Apoptosis in radical prostatectomy specimens from treated patients was<br />

significantly higher than in c<strong>on</strong>trol subjects (p = 0.0018), specifically<br />

in regi<strong>on</strong>s of low to moderate-grade cancer (Gleas<strong>on</strong> grade 1–3). No adverse<br />

events related to the treatment were reported (18).<br />

Pharmacokinetics and pharmacodynamics<br />

The pharmacokinetics of red clover isoflav<strong>on</strong>es were investigated after<br />

l<strong>on</strong>g-term administrati<strong>on</strong> as a <strong>on</strong>ce-daily dietary supplement. Fourteen<br />

subjects who had been c<strong>on</strong>suming a low-isoflav<strong>on</strong>e c<strong>on</strong>taining diet for<br />

2 weeks were given an oral dose of two isoflav<strong>on</strong>e tablets (approximately<br />

80 mg of total isoflav<strong>on</strong>es) daily for 2 weeks. Before the study day the<br />

participants fasted overnight and were then administered their last dose<br />

the next morning. Plasma samples were collected for a 48-hour period<br />

after the last dose. Plasma isoflav<strong>on</strong>es were assayed by high-performance<br />

liquid chromatography. The results dem<strong>on</strong>strate that trough plasma levels<br />

were significantly higher for daidzein and genistein after l<strong>on</strong>g-term<br />

dosing than levels recorded prior to the commencement of the study and<br />

plasma levels of isoflav<strong>on</strong>es after l<strong>on</strong>g-term dosing were in the range previously<br />

reported in populati<strong>on</strong>s that c<strong>on</strong>sume an isoflav<strong>on</strong>e-rich diet. The<br />

plasma half-lives observed after l<strong>on</strong>g-term administrati<strong>on</strong> were c<strong>on</strong>sistent<br />

with <strong>on</strong>ce-daily administrati<strong>on</strong>. Isoflav<strong>on</strong>es have pharmacokinetic characteristics<br />

that suggest that <strong>on</strong>ce-daily administrati<strong>on</strong> is adequate when<br />

they are administered l<strong>on</strong>g-term (40).<br />

The absorpti<strong>on</strong> of isoflav<strong>on</strong>es varies substantially between individuals.<br />

A single-blind, randomized, placebo-c<strong>on</strong>trolled, cross-over trial<br />

involving 14 subjects investigated whether isoflav<strong>on</strong>e absorpti<strong>on</strong> differs<br />

depending <strong>on</strong> whether the isoflav<strong>on</strong>es originated from soy beans or<br />

from red clover. Soy bean isoflav<strong>on</strong>e glycosides and red clover isoflav<strong>on</strong>e<br />

aglyc<strong>on</strong>es were incorporated into a breakfast cereal and eaten<br />

daily for 2 weeks each, separated by a 2-week c<strong>on</strong>trol or washout period.<br />

The excreti<strong>on</strong> of isoflav<strong>on</strong>es in urine was measured over a 24-hour<br />

period; approximately 25% of each isoflav<strong>on</strong>e was recovered in urine,<br />

suggesting that similar amounts were absorbed irrespective of their<br />

glycoside/aglyc<strong>on</strong>e nature or the differing compositi<strong>on</strong>s of their sources<br />

(daidzein and genistein in soy beans and form<strong>on</strong><strong>on</strong>etin and biochanin<br />

in red clover). Although interindividual variability was high, there was<br />

less intraindividual variability; the amounts excreted when subjects<br />

c<strong>on</strong>sumed the two sources of isoflav<strong>on</strong>e were correlated (r = 0.69;<br />

p = 0.007) (41).<br />
