INTRODUCTION<br />

Respiratory syncytial virus (RSV) is a common cause of lower respiratory tract disease and<br />

hospitalization <strong>in</strong> <strong>in</strong>fants and young <strong>children</strong> (1-4), and therefore a major cause of healthcare<br />

utilization (5).<br />

Severe RSV bronchiolitis has been associated with later development of abnormal pulmonary<br />

<strong>function</strong>, wheez<strong>in</strong>g, asthma and allergic sensitization (6-11), but it is unclear whether severe<br />

RSV bronchiolitis causes wheez<strong>in</strong>g, or genetic predisposition or other environmental risk<br />

factors <strong>in</strong>crease the propensity to such exaggerated response to RSV.<br />

The aim of this study was to compare the long-term outcome of asthma, allergy and<br />

pulmonary <strong>function</strong> <strong>in</strong> monozygotic (MZ) tw<strong>in</strong> pairs discordant for hospitalization with<br />

verified RSV bronchiolitis <strong>in</strong> <strong>in</strong>fancy as a surrogate marker of the RSV disease severity. Any<br />

differential long-term effect from RSV disease severity <strong>in</strong> these genetically identical tw<strong>in</strong>s<br />

would suggest a causal role of RSV.<br />

The study was approved by the ethics committee of Copenhagen (KA-20060022) and by the<br />

Danish Data Protection Agency (J.nr. 2005-41-5163 and J.nr. 2005-2311-0121). The <strong>children</strong><br />

were enrolled after written consent was obta<strong>in</strong>ed from the parents or guardians.<br />

Registries used for recruitment of the study population<br />

The Danish Civil Registration System registers every Danish citizen by a unique personal<br />

identification number, provid<strong>in</strong>g a key for l<strong>in</strong>k<strong>in</strong>g register <strong>in</strong>formation (12).<br />

The Danish Tw<strong>in</strong> Registry conta<strong>in</strong>s complete ascerta<strong>in</strong>ment of all live-born tw<strong>in</strong>s s<strong>in</strong>ce 1968.<br />

Zygosity <strong>in</strong>formation <strong>in</strong> the registry is determ<strong>in</strong>ed from questions on similarity and mistaken<br />
