into the bank account of the Manager (bank charges will be imposed).
I hereby confirm that I have read, acknowledge and agree to all the Remarks set out on page 6 of this form.
If no Fidelity Account under my name can be reasonably identified according to the personal data provided under this form, or the identified
Fidelity Account is not operative as considered by the Manager, I further acknowledge and agree that:
1. I would like the Manager to contact me by using my contact details as stated in Section A to facilitate the account opening and/or
payment process(es);
2. any incomplete or inaccurate information may cause delay; and
3. if an operative Fidelity Account under my name is not available by 5 working days after receipt of this form by the Trustee or in the case
of retirement, by 5 working days after I become entitled to payment of accrued benefits, whichever is later, all of my accrued benefits
will be paid by cheque in Hong Kong Dollar to be sent to my correspondence address as stated in Section A. The payment will be
made within the applicable period as specified in the Mandatory Provident Fund Schemes (General) Regulation.

累 算 權 益 申 索 表 格 ( 第 MPF(S) - W 號 表 格 ) 填 報 須 知
1) For claims of payment on grounds of death, only the personal representative within the meaning of the Mandatory Provident Fund Schemes Ordinance
may act on behalf of the deceased scheme member to claim for payment of the member's accrued benefits. This includes a personal representative
within the meaning of the Probate and Administration Ordinance (Cap. 10) and the Official Administrator who gets in and administers an estate of a
deceased scheme member in a summary manner without a grant or other legal formality under section 15 of that Ordinance. If there is more than
one personal representative and the personal representatives have not authorized one of the representatives to act on behalf of other representatives
to lodge the claim, all the personal representatives should submit the Claim Form jointly. Please use additional blank sheet to provide details of the
claimants under Section A. Under such circumstances, this Form needs to be signed by all of the personal representatives. For claims of payment on
grounds of total incapacity, either the member or a committee of estate / guardian appointed under the Mental Health Ordinance (Cap. 136) to act on
behalf of the member may lodge the claim for payment of accrued benefits.

(2) Claimants or scheme members should give their passport numbers ONLY when they do NOT possess HKID cards.

(3) If a claimant/scheme member has more than one account in a registered scheme, the claimant should fill in one form for payment of accrued benefits
in respect of all accounts within one scheme. If a claimant/scheme member has accounts in more than one scheme, the claimant should fill in one
form for each scheme.
如 果 申 索 人 / 計 劃 成 員 在 同 一 個 註 冊 計 劃 內 擁 有 超 過 一 個 帳 戶 , 則 在 提 出 支 付 累 算 權 益 的 申 索 時 , 只 須 就 該 等 同 屬 一 個 計 劃 的 所 有 帳 戶 填 報 一 份 表 格 ; 但 若
申 索 人 / 計 劃 成 員 不 止 在 一 個 計 劃 開 立 帳 戶 , 則 須 就 每 個 計 劃 填 報 一 份 表 格 。
(4) A scheme member who does not have the month and/or day of birth printed on the HKID card may provide evidence as to the month and/or day by
using one of the following methods for the purpose of withdrawal of MPF accrued benefits on the ground of retirement/early retirement:
(i)
(ii)
using the birth date as shown on an official document (e.g. a travel document or a statutory declaration of the member's date of birth); or
using the day and month of the issue date of the HKID card of the member.
If the member has not used either of the two methods above to provide evidence as to the month and day, then in the absence of any other evidence,
the trustee should, where the HKID card shows only the year and month of birth (and not the day of birth) use the last day of the month, and where the
HKID cared shows only the year of birth (and not the day of birth) use the last day of the month, and where the HKID card shows only the year of birth
(and neither the month nor day of birth), use the last day of the year as shown on the HKID card as the birth date of the scheme member.
Please note that mandatory contributions in respect of the scheme member (if any) will cease on the day when the scheme member reaches age 65
based on the evidence provided by the scheme member of defaulted above.
就 基 於 退 休 / 提 早 退 休 的 理 由 而 提 取 強 積 金 累 算 權 益 而 言 , 如 計 劃 成 員 的 香 港 身 份 證 並 未 印 有 出 生 月 份 及 / 或 日 子 , 則 可 採 用 以 下 其 中 一 種 方 法 , 就
其 出 生 月 份 及 / 或 日 子 提 供 證 據 :
(i)
採 用 某 份 官 方 文 件 ( 例 如 旅 遊 證 件 或 有 關 計 劃 成 員 的 出 生 日 期 的 法 定 聲 明 ) 所 顯 示 的 出 生 日 期 ; 或

