MPH Friends of the Arts<br />
STUDENT GRANTS – 2011 / 2012<br />
Faculty-sponsored requests from upper school students are invited that align with the FOA’s goal of making<br />
funds available for student enrichment in all the arts at MPH. A limited number of requests of up to $100 will<br />
be considered that meet the following criteria. Questions may be addressed to the FOA’s grant coordinator,<br />
Suzette Melendez, at suzette@twcny.rr.com.<br />
Deadline: Request must be received before January 30, 2012<br />
Criteria<br />
• Requests must be for art-related activities that support students’ academic requirements and cannot<br />
be funded out of the MPH budget.<br />
• Requests must be submitted through a faculty sponsor who agrees to serve as the preceptor for the<br />
student project.<br />
• Requests must have parent sign-off and be approved in writing by the head of the upper school.<br />
• Priority will be given to proposals that benefit a significant number of students and/or bring continued<br />
value to students over time.<br />
<strong>Application</strong> Process<br />
• Faculty member identifies a student project eligible for FOA funding.<br />
• <strong>Student</strong> prepares the following one-page application for review/sign-off by their faculty sponsor,<br />
parent(s), and upper school head.<br />
• <strong>Student</strong> attends FOA meeting (as feasible) to present their project idea.<br />
• Requests are considered on a first come; first serve basis. Funding decisions are based on the criteria<br />
above, the completeness and quality of proposals, and the appropriateness of the proposed budget.<br />
Applicants should not presume that their application will/can be funded.<br />
• <strong>Student</strong>s with funded projects present the results of their project at an FOA meeting.

1. Please describe the program or activity for which funds are requested, how it supports your academic<br />
requirements and goals, and how it meets the criteria for funding.<br />
2. How will, and approximately how many students will be directly affected by/benefit from this program or activity<br />
either as part of the activity itself or as the result of the activity being continued beyond the current year What<br />
grade levels will benefit and how<br />
3. Please provide a budget that outlines the expenses associated with the project, the total cost, the amount being<br />
requested from FOA (maximum of $100), and how additional funds will be raised if the total cost is greater than<br />
what is being asked of the FOA.<br />
