Bipolar coagulation with ERBE BiClamp is an established electrosurgical<br />

procedure for the safe coagulation of vascularized tissue structures<br />

such as the mesentery, the peritoneum or the omentum. In gynecology,<br />

the BiClamp procedure allows a new innovative technique for vaginal<br />

hysterectomy to be carried out. Coagulation with BiClamp means that<br />

the preparation and dissection of individual vessels can usually be<br />

dispensed with. Additional ligatures or conventional coagulation are<br />

generally not required. The procedure is quick, reliable and effective.<br />

It is also economical as the instruments are re usable and the procedure<br />

saves suture material and clips.<br />

The VIO electrosurgical system supports bipolar coagulation with ERBE<br />

BiClamp with its special patented hardware and software which were<br />

specifically developed for use with BiClamp, providing waveforms and<br />

voltages as required for different procedures – especially adjusted to<br />

this application technique. You do not need a separate unit in addition<br />

to the ERBE VIO System, you only need additional software.<br />

Operating principle<br />

Coagulation with BiClamp is essentially based on two mechanisms. Firstly, there are<br />

the specially tested bipolar waveforms and electrically voltages, produced by the new<br />

electrosurgical generator of the ERBE VIO System. Then, the particular features of the<br />

ERBE BiClamp create a parchment-like coagulation seam during bipolar coagulation, an<br />

indication that the vessels have been properly sealed.<br />

Surgical procedure<br />

The tissue which is to be separated is grasped with the BiClamp instrument.<br />

Using the ERBE electrosurgical VIO System a bipolar electrosurgical<br />

current – with an optimally adjusted waveform, voltage and current<br />

density – is applied and the tissue is coagu lated.<br />

The patented AUTO STOP function auto matically stops the coagulation<br />

process when optimal coagulation is achieved. Lateral thermal damage is<br />

thus avoided.<br />

The instrument is then opened and the tissue is separated mechanically<br />

down the centre of the visible coagulation area. In some cases it may<br />

be help ful to carry out two BiClamp<br />

applications, one next to the other, and<br />

then to cut the tissue between the coagulation<br />

seams. Coagulation with BiClamp is<br />

so effective that an additional ligature or<br />

coagulation is usually not required.<br />
