Dear Parents,
HOMEWORK REQUEST FORM
As homework is meant to reinforce what is covered during the
class day, the faculty and administration strongly advise that
students be given time to rest and recover when they are ill, and
therefore extends the due dates for assignments students miss due
to illness. For each day a student is absent, he/she will receive a
one-day extension.
In the instance that you would like for your student to receive
his/her homework during an absence, you will need to do the
following:
• Report your student’s absence by 9:00 a.m. each day
• Request homework for your student – homework is not
automatically prepared
• Complete and return the form on the reverse side for your
student’s homeroom teacher prior to the first day of school
Please write down or keep a copy of this request. Homeroom
teachers are responsible for distributing homework when
requested. The school office will not have a copy of your
homework requests. Additional homework request forms are
available at www.stpiuslombard.org/school under the Parents tab.
The form on the reverse side of this letter should be returned to the
school office prior to the first day of school.
