Tranquility Entrance and follow the signs<br />

to the Eye Clinic and <strong>Minor</strong> <strong>Surgery</strong><br />

Registration Desk.<br />

<strong>Minor</strong> <strong>Surgery</strong> - <strong>Patient</strong> <strong>Instructions</strong><br />

FOR YOUR VISIT<br />

1. On the day of your surgery there is<br />

no food restrictions,you can eat<br />

normally.<br />

2. Wear comfortable clothing that can<br />

be removed easily.<br />

3. If you do not speak English,<br />

please bring an adult with you who<br />

is able to translate in your<br />

language.<br />

4. You will receive a local anaesthetic<br />

or “freezing” in the area where<br />

your procedure will be performed.<br />

5. With a few exceptions, most<br />

patients may leave shortly after<br />

their surgery.<br />

6. Please report to the Registration<br />

Desk for <strong>Minor</strong> <strong>Surgery</strong> and Eye<br />

Clinic, 15 to 30 minutes before<br />

your appointment.<br />

7. The hospital is not responsible for<br />

any loss or damage of personal<br />

items or clothing.<br />

8. Late arrivals may have their<br />

procedure cancelled.<br />

9. Please notify your surgeon’s office<br />

48 hours in advance if you wish to<br />

change or cancel an appointment.<br />

Please note:<br />

<strong>Surgery</strong> time may be subject to<br />

change.<br />

After Your Procedure:<br />

Your doctor will give instructions to you.<br />

Please bring the following<br />

information with you on the<br />

day of your surgery:<br />

• Your hospital Blue Card. You will<br />

be asked to check your card to<br />

ensure all of the information is<br />

accurate and the spelling is correct.<br />

• Your Ontario <strong>Health</strong> Card<br />

• Your date of birth<br />

• Your family/referring doctor’s name<br />

and address<br />

PLEASE BE AWARE THAT:<br />

SJHC is a Smoke-Free facility.<br />

SMOKING IS<br />

NOT PERMITTED<br />

on hospital<br />

property.<br />

The use of cellular phones is<br />

PERMITTED within the<br />

hospital in designated areas.<br />

There are signs<br />

that indicate the<br />

areas where the<br />

use of wireless<br />

devices is NOT<br />

PERMITTED.
