• Application Forms must be completed in blue or black ink, and in print (not cursive). Before being copied, the application form must be signed on page 4 (please find application form instructions below). 3. Proof of residence in Miami Consulate´s jurisdiction (FL, GA or SC): Original and photocopy of College/University ID and of U.S. Driver’s License with current address. 4. 2 color passport sized photos: Passport photos can be taken at any Kinko’s, Walgreens, CVS, etc. Photos must be RECENT with white background and professionally taken. Photos must be GLUED to each application form in the top right corner. DO NOT STAPLE PHOTOS TO APPLICATION FORMS! 5. Letter of acceptance as a full time student from Spain's University/School or US program: You MUST contact Lisa Anthony at lanthony@syr.edu for these letters, at least two weeks prior to your visa appointment, so they may be sent to you. 6. Information about the study abroad program. PLEASE contact Lisa Anthony for this information. 7. Proof of Health Insurance: Provide Original Letter guaranteeing full International Health insurance at a minimum of €30,000 Euros, including the necessary Repatriation Coverage for the total period of stay in Spain. Please contact Lisa Anthony, at least two weeks prior to your visa appointment, so SU Abroad can provide you with this letter. 8. Proof of financial means during your stay: SU Abroad will provide the following: • Letter from the University or School in Spain or in the USA assuming full financial responsibility during your stay. Please contact Lisa Anthony for this letter, at least two weeks prior to your visa appointment, so they may be sent to you. 9. Proof of accommodation in the form of: • Letter from the study-abroad program specifying the address where the student will be staying. Please contact Lisa Anthony for this information. 10. US Postal Service Money Order payable to the “Consulate General of Spain” for $160.00 (US citizens) or $78.60 (most non-US citizens). For nationals of Russia, Macedonia, (ARYM), Montenegro, Serbia, Albania and Moldavia the fee is $45.85. Do not submit cash, personal checks, or bank checks because the Consulate will not accept them. These can be purchased at any US Post Office. 11. Pre-paid “US Express Mail” Envelope from the US Postal Service*, Self-Addressed with $19.95 in stamps. Once the visa is affixed to your passport (visa processing time takes about 6 weeks), you can either go pick it up in person or choose this option, where the Spanish Consulate will mail your passport back to you using this envelope. In the addressee section of your mailing label, write the address where you expect to be over the summer. In the “Sender” portion of the label, please write: SU Abroad 106 Walnut Place Syracuse, NY 13244 *Please, do not send any other type of envelope (e.g. US Priority, DHL, UPS, FedEx, etc.) as the Spanish Consulate will not accept it. *Envelopes without a tracking number will not be accepted ************************************************************
Academic Year Students (180+ days) ONLY: Medical Certificate (original and photocopy): Doctor’s recent statement (no older than 3 months) in doctor’s letterhead or with medical center’s stamp, stating: “This health certificate verifies that Mr./Ms ...... is free of drug addiction, mental illness and does not suffer from any disease that could cause serious repercussions to public health according to the specifications of the international sanitary regulation of 2005. These contagious diseases include, but are not limited to smallpox, poliomyelitis by wild poliovirus, the human influenza caused by a new subtype of virus and the severe acute respiratory syndrome (SARS), cholera, pneumonic plague, yellow fever, viral hemorrhagic fevers (e.g.: Ebola, Lassa, Marburg), West Nile Virus and other illnesses of special importance nationally or regionally (e.g.: Dengue Fever, Rift Valley Fever and meningococcal disease.) Ms./Mr. ______________ is a very healthy individual in all senses, she/he has no pre-existing medical conditions, and she/he is capable of traveling abroad.”. * The medical certificate must be signed by a M.D. *The original must be translated into Spanish and submitted along with a photocopy, contact a legal translator. 12. Certification of Absence of Police Records (original and photocopy): FBI or State Police Criminal Background Check legalized with the “Apostille of the Hague Convention” from the U.S. A police record must be obtained (criminal background check) from all non EU countries where you have lived for the past five years for more than 6 months, legalized by the Spanish Consulate or Embassy in the respective countries. If the Certificate is issued outside the USA it must be duly legalized by the corresponding Consulate of Spain. LOCAL POLICE BACKGROUND CHECK WILL NOT BE ACCEPTED. Original translated into Spanish and one copy will be needed (contact a legal translator). *The original must be translated into Spanish and submitted along with a photocopy, contact a legal translator. *FBI background checks can take 8-12 weeks to get back from the FBI. ************************************************************************************************** As visa requirements can change, please check the Consulate’s website prior to your visa appointment to ensure you have the proper required documentation.