如 計 劃 成 員 沒 有 採 用 以 上 任 何 一 種 方 法 就 其 出 生 月 份 及 / 或 日 子 提 供 證 據 , 則 受 託 人 在 沒 有 其 他 證 據 的 情 況 下 , 應 以 計 劃 成 員 的 香 港 身 份 證 所 顯 示 的
出 生 月 份 的 最 後 一 日 ( 如 該 香 港 身 份 證 只 顯 示 出 生 年 份 及 月 份 ), 或 以 計 劃 成 員 的 香 港 身 份 證 所 顯 示 的 出 生 年 份 的 最 後 一 日 ( 如 該 香 港 身 份 證 只 顯 示 出
生 年 份 , 而 沒 有 出 生 月 份 及 日 子 ), 作 為 其 出 生 日 期 。
請 注 意 , 就 計 劃 成 員 作 出 的 強 制 性 供 款 ( 如 有 的 話 ), 將 根 據 計 劃 成 員 提 供 的 證 據 , 或 按 上 述 預 設 的 出 生 日 期 計 算 , 於 計 劃 成 員 年 滿 65 歲 當 日 為 止 。
(5) In processing a claim of payment, the approved trustee of the scheme may request the claimant to produce the original copies of the documents for
checking purpose, if necessary.
處 理 付 款 申 索 時 , 計 劃 的 核 准 受 託 人 可 在 必 要 時 要 求 申 索 人 提 交 文 件 的 正 本 以 核 對 資 料 。
(6) For a claimant/scheme member who does NOT posses a HKID card, a copy of the passport (only pages with personal particulars and passport
number) should be given to the trustee concerned for verification of the passport number if the claimant/scheme member does not wish to present the
passport in person for verification.
如 申 索 人 或 計 劃 成 員 沒 有 香 港 身 份 證 , 而 又 不 擬 親 身 出 示 護 照 以 供 核 對 , 則 須 提 供 護 照 副 本 ( 只 載 有 個 人 資 料 及 護 照 號 碼 之 頁 ) 供 受 託 人 核 對 護 照 號 碼 。
(7) A claimant who is required to make a statutory declaration for a claim shall complete the relevant statutory declaration form and make a statutory
declaration. The signed statutory declaration form shall be attached to Form MPF(S) - W. The statutory declaration must be a valid statutory declaration
in the place where the declaration is made (e.g. in Hong Kong, the statutory declaration should be made before and signed by a Commissioner for
Oaths at a Public Enquiry Service Centre of the Home Affairs Department/ a Notary Public/ Justice of Peace). A statutory declaration made in a place
other than Hong Kong is also acceptable provided that it is made before and signed by a Notary Public or a person authorized under the law of that
place to administer an oath or take a statutory declaration.
須 就 某 項 申 索 作 出 法 定 聲 明 的 申 索 人 , 須 填 報 有 關 聲 明 表 格 及 作 出 法 定 聲 明 。 申 索 人 須 把 簽 妥 的 聲 明 表 格 夾 附 於 第 MPF(S) – W 號 表 格 。 該 法 定 聲 明 必

(8) Except for a claimant who also claims long service payment on grounds of permanent unfitness for his/her present job under the Employment
Ordinance (Cap. 57), a claimant shall ask his/her medical practitioner to fill in Form MPF(S) - W(M) and attach it to Form MPF(S) - W. A medical
practitioner who signs Form MPF(S) - W(M) must be either

