This agreement is made between <strong>USF</strong> Dining Services and the <strong>Meal</strong> <strong>Plan</strong> payer and recipient, who agree to the following:<br />
Payment Provisions<br />
<strong>USF</strong> <strong>Meal</strong> <strong>Plan</strong> <strong>Contract</strong> - <strong>Summer</strong> A <strong>2012</strong><br />
1. A $29 administrative fee will be charged for any returned checks.<br />
2. A <strong>Meal</strong> <strong>Plan</strong> deferment is available only for students receiving financial aid through the university in the form of scholarships,<br />
accepted loans or grants. A minimum dollar amount of financial aid eligibility will be established by dining services each<br />
semester to qualify for a <strong>Meal</strong> <strong>Plan</strong> deferment. Students eligible for a <strong>Meal</strong> <strong>Plan</strong> deferment must pay a one-time $25 nonrefundable<br />
administrative fee, which is due with the signed contract. If signing up for a <strong>Summer</strong> A & B meal plan deferment, a one-time $25<br />
non-refundable administrative fee is due at the time of sign up for both <strong>Summer</strong> A & B meal plans. This administrative fee is not<br />
applied to the <strong>Meal</strong> <strong>Plan</strong>’s total price. The total balance is due no later than Friday, June 11, <strong>2012</strong>. Your <strong>Meal</strong> <strong>Plan</strong> will be placed<br />
on hold if full payment is not received by the above due dates.<br />
3. Unpaid balances will subject recipients to University Academic Hold status, which may prevent them from registering, receiving<br />
transcripts and/or graduating. You will not receive an invoice for any balances owed.<br />
4. You must submit payment at sign up to <strong>USF</strong> Dining Services in person, by phone, mail or online (see contact info below). You will not<br />
receive an invoice for any balances owed. <strong>Summer</strong> A meal plan payment must be made prior to activation.<br />
5. You may upgrade a <strong>Meal</strong> <strong>Plan</strong> at any time in the semester. Any additional money owed is due at the time of upgrade.<br />
6. Cancellations After Grace Period: All requests for <strong>Meal</strong> <strong>Plan</strong> cancellations must be accompanied by documentation from <strong>USF</strong> that you<br />
have withdrawn from the university. Refunds will be prorated for all <strong>Meal</strong> <strong>Plan</strong>s, including block plans and Dining Dollars accounts, from<br />
the date <strong>USF</strong> Dining Services receives such documentation. All cancellations, regardless of reason, are subject to a $50 cancellation fee.<br />
If you officially withdraw from the university and you notify us prior to the start of the academic semester, your <strong>Meal</strong> <strong>Plan</strong> contract will be<br />
canceled and you are entitled to a full refund. If you officially withdraw from the university after the start of the academic semester, or<br />
inform us of your withdrawal after the academic semester has begun, you receive a prorated refund.<br />
7. <strong>Summer</strong> <strong>Meal</strong> <strong>Plan</strong> purchase does not apply to the university-mandated <strong>Meal</strong> <strong>Plan</strong> requirement for first-time residents during Fall and<br />
Spring semesters.<br />
<strong>Meal</strong> <strong>Plan</strong> Provisions<br />
1. Your <strong>USF</strong> ID, which serves as meal card, is required for all transactions - no exceptions.<br />
2. <strong>Meal</strong> <strong>Plan</strong>s are not transferrable to others. <strong>Plan</strong> participant may bring in guests with the Dining Dollars portion of their account only.<br />
3. Unused Dining Dollars expire Saturday, August 4, <strong>2012</strong>.<br />
4. The meal week runs from Wednesday breakfast to Tuesday dinner. Unused meals on the Any 15 plans do not carry forward to the next<br />
week. <strong>Meal</strong>s reset every Wednesday morning.<br />
5. Participants are entitled to one meal per meal period (does not apply to Dining Dollars). <strong>Summer</strong> hours listed on this contract are subject to change.<br />
I. Juniper Dining Hours of Operation: Monday-Friday 7:00 a.m.-7:00 p.m. Saturday- Sunday 10:00 a.m.- 7:00 p.m.<br />
6. Unused meals on the BULLBlock 60 expire Friday, June 22, <strong>2012</strong> with lunch. <strong>Meal</strong>s cannot be used for guests.<br />
7. Carry-out is available and is provided to you as a courtesy. You must tell the cashier you want your meal carried out at the time you<br />
enter the facility. You may not take a seat in the facility once you have your carry-out box. No double portions on any one item. Only one<br />
beverage cup is allowed, and must be used for beverages only. The carry-out box must be fully closed when you leave (no filling up lid<br />
and base separately). If you require unlimited seconds, you must dine in. Failure to follow these rules will result in the forfeiture of your<br />
carry-out privileges.<br />
8. If your ID card is lost or stolen, you must report it to the Dining Services office and the <strong>USF</strong> Card office, located in the Marshall Center.<br />
You will receive a temporary card for up to five days at a fee of $10. You must give us a new card number within the five days, or your<br />
<strong>Meal</strong> <strong>Plan</strong> will be placed on hold. You will receive your $10 back if you return the temporary card.<br />
9. <strong>Meal</strong>s start Friday, May 11, <strong>2012</strong> with Dinner at Juniper Dining. The Fresh Food Company facility will be closed for the <strong>Summer</strong> <strong>2012</strong><br />
semester. <strong>USF</strong> Dining Services reserves the right to alter services or hours of operation. Notice will be given with modified schedules<br />
posted in all dining locations and online. Dining halls may be open prior to residence hall opening dates – entry during those times by<br />
cash or Dining Dollars only. The <strong>Meal</strong> <strong>Plan</strong> ends Friday, June 22, <strong>2012</strong> with Lunch.<br />
