Dear <strong>St</strong>. Mary’s Member,<br />

Thank you for choosing a <strong>Humana</strong> Health Plan. We have put together some information which will be<br />

helpful as you begin accessing your new benefits in January 2011.<br />

ID Cards:<br />

Very soon you will receive your <strong>Humana</strong> ID cards in the mail. Your member ID card is proof that you are<br />

a <strong>Humana</strong> member. It contains important information about your plan. Keep your card with you at all<br />

times and show it to your provider each time you receive services. If you do not receive your card by<br />

January 3, please call 1-800-4HUMANA, and one of our Customer Care Specialists can help you.<br />

Plan A – Personal Care Account Members, please note that the attachment that will accompany your<br />

insurance card incorrectly states that your insurance card is to be used as your Flexible Spending<br />

Account (FSA) card, as well as your Personal Care Account (PCA) card and that you can use your PCA<br />

allowance/funds towards prescriptions. Please note the PCA card can only be used for costs associated<br />

with medical care – this does not include prescriptions. If you elected to participate in a Healthcare FSA,<br />

you will receive a card from ADP to be used specifically for items eligible for the Healthcare FSA, such as<br />

prescription co-pays.<br />

Understanding Your Plan:<br />

<strong>St</strong>. Mary’s and <strong>Humana</strong> have been working together over the past several months to offer you the plans<br />

you need to meet the healthcare coverage needs of you and your family. Enclosed with this letter,<br />

please find an overview of each of the plans offered to you as a <strong>St</strong>. Mary’s Employee. This should be<br />

used as a quick reference guide to help you understand your benefits.<br />

During your Open Enrollment period, a commitment was made by <strong>St</strong>. Mary’s and <strong>Humana</strong> to ensure you<br />

have adequate access to physician services in the Tier 1 network. In order to offer you the best benefit<br />

and ensure you and your family have easy access to physicians, <strong>St</strong>. Mary’s has enhanced the Tier 2<br />

Network physician benefit so that it pays the same as the Tier 1 Network benefit. Basically, as <strong>St</strong>. Mary’s<br />

members, both you and your covered dependents will receive the Tier 1 benefit whether you choose to<br />

see a Tier 1 or Tier 2 physician. This Tier 1 coverage applies to office visits, inpatient service and<br />

outpatient services. However, please note that if you choose to see a Tier 2 physician, in-office<br />

procedures, including lab and x-rays provided in the doctor’s office, will only be covered with the Tier<br />

2 network benefit.<br />

Finding a Provider in your Network:


It’s easy to find a provider who is in your network and close to home. <strong>St</strong>. Mary’s has its own <strong>Humana</strong><br />

web page. Beginning January 2011, just go to stmarys.humana.com to find links to <strong>St</strong>. Mary’s Tier 1<br />

network, your Tier 2 network (which is the <strong>Humana</strong>/ChoiceCare network) and lots of other helpful<br />

information about benefits and services available to you and your family. If you are unable to access<br />

information online, you can always call <strong>Humana</strong>’s Customer Care Specialists for assistance at 1-800-<br />

4HUMANA beginning January 2011.