(I) a registered medical practitioner who is registered under the Medical Registration Ordinance, (Cap.161), i.e., 根 據 《 醫 生 註 冊 條 例 》( 第 161 章 )
註 冊 的 註 冊 醫 生 , 即 :
(a) a person who is duly registered as a medical practitioner with the Medical Council of Hong Kong, or 在 香 港 醫 務 委 員 會 正 式 註 冊 為 醫 生 的 人
士 ; 或
(b) a person who is deemed to be registered as a medical practitioner under the Medical Registration Ordinance (i.e. persons who are exempted
from registration). 獲 視 作 為 根 據 《 醫 生 註 冊 條 例 》 註 冊 成 為 醫 生 的 人 士 ( 即 獲 豁 免 無 須 註 冊 的 人 士 )。
Or 或
(II) a registered Chinese medicine practitioner, within the meaning assigned to it by section 2 of the Chinese Medicine Ordinance (Cap.549).《 中 醫 藥 條
例 》( 第 549 章 ) 第 2 條 所 界 定 的 註 冊 中 醫 。
For a claimant who also claims long service payment on grounds of permanent unfitness for his/her present job, he/she may use the form "Certificate
of an employee's permanent unfitness for a particular type of work" under the Employment Ordinance used for the purpose to substitute for the Form
MPF(S) - W(M) for the purpose of claiming payment of MPF accrued benefits on grounds of total incapacity.
申 索 人 如 按 《 僱 傭 條 例 》( 第 57 章 ) 以 永 久 不 適 合 擔 任 其 現 時 工 作 為 理 由 同 時 申 索 長 期 服 務 金 , 則 可 按 《 僱 傭 條 例 》 填 寫 「 僱 員 永 久 不 適 合 擔 任 某 類 工 作 的
證 明 書 」, 以 替 代 第 MPF(S)-W(M) 號 表 格 。 該 表 格 是 供 因 完 全 喪 失 行 為 能 力 而 提 出 支 付 強 積 金 累 算 權 益 申 索 的 人 士 填 報 的 。
(9) For a self-employed person or a former self-employed person who claims payment on grounds of total incapacity, there is no need to produce an
employer letter.
基 於 完 全 喪 失 行 為 能 力 的 理 由 而 提 出 累 算 權 益 申 索 的 現 或 前 自 僱 人 士 無 須 提 出 僱 主 證 明 書 。
(10) For a former employee whose last employment has been terminated before total incapacity and who is unable to obtain a letter from the last employer
certifying that that contract of employment for that particular kind of work has been terminated or has been unemployed for more than 7 years, the
claimant must provide the trustee with a statutory declaration stating that contract of employment for the particular kind of work as specified in the
medical certificate has been terminated.
如 前 任 僱 員 在 他 完 全 喪 失 行 為 能 力 之 前 最 後 從 事 的 工 作 終 結 , 令 他 未 能 取 得 最 後 僱 主 發 出 的 信 件 , 證 明 關 於 該 特 定 種 類 工 作 的 僱 傭 合 約 已 終 止 或 他 己 失 業
超 過 7 年 , 則 申 索 人 必 須 向 受 託 人 提 供 法 定 聲 明 , 述 明 醫 生 證 明 書 上 所 指 明 關 於 該 特 定 種 類 工 作 的 僱 傭 合 約 已 終 止 。
(11) The "Entry Permit for Hong Kong and Macau Residents ( 港 澳 居 民 往 來 內 地 通 行 證 ) " is issued at the China Travel Service (Hong Kong) Limited on
behalf of the Public Security Bureau of Guangdong, PRC.
「 港 澳 居 民 往 來 內 地 通 行 證 」 由 香 港 中 國 旅 行 社 代 表 中 國 廣 東 省 公 安 廳 發 出 。
Remarks:
By selecting payment method (iii) Payment into Fidelity Personal Investments Account on page 3, I acknowledge and agree that:
1. by electing this payment option, notwithstanding any provision that may be to the contrary, I hereby expressly consent and authorise, and
request the Trustee or its agent or service provider to disclose and provide my personal data, including, but not limited to, my full name, HK
identity card number/passport number, and address to the Manager or its service provider as may be considered necessary by the Trustee for
the purposes of processing my requests made under this claim form;
2. once the Trustee effects the transfer of my accrued benefits into my Fidelity Account in accordance with my instructions above ("Transfer"), the
relevant accrued benefits should be treated as being paid to me;
3. the receipt of my accrued benefits by my Fidelity Account (i.e. by transferring my accrued benefits directly into the bank account of the Manager
as mentioned above) shall be a complete discharge to the Trustee and exonerate it from all further liability or responsibility in relation to my
participation in the Fidelity Retirement Master Trust and the accrued benefits that are transferred;
4. my Fidelity Account is unrelated to the Trustee. The Trustee takes no part in and shall not in any circumstances be responsible for and does
not have any control over my Fidelity Account and any investment which I may make thereunder. The requested Transfer is not made pursuant
to the Trustee's suggestion, representation or recommendation in any way. The Trustee's agreement to effect the Transfer is not, and must
not to be taken in any way as, an invitation or offer to sell or recommendation or representation by the Trustee of the merits or suitability of the
requested Transfer and any investments which I may make thereunder;
5. any investment that I may make under the Fidelity Account is made at my own risk and I am aware of such risks involved. I understand that if
in doubt, I should seek independent professional advice in relation to the requested Transfer and any investments which I may make under the
Fidelity Account.

