adults with neurological and
physical disability.

REHABILITATION TEAM
Team members include:
• Occupational Therapists
Work with people to increase their
ability to carry out every day tasks, e.g.
cooking, returning to work, coping with
memory difficulties, managing fatigue.
• Physiotherapists
Help to improve walking and moving
independently. Provide training and advice
for physical, respiratory and neurological
symtoms for clients and their family/carers.
• Speech & Language Therapists
Develop people’s ability to communicate through
speech or alternative systems. Also provide
help with eating and drinking difficulties.
• Disability Counsellor
An opportunity to explore feelings and problems
in a creative, supportive and confidential way.
• Rehabilitation Assistants
Assist therapists to carry out
treatment programmes.
• Clinical Psychology & Neuropsychology
Specialist assessment and treatment for people
who have suffered changes in their behaviour,
emotions, and/or thinking skill (e.g. memory)
• Consultant Allied Health Professional in
Neurological Rehabilitation
Provides assessment and makes
recommendations for people who require
specialist neurological rehabilitation packages
in hospital or at home.
What we do:
The team has two specialties:
1) Neurological Conditions
2) Physical Conditions
People are seen in the most appropriate
place for them. This may be at home, at
work, college or on site at St Leonard’s.
A free transport service is available.
Following a thorough assessment and discussion,
we work with people to identify therapy goals
and offer tailor made rehab programmes.
We usually work with people for around six
sessions before reviewing their goals with
them. This allows us to assess progress
and discuss plans for the future. All clients
seen by the team are provide with a
summary of the input they recieved.
We will involve any relevant person involved
in your care to help enhance your rehab.
Some examples of our work:
• Giving advice to help people cope
with specific problems or crises.
• Group to help balance
• Group to help COPD
• Helping people make choices about
their disability and coming to terms
with the impact of their disability
• Helping increase someone’s confidence and
independence in looking after themselves
• Supporting carers and family members
• Helping people to access community
services such as leisure centres
and voluntary organisations
• Preventing admission to hospital
• Helping people to return home
following a hospital admission.
How to get the service:
• Any health or social care professional
can refer to the service. However if you
are not known to the service, a GP or
Consultant referral may be required.
• People with a progressive neurological
condition e.g. Multiple Sclerosis or
Parkinson’s disease, who are known
to the team can self refer.
• People who are referred to the team must be
aged 16 years or over, live in City & Hackney
and be ready and motivated to participate
fully in a goal directed rehabilitation process.
