APPLICATION FOR A DERIVATIVE RESIDENCE CARD - FORM DRF1<br />

If section 3 has been completed: evidence that <strong>the</strong> person for whom you are <strong>the</strong> primary<br />

carer is a British citizen, such as a valid British passport, registration document, nationality<br />

status letter, naturalisation certificate or a certificate of entitlement to <strong>the</strong> right of abode in<br />

<strong>the</strong> <strong>UK</strong>. Evidence that <strong>the</strong> British citizen is dependent on you, such as court orders, care<br />

responsibilities, medical evidence.<br />

If section 4 or 5 has been completed: Evidence that <strong>the</strong> EEA national parent exercised<br />

Treaty rights in <strong>the</strong> <strong>UK</strong> as a worker. This can include contracts of employment, letter(s) from<br />

<strong>the</strong> EEA national’s <strong>form</strong>er employer(s) confirming <strong>the</strong>ir employment, wage slips featuring<br />

a full National Insurance Number. Evidence that <strong>the</strong> EEA national was residing in <strong>the</strong> <strong>UK</strong><br />

at a time when <strong>the</strong> relevant child was in education in <strong>the</strong> <strong>UK</strong>. Evidence of attendance at<br />

an educational establishment in <strong>the</strong> <strong>UK</strong> for <strong>the</strong> relevant child, including evidence that <strong>the</strong>y<br />

are currently in education in <strong>the</strong> <strong>UK</strong>. Evidence that <strong>the</strong> child was in <strong>the</strong> <strong>UK</strong> whilst <strong>the</strong> EEA<br />

national was a worker.<br />

If section 6 has been completed as <strong>the</strong> dependant of <strong>the</strong> primary carer of a selfsufficient<br />

EEA national child: Evidence of your relationship to your primary carer such as<br />

a birth certificate or adoption certificate. Evidence of <strong>the</strong> relationship of your primary carer<br />

to <strong>the</strong> relevant EEA national child. Evidence as relevant for Section 2 as listed above.<br />

If section 6 has been completed as <strong>the</strong> dependant of <strong>the</strong> primary carer of a British<br />

citizen: Evidence of your relationship to your primary carer such as birth certificate or<br />

adoption certificate. Evidence of <strong>the</strong> relationship of your primary carer to <strong>the</strong> relevant British<br />

citizen. Evidence of British nationality of <strong>the</strong> relevant child. Evidence of dependency of <strong>the</strong><br />

British citizen on your primary carer, for example court orders, care responsibilities.<br />

If section 6 has been completed as <strong>the</strong> dependant of <strong>the</strong> primary carer of a child of<br />

an EEA national worker or <strong>form</strong>er worker where <strong>the</strong> relevant child is in education in<br />

<strong>the</strong> <strong>UK</strong>: Evidence of your relationship to your primary carer, for example birth certificate or<br />

adoption certificate. Evidence of <strong>the</strong> relationship of your primary carer to <strong>the</strong> relevant child.<br />

Evidence that <strong>the</strong> EEA national parent exercised Treaty rights in <strong>the</strong> <strong>UK</strong> as a worker. This<br />