(「 計 劃 」) 及 已 轉 移 的 累 算 權 益 , 所 有 進 一 步 的 賠 償 責 任 或 其 他 責 任 ;
4. 本 人 的 富 達 帳 戶 與 受 託 人 沒 有 任 何 關 係 。 受 託 人 沒 有 參 與 及 並 不 應 在 任 何 情 況 下 , 對 本 人 的 富 達 帳 戶 及 在 此 富 達 帳 戶 內 的 投 資 , 負 上 任 何 責 任 及
有 任 何 的 控 制 權 。 此 轉 移 是 沒 有 根 據 受 託 人 以 任 何 方 式 的 建 議 、 陳 述 或 推 薦 而 執 行 。 受 託 人 對 實 行 此 轉 移 的 協 議 下 , 對 本 人 的 富 達 帳 戶 及 在 此
富 達 帳 戶 內 的 投 資 , 不 是 及 不 應 在 任 何 方 式 下 , 被 視 作 受 託 人 的 邀 請 、 給 予 銷 售 、 推 薦 、 可 取 或 合 適 的 表 示 ;
5. 本 人 承 擔 在 富 達 帳 戶 內 的 任 何 投 資 的 所 有 風 險 並 意 識 所 涉 及 的 風 險 。 本 人 明 白 如 對 此 轉 移 及 有 關 在 此 富 達 帳 戶 內 的 投 資 有 任 何 疑 問 , 應 尋 求 獨

(a) From time to time, it is necessary for clients and various other individuals (“data subjects”) to supply the Manager and/or the Trustee with data in
connection with various matters such as account opening or continuations, or provision of services to clients and other individuals.
客 戶 及 其 他 人 士 (「 資 料 當 事 人 」) 在 開 立 或 延 續 帳 戶 , 或 經 理 人 及 / 或 受 託 人 向 客 戶 及 其 他 人 士 提 供 服 務 等 情 況 下 , 必 須 不 時 向 經 理 人 及 / 或 受 託 人 提 供
有 關 資 料 。
(b) Although it is not generally obligatory for a data subject to provide personal data, failure to supply such data may result in the Manager and/or Trustee
being unable to open an account or continue services to clients.
雖 然 資 料 當 事 人 一 般 而 言 並 無 義 務 提 供 個 人 資 料 , 但 若 資 料 當 事 人 未 能 提 供 該 等 資 料 , 可 能 導 致 經 理 人 及 / 或 受 託 人 無 法 開 立 帳 戶 或 繼 續 向 客 戶 提 供
服 務 。
(c) It is also the case that data are collected or received from data subjects from time to time in the ordinary course of the continuation of the Manager
and/or the Trustee’s relationship with them, for example, when clients write cheques, effect transactions, attend seminar/events or generally
communicate verbally or in writing with the Manager and/or the Trustee.
在 持 續 的 正 常 業 務 往 來 中 , 經 理 人 及 / 或 受 託 人 不 時 亦 會 向 資 料 當 事 人 收 集 或 接 收 資 料 , 例 如 當 資 料 當 事 人 簽 發 支 票 、 進 行 交 易 、 出 席 講 座 / 活 動 , 或
與 經 理 人 及 / 或 受 託 人 的 一 般 口 頭 或 書 面 通 訊 。
(d) The purpose for which data relating to a data subject may be used will vary depending on the nature of the data subject’s relationship with the
Manager and/or the Trustee. These purposes may comprise any or all of the following:
資 料 當 事 人 之 資 料 的 用 途 將 視 乎 其 與 經 理 人 及 / 或 受 託 人 的 關 係 性 質 而 有 所 不 同 , 可 能 包 括 下 列 任 何 或 所 有 用 途 :
(i) the processing of an application for an account;

the daily operation of the services provided to clients;

(iii) marketing services and products (please see further details in paragraph (e) below);

