programs such as this,” said Al O’Connell, <strong>MIMA</strong>’s chief<br />
