填 寫 證 券 借 用 及 借 出 協 議 登 記 表 格 須 知
( 一 ) 如 屬 個 人 , 請 填 寫 香 港 身 分 證 號 碼 ( 如 無 身 分 證 , 則 請 填 寫 護 照 號 碼 ) 。 如 屬 公 司 , 請 填
寫 商 業 登 記 號 碼 , 如 有 的 話 。
( 二 ) 本 表 格 必 須 由 借 用 人 填 寫 及 簽 署 。 全 部 姓 名 必 須 以 英 文 正 楷 填 寫 。
( 三 ) 借 方 必 須 遞 交 下 列 文 件 , 以 便 辦 理 登 記 手 續 :
(i) 證 券 借 用 及 借 出 協 議 的 簽 立 文 本 一 份 和 該 協 議 的 認 證 真 確 副 本 一 份 或 該 證 券 借
用 及 借 出 協 議 的 認 證 真 確 副 本 兩 份 ; 及
(ii) 填 妥 的 登 記 表 格 兩 份 ; 及
(iii) 開 列 登 記 費 用 港 幣 二 佰 七 十 元 正 的 支 票 , 抬 頭 書 明 祈 付 「 香 港 特 別 行 政 區 政
府 」。
( 四 ) 登 記 表 格 上 借 用 人 的 地 址 將 會 被 用 作 有 關 本 借 用 及 借 出 協 議 的 通 訊 地 址 。
Notes for completing the Stock Borrowing and Lending Agreement Registration Form
(1) For individuals, please enter Hong Kong Identity Card Numbers (or if not available Passport
Numbers). For Corporations, please state Business Registration Numbers, if any.
(2) This form must be completed and signed by the borrowers. All names should be entered in
English and in BLOCK LETTERS.
(3) For registration, Borrowers are required to submit :
(i) an executed copy of the Stock Borrowing and Lending Agreement and a certified true
copy of the agreement OR two certified true copies of the Stock Borrowing and Lending
Agreement, AND
(ii) 2 copies of this Registration Form, duly completed, AND
(iii) a cheque for payment of the registration fee of HK$270.00 in favour of "The
Government of the Hong Kong Special Administrative Region".
(4) All future correspondence will be sent to the address of the borrower which appears in the
Registration Form.
