insecticides in use in 200 sites, which will be selected to be representative of<br />
the malaria-ecological patterns in the country. Each year around 40-50 such<br />
sites covering all eco-type zones will be covered with states Entomologist unit<br />
and field stations of NIMR and VCRC. This study will help <strong>NVBDCP</strong> to evolve a<br />
comprehensive insecticide resistance map for the country and will help betterinformed<br />
insecticide policies. DDT continues to be used for kala-azar<br />
elimination. To date widespread resistance has not been reported but<br />
continued vigilance to determine development of resistance is necessary.<br />
Protocols will be developed by RMRI and in collaboration with the Regional<br />
Director. Insecticide resistance for KA vector will be undertaken in at least 3<br />
sites every year and these sites would be selected in consultation with the<br />
affected states.<br />
(e) Pharmaco-vigilance focusing on the first-line ACT and Miltefosine.<br />
As ACT and Miltefosine are newly adopted in country on a large scale, it is<br />
important to monitor safety in the program conditions. In due course, new<br />
partner drugs may be considered for ACT and new promising drugs on Kalaazar<br />
may be added in the list of first line drugs for kala-azar treatment. A<br />
protocol for prospective monitoring, coordinated with drug susceptibility<br />
testing, in 5 sites has therefore been prepared by NIMR and similarly<br />
pharmaco vigilance would be undertaken in 3 districts included under the kalaazar<br />
elimination program by RMRI.<br />
(f) Operational research and impact evaluation<br />
A list of priorities for operational research (malaria & kala-azar) under this<br />
project has been established. The operational research projects will be carried<br />
out by research institutes based in country, where appropriate, in collaboration<br />
with overseas partners. <strong>NVBDCP</strong> /Regional offices will be co–investigator/<br />
facilitator for all operational research projects. The list includes:<br />

Evaluation of different delivery models in public private partnerships<br />
including private providers for curative services in malaria control and kalaazar<br />
elimination;<br />
Cost effective active case detection for malaria and kala-azar;<br />
Treatment of PKDL;<br />
Research on shorter duration treatment of kala-azar with combination<br />
drugs;<br />
Increasing the access to diagnostic and treatment services to the poor and<br />
people living in inaccessible areas;<br />
Environmental studies and impact of climate change in relation to disease<br />
