F<strong>ru</strong>ctus Momordicae<br />

Adverse reacti<strong>on</strong>s<br />

The reported adverse effects of the c<strong>ru</strong>de d<strong>ru</strong>g include hypoglycaemic<br />

coma and c<strong>on</strong>vulsi<strong>on</strong>s in children, increases in -glutamyl transferase and<br />

alkaline phosphatase levels, and headaches (53).<br />

C<strong>on</strong>traindicati<strong>on</strong>s<br />

Owing to potential abortifacient effects and possible teratogenicity (54,<br />

55), the seeds of the c<strong>ru</strong>de d<strong>ru</strong>g should not be taken during pregnancy.<br />

Owing to reported adverse events such as severe hypoglycaemia and<br />

c<strong>on</strong>vulsi<strong>on</strong>s in children, the c<strong>ru</strong>de d<strong>ru</strong>g and its preparati<strong>on</strong>s should not be<br />

administered to children or taken during breastfeeding (53).<br />

Warnings<br />

Patients with liver disorders should seek advice from their health care<br />

professi<strong>on</strong>al before taking any c<strong>ru</strong>de d<strong>ru</strong>g preparati<strong>on</strong>.<br />

Precauti<strong>on</strong>s<br />

D<strong>ru</strong>g interacti<strong>on</strong>s<br />

The f<strong>ru</strong>it and preparati<strong>on</strong>s thereof may have additive effects when taken<br />

with other glucose-lowering agents; however these interacti<strong>on</strong>s need to<br />

be investigated (53).<br />

Pregnancy: teratogenic effects<br />

Momorcharins, isolated from the seeds of the c<strong>ru</strong>de d<strong>ru</strong>g have been<br />

shown to induce early and midterm aborti<strong>on</strong>s in mice and were teratogenic<br />

in cultured mouse embryos at the early organogenesis stage. Morphological<br />

abnormalities were observed in the head, t<strong>ru</strong>nk and limbs of<br />

the embryos (54, 55).<br />

The genotoxic potential of extracts of the c<strong>ru</strong>de d<strong>ru</strong>g was assessed in<br />

the Salm<strong>on</strong>ella typhimurium microsome activati<strong>on</strong> assay and the alkaline<br />

single-cell gel electrophoresis (COMET) assay. The extract did not produce<br />

a positive resp<strong>on</strong>se in strains TA98 and TA100 with or without metabolic<br />

activati<strong>on</strong>, but produced an increase above negative c<strong>on</strong>trol values<br />

in the COMET assay (56).<br />