(iv) for the purposes of any party having at any time obligations under the relevant Retirement Product in relation to a member participating in such
Retirement Product (e.g. calculating an employer’s long service or severance payment accrued liability);
任 何 人 士 就 成 員 所 參 與 的 相 關 退 休 產 品 隨 時 履 行 任 何 責 任 ( 例 如 計 算 僱 主 應 付 的 長 期 服 務 金 或 遣 散 費 );
(v) meeting disclosure and compliance requirements under any laws or regulatory requirements applicable to the Manager and/or the Trustee or
any of their affiliates in Hong Kong or elsewhere from time to time;
根 據 不 時 適 用 於 經 理 人 及 / 或 受 託 人 或 其 在 香 港 或 海 外 的 任 何 聯 營 公 司 的 任 何 法 例 或 法 規 要 求 , 履 行 資 料 披 露 及 合 規 規 定 ;
(vi) any purpose related to the administration of the relevant Retirement Products or the data subject’s participation therein;
與 處 理 相 關 退 休 產 品 或 資 料 當 事 人 在 其 中 參 與 有 關 行 政 上 任 何 用 途 ;
(vii) with respect to MPF data, researching, designing, and launching MPF-related products and services to mandatory provident fund scheme
members;
有 關 強 積 金 的 資 料 , 以 用 作 研 究 、 設 計 、 和 推 出 與 強 積 金 有 關 的 產 品 及 服 務 , 予 強 制 性 公 積 金 計 劃 成 員 ;
(viii) with respect to MPF data, designing and organising seminars/events/forums to mandatory provident fund scheme members;
有 關 強 積 金 的 資 料 , 以 用 作 規 劃 及 籌 備 講 座 / 活 動 / 論 壇 予 強 制 性 公 積 金 計 劃 成 員 ;
(ix) providing alerts, newsletter with contents relevant to MPF including market information and investment education materials;
提 供 內 容 與 強 積 金 相 關 的 提 示 服 務 、 通 訊 包 括 市 場 資 訊 及 投 資 教 育 資 料 ;
(x)
designing and conducting surveys/questionnaires for client profiling/segmentation, statistical analysis, improving and furthering the provision of
MPF services by Fidelity;
設 計 及 進 行 問 卷 調 查 / 統 計 分 析 , 以 作 客 戶 檔 案 分 析 / 分 類 之 用 ; 改 善 及 擴 展 富 達 提 供 的 強 積 金 服 務 ;
(xi) with respect to non-MPF data, researching, designing, and launching financial, investment, wealth management, securities and insurance
services or related services and products to non-mandatory provident fund scheme members;
有 關 非 強 積 金 的 資 料 , 以 用 作 研 究 、 設 計 、 和 推 出 金 融 、 投 資 、 財 富 管 理 、 證 券 及 保 險 服 務 或 相 關 服 務 和 產 品 , 予 非 強 制 性 公 積 金 計 劃 成 員 ;
(xii) with respect to non-MPF data, designing and organising financial and investment seminars/events/forums to non-mandatory provident fund
scheme members; and
有 關 非 強 積 金 的 資 料 , 以 用 作 規 劃 及 籌 備 金 融 、 投 資 講 座 / 活 動 / 論 壇 予 非 強 制 性 公 積 金 計 劃 成 員 ; 及
(xiii) purposes directly related to the above.

(e) USE OF DATA IN DIRECT MARKETING 資 料 作 直 銷 業 務 推 廣 用 途
The Manager and/or the Trustee intend to use the data subject’s data (as may be collected by the Manager and/or the Trustee) in direct marketing
and the Manager and/or the Trustee requires the data subject’s consent (which includes an indication of no objection) for that purpose. In this
connection, please note that:

(1) MPF-related services and products offered by the Manager and/or the Trustee;

(2) reward, loyalty or privileges programmes, and promotional offers in relation to MPF; and
強 積 金 性 的 獎 勵 、 長 期 客 戶 或 優 惠 計 劃 及 優 惠 推 廣 ; 及
(3) invitations to MPF-related seminars/events/forums.
邀 請 參 與 強 積 金 相 關 的 講 座 / 活 動 / 論 壇 。

