Under the terms of ferpa, columbus state<br />

the following as directory<br />

information that may be released to<br />

those requesting it unless the<br />

student specifically requests<br />

otherwise by submitting written<br />

notification to records and<br />

registration:<br />

• Name<br />

• Address (home/present)<br />

• Telephone number (home)<br />

• Program of study/technology<br />

• Participation in officially recognized<br />

activities and sports<br />

• Weight and height of members of<br />

athletic teams<br />

• Enrollment status (less than half<br />

time, half-time, part-time, full-time,<br />

over full-time, inclusive dates and<br />

quarters/semesters of enrollment)<br />

• Degrees, certificates, transfer<br />

module and awards received<br />

(including deans’s list and academic<br />

honors list)<br />

• Most recent previous educational<br />

agency or institution attended<br />

The following information may not<br />

be released to anyone without<br />

written consent of the student:<br />

• Grades<br />

• Schedules<br />

• Ethnicity<br />

• Social Security Numbers<br />

• Cougar ID Numbers<br />

• Student Identification Numbers<br />

• E-mail Addresses<br />
