assessment and therapy for<br />
adults with neurological and<br />
physical disability.<br />

THE ADULT COMMUNITY<br />
REHABILITATION TEAM<br />
Team members include:<br />
• Occupational Therapists<br />
Work with people to increase their<br />
ability to carry out every day tasks, e.g.<br />
cooking, returning to work, coping with<br />
memory difficulties, managing fatigue.<br />
• Physiotherapists<br />
Help to improve walking and moving<br />
independently. Provide training and advice<br />
for physical, respiratory and neurological<br />
symtoms for clients and their family/carers.<br />
• Speech & Language Therapists<br />
Develop people’s ability to communicate through<br />
speech or alternative systems. Also provide<br />
help with eating and drinking difficulties.<br />
• Disability Counsellor<br />
An opportunity to explore feelings and problems<br />
in a creative, supportive and confidential way.<br />
• Rehabilitation Assistants<br />
Assist therapists to carry out<br />
treatment programmes.<br />
• Clinical Psychology & Neuropsychology<br />
Specialist assessment and treatment for people<br />
who have suffered changes in their behaviour,<br />
emotions, and/or thinking skill (e.g. memory)<br />
• Consultant Allied Health Professional in<br />
Neurological Rehabilitation<br />
Provides assessment and makes<br />
recommendations for people who require<br />
specialist neurological rehabilitation packages<br />
in hospital or at home.<br />
What we do:<br />
The team has two specialties:<br />
1) Neurological Conditions<br />
2) Physical Conditions<br />
People are seen in the most appropriate<br />
place for them. This may be at home, at<br />
work, college or on site at St Leonard’s.<br />
A free transport service is available.<br />
Following a thorough assessment and discussion,<br />
we work with people to identify therapy goals<br />
and offer tailor made rehab programmes.<br />
We usually work with people for around six<br />
sessions before reviewing their goals with<br />
them. This allows us to assess progress<br />
and discuss plans for the future. All clients<br />
seen by the team are provide with a<br />
summary of the input they recieved.<br />
We will involve any <strong>relevant</strong> person involved<br />
in your care to help enhance your rehab.<br />
Some examples of our work:<br />
• Giving advice to help people cope<br />
with specific problems or crises.<br />
• Group to help balance<br />
• Group to help COPD<br />
• Helping people make choices about<br />
their disability and coming to terms<br />
with the impact of their disability<br />
• Helping increase someone’s confidence and<br />
independence in looking after themselves<br />
• Supporting carers and family members<br />
• Helping people to access community<br />
<strong>services</strong> such as leisure centres<br />
and voluntary organisations<br />
• Preventing admission to hospital<br />
• Helping people to return home<br />
following a hospital admission.<br />
How to get the service:<br />
• Any health or social care professional<br />
can refer to the service. However if you<br />
are not known to the service, a GP or<br />
Consultant referral may be required.<br />
• People with a progressive neurological<br />
condition e.g. Multiple Sclerosis or<br />
Parkinson’s disease, who are known<br />
to the team can self refer.<br />
• People who are referred to the team must be<br />
aged 16 years or over, live in City & Hackney<br />
and be ready and motivated to participate<br />
fully in a goal directed rehabilitation process.<br />