Non-MPF member data 非 強 積 金 成 員 資 料
(1) financial, investment, wealth management, securities and insurance services or related services and products;
金 融 、 投 資 、 財 富 管 理 、 證 券 及 保 險 服 務 或 相 關 服 務 和 產 品 ;
(2) Non-MPF related reward, loyalty or privileges programmes, and promotional offers; and
非 強 積 金 性 的 獎 勵 、 長 期 客 戶 或 優 惠 計 劃 及 優 惠 推 廣 ; 及
(3) invitations to financial and investment seminars/events/forums.
邀 請 參 與 金 融 、 投 資 講 座 / 活 動 / 論 壇 。
(f) Data collected may be maintained for such period as may be required by law or as otherwise prudent in relation to administration of the relevant
Retirement Products and may be retained after the data subject ceases to be a client or have a beneficial interest in the relevant Retirement Products.
所 收 集 的 資 料 可 於 法 例 規 定 或 審 慎 管 理 相 關 退 休 產 品 所 須 的 時 限 內 儲 存 , 並 可 於 資 料 當 事 人 不 再 為 客 戶 或 不 再 於 相 關 退 休 產 品 中 擁 有 實 益 權 益 後 繼 續

(g) Data held by the Manager and/or the Trustee relating to a data subject will be kept confidential but the Manager and/or the Trustee may provide such
information to the following parties whether inside or outside Hong Kong for the purposes set out in paragraph (d):

(i) the Manager or the Trustee (as the case may be), the ultimate holding company of the Manager and/or the Trustee and/or their subsidiaries and/
or affiliates;

受 委 任 就 強 制 性 公 積 金 計 劃 成 員 的 資 料 以 設 計 、 研 究 、 推 出 或 宣 傳 經 理 人 及 / 或 受 託 人 與 強 積 金 有 關 的 產 品 或 服 務 ;
(iv) persons appointed to design, research, launch or promote the products or services of the Manager and/or the Trustee for data relating to nonmandatory
provident fund scheme members;
受 委 任 就 非 強 制 性 公 積 金 計 劃 成 員 的 資 料 以 設 計 、 研 究 、 推 出 或 宣 傳 經 理 人 及 / 或 受 託 人 的 產 品 或 服 務 ;
(v) the employees, officers, directors and agents of the Manager, the Trustee or any of the parties in (i) to (iii) above;

(vi) the employer (or former employer) of any member participating in a relevant Retirement Product where permitted or required by law;

(vii) any third party service provider employed to provide administrative, computer, telecommunications, printing, letter-shopping, mailing or other
services to the Manager and/or the Trustee in connection with the operation of its business;

(viii) external service providers (including but not limited to printing houses, mailing houses, telecommunication companies, public relation companies,
advertising agency, telemarketing companies, data processing companies, storage companies, call centres, market research firms, and
information technology companies that the Manager and/or the Trustee engages for the purposes set out in paragraph (e); and

(ix) any party (including, but not limited to, any regulatory authorities, governmental authorities, tax, law enforcement authorities) entitled thereto by
law or regulation (whether statutory or not).

(h) Under the Personal Data (Privacy) Ordinance, any individual has the right:
根 據 個 人 資 料 ( 私 隱 ) 條 例 , 任 何 人 士 均 有 權 :
(i) to check whether the Manager and/or the Trustee holds data about him/her and of access to such data;
查 核 經 理 人 及 / 或 受 託 人 是 否 持 有 其 資 料 及 查 閱 該 等 資 料 ;
(ii) to require the Manager and/or the Trustee to correct any data relating to him/her which are inaccurate;
要 求 經 理 人 及 / 或 受 託 人 更 正 任 何 有 關 該 名 人 士 的 不 正 確 資 料 ;
(iii) to ascertain the Manager and/or the Trustee’s policies and practices in relation to data and to be informed of the kind of personal data held by
the Manager or the Trustee; and
確 定 經 理 人 及 / 或 受 託 人 有 關 資 料 的 政 策 和 慣 例 , 以 及 獲 通 知 經 理 人 及 / 或 受 託 人 持 有 個 人 資 料 的 類 別 ; 及
(iv) to object to the use and/or provision of his/her personal data for marketing purposes; and neither the Manager nor the Trustee will use his/her
personal data for these purposes if he/she communicates his/her objection to the Manager or the Trustee (as the case may be).

(i) In accordance with the terms of the Personal Data (Privacy) Ordinance, the Manager and/or the Trustee has the right to charge a reasonable fee for
the processing of any data access request.
根 據 個 人 資 料 ( 私 隱 ) 條 例 的 條 款 , 經 理 人 及 / 或 受 託 人 有 權 就 處 理 任 何 查 閱 資 料 的 要 求 徵 收 合 理 費 用 。

your personal data for direct marketing purposes or if you would like to make a request for access to data or correction of data or for information
